Introduction

• The steps in the nursing process are used to achieve desired goals or outcomes that are identified for your patient

• The nursing process allows the nurse to deliver care in a systematic way

• There are five steps in the nursing process: assessment, diagnosis, planning, implementation, and evaluation

Assessment

• The first step is collecting data related to clients, their problems, and their needs

• Data collection can be done through interviewing, physical assessment, chart review, and diagnostic testing

Diagnosis

• Analysis and interpretation of data• NANDA Nursing Diagnosis

Planning

• This step involves prioritizing needs• Identifying goals• Selecting solutions

Implementation

• Putting the plan into action• What the nurse does• Key words: maintain, position, assess, evaluate

Evaluation

• Assessing the effectiveness of the interventions or actions• Changing the plan as needed• The goals are what the client will do, not the nurse
