The Aesthetic Science Institute, LLC. has graduated professionals in upstate NY, and beyond. Many of our students have opened their own spas and skin

care clinics, while others have flourished in spas and medical settings, health facilities, sales and marketing; and training and education.

A specialized license in hair removal allows a technician to perform waxing services to remove unwanted hair from the face and body for “aesthetic” purposes or medical reasons.

The Nail Specialty License is a perfect addition to an esthetic or waxing license; as well as providing for a lucrative career on its’ own. Nail Specialist perform natural and artificial nail care services on the hands and the feet. Licensed Nail Specialists are in high demand in specialty nail salons, salons, spas and even in medical offices.

It has always been my goal to provide an educational environment for estheticians to have an institution in which they could learn the basics, perfect the skills, embrace the opportunities and surpass even their own expectations of what a skin care professional can be.

The last several years have allowed me to adapt, perfect and grow into a place that can deliver those goals. Years of experience as a global educator and speaker have been integrated into the details of the

Governing Body & Faculty

The faculty and staff at The Aesthetic Science Institute, LLC. are committed to building an instructional framework of growth, success and opportunity for all of our students. Years of experience in the industry as instructors, practitioners, business owners and leaders have allowed the team to create an exceptional educational environment to prepare students for the profession.

Michelle D’Allaird-Brenner Michelle is a NY State licensed esthetician and International CIDESCO Diplomat. She is a consultant and educator for international cosmetic companies around the world providing education and training in over thirty countries worldwide. Michelle is a contributing author to major industry trade magazines, as well as a host and speaker for International Esthetics conferences in Miami, Philadelphia, Dallas and Long Beach, CA. She is also a co-author of Salon Fundamentals esthetics textbook and Milady’s Standard Makeup textbook. Michelle’s experience lies in education and curriculum development for advanced esthetic, medical esthetics and laser courses. She is also a Chair Person of the NYS Appearance Enhancement Advisory Board.

Carlee Clark (Director) Carlee is a graduate of The Aesthetic Science Institute, LLC., and has been a NYS licensed esthetician since 2008. Carlee brings 15 years of Sales and Marketing experience to the team. Having a strong passion for make-up application, she is certified in eyelash extension application, as well as airbrush make-up and continues to provide services for weddings in the Capital District.

Lisa Wangler (Client and Clinic Administrator) Lisa is a licensed esthetician with over twenty years in corporate project management. She oversees retail and client operations as well as student/client clinic protocols and services. Andrea Gregaydis ( Lead Instructor) Andrea is a graduate of the Aesthetic Science Institute, a CIDESCO Diplomat and Lead Instructor. She has been a practicing Esthetician and Nail Technician since 2007 and has worked for several spas in the Saratoga/North Greenbush area. Andrea teaches Advanced Esthetics, Esthetic and Nail Specialty classes, as well as a variety of continuing education classes. Heather Guy Heather is a licensed esthetician and licensed instructor with years of experience in retail management and customer service. Her expertise allows her to implement life experience into the classroom setting..

Christina Armstrong Christina is a licensed esthetician and NYS Instructor. Christina has years of both practical and instructional experience and teaches both core curriculum as well as advanced education classes and waxing.

Sugey Morales Sugey is a licensed Nail Technician and Cosmetologist with over twenty years experience in the beauty profession. Her Master training is integrated into a detailed and creative Nail Specialty curriculum at AESTHETIC SCIENCE INSTITUTE.

Aesthetic Science Institute The Aesthetic Science Institute, LLC. opened in 2003, and was expanded in 2005 to a 5000 square foot facility. In 2017 the Institute was once again relocated to a brand new building, occupying 8000 sq feet. This location features three classrooms, one esthetics

clinic with fourteen facial stations, a full body treatment room, waxing clinic, make-up studio, student lounge, employee lounge, laundry room and four bathrooms.

The Latham location is a stand alone three story building. It occupies the top two floors of the building with ample parking and easy access from all major highways.

This includes practical time in the public clinic in which students perform services.

This program includes a designated time dedicated to business practices, development and professional expectations. Students are evaluated by taking a written examination and/or a practical examination as well as having a minimum number of hours before performing clinic duties. Written examinations are given at the completion of each chapter, as well as mid term and a final. Practical examinations are given for each practical application taught in the course.

1200 Hour Advanced Esthetics Description

One of only ten schools in the United States and less than 200 around the world.

