• HCA Education and Research (HCA E&R) is a 501(c)(3) not-for-profit charitable organization whose mission is “to foster and strengthen education and research regarding home health and related support services.” HCA E&R is an affiliate of the Home Care Association of New York State, promoting “the quality and accessibility of health care and support at home.”

• Selfhelp Community Services is a comprehensive community service organization providing home care, community based services and senior housing. Selfhelp also provides a nationally recognized and specialized program of care and support at home for survivors of the Holocaust. Selfhelp is the pioneer and operator of the Virtual Senior Center.

Mother Cabrini Health Foundation Grant to HCA

• A COVID-19 Emergency Grant has been awarded by the Mother Cabrini Health Foundation to HCA E&R to address isolation and its consequences for the homebound elderly and medically vulnerable by providing statewide availability of VSC services through home care, built upon the Selfhelp VSC platform and model.

Underlying Need

• There are extreme medical, social and life risks and consequences of isolation in the homebound elderly and disabled, which have added urgency under COVID-19.

• During the COVID-19 surge, 86% of NYS home care agencies surveyed in a 300 agency sample by HCA cited the imperative of patient isolation.

Grant Components

• Statewide outreach, education and work with providers and affiliate partners to assist in VSC availability for their patients.

• Expansion of the Selfhelp VSC technological platform for robust statewide participation.

• VSC technical support.

• Seed funding for program development.

Selfhelp Partnership Under Grant

• The program builds upon the Selfhelp VSC model and platform, and incorporates Selfhelp as a grant partner.

• Selfhelp is the VSC pioneer and operator of the VSC platform. Selfhelp is a home and community-based services agency, and an organizational member of HCA.

• The VSC platform will enable home care providers to participate and enroll their patients directly in VSC, or through “regional affiliates” who could act as hubs for providers in the region.

• Potential “affiliates” could be local Area Agencies on Aging (AAAs), home care providers and/or managed care plans.

• Overall goal: creating the availability of, and offering, the VSC service to all home care providers (and their partners) for patients statewide.

• Beneficially impact the social and clinical determinants of health, and the overall health care system, in multiple ways:

Ø Support seniors and medically vulnerable individuals isolated at home, providing a means of crucial social interaction and engagement, virtual health care oversight and safety.

Ø Provide a mechanism for patient and caregiver education and self-care/self-management tools.

Ø Reduce the risk of depression; help detect changes in mental status which signal other high-risk medical and mental health conditions requiring intervention; and reduce risk of hospitalizations, emergency episodes and injury/ deterioration due to consequences of isolation.

Ø Create a new service in the infrastructure for the elderly, disabled, and for broad system goals of health promotion and prevention, safety, quality of care, quality of life, and self-care/self-management.

Ø With this new part of the infrastructure, also promote system goals of improved population health and health care cost reduction.