This elite curriculum is approved by the NYS Department of Education and Comite International d’Esthetique et de Cosmetologie. Upon completion of this program graduates are eligible for licensure as a NYS Esthetician and as a NYS Nail Specialist and eligible to sit for the international CIDESCO examination. After successful passing of the international exam and 600 hours work experience graduates will be awarded the prestigious International CIDESCO Certificate. This elite program is not designed for everyone, but for those seeking to be amongst the most elite beauty therapists in the world. Requirements far exceed those mandated by state licensure, but set recipients apart as the top ten percent of beauty therapists around the globe that are trained on a holistic, full body approach to health, beauty and wellness.

Instructional staff includes NYS Licensed instructors, licensed estheticians, licensed nail technicians, licensed massage therapists and CIDESCO Diplomat. The 1200 Hour Advanced Esthetics Curriculum include instruction in the fundamental core topics of esthetics as well as the most advanced topics in treatments, technology, ingredients and equipment. The program at The Aesthetic Science Institute, LLC. exceeds the NYS mandated hours by 50 hours. The 300 hour program provides enhanced practical application and preparation time for state board as well as entering the workplace. This increase in hours also allows the program to be eligible for title IV funding for those students who qualify.

This program includes a designated time dedicated to business practices, development and professional expectations.

Students are evaluated by taking a written examination and/or a practical examination as well as having a minimum number of hours before performing clinic duties. Written examinations are given at the completion of each chapter, as well as midterm and a final. Practical examinations are given for each practical application taught in the course.

Waxing Program Objectives The objectives of the Waxing Curriculum is to provide the student with complete knowledge, understanding and practical experience necessary to pass the New York State Board Exam. Graduates will be prepared to go forth as a licensed professional hair removal technician possessing the ability to perform a wide variety of hair removal techniques.

The Aesthetic Science Institute, LLC. Tuition Refund Terms

Tuition Refund A student, or legal guardian, who cancels within 7 days of signing the enrollment agreement but before instruction begins, or not accepted for enrollment shall receive all monies returned with the exception of the non-refundable enrollment fee. Thereafter, a student will be liable for

1. the non-refundable enrollment fee plus 2. the cost of the kit accepted, 3. tuition liability as of the student's last date of

physical attendance.

Any monies due a refund shall be made within 45 days of cancellation or withdrawal. The official cancellations or withdrawal date will be determined by the postmark on written notification, or the date said notification is delivered to the school administrator in person.

In the event that a student on an approved leave of absence does not return or is expelled, the date of withdrawal shall be the earlier of the date of expiration of the leave of absence or the date the student notifies the institution that the student will not be returning.

Any monies due a student who unofficially withdrawals shall be refunded within 45 days of a determination by the institution that the student has withdrawn without notifying the institution, this shall occur no more than 30 days from the last day of attendance.

Unofficial withdrawls are determined by the last date of attendance which is monitored by daily attendance. Refund calculations are based upon scheduled, not actual, hours. Liability is calculated as of the student’s last day of attendance.

The school may keep all fees for non-refundable enrollment and kits accepted. Once the kit and books are received the student is responsible for the total amount due. If for any reason the student is terminated or withdraws from the program, they can return only their textbook or workbooks that are in brand new condition

within 20 days of their withdrawal date. The kit is a sanitary item and once it is opened it cannot be returned for any reason. Per the terms listed above the student will be charged full price for kits and any unreturnable books upon termination or withdrawal from this program. Therefore the unreturnable items will also be excluded from the R2T4 step 5 prorated cost of attendance.

Students who withdraw or terminate prior to course completion are charged a cancellation or administrative fee of $150.00. This refund policy applies to tuition and fees charged in the enrollment agreement. Other miscellaneous charges the student may have incurred at the institution (EG: extra kit materials, books, products, unreturned school property, etc.) will be calculated separately at the time of withdrawal. All fees are identified in the catalog and enrollment agreement.

Refund Policy applies to all terminations, by either party, including student decision, course or program cancellation, non-acceptance or school closure.

Tuition liability is divided by the number of quarters in the program. Total tuition liability is limited to the quarter during which the student withdrew or was terminated, and any previous quarters completed.

Program or Course Cancellation Policy If a program or course is cancelled subsequent to a student’s enrollment, and before instruction in the program or course has begun, the school shall at its option: Provide a FULL REFUND of all monies paid.

School Closure Policy If the school closes permanently and ceases to offer instruction after students have enrolled, or if a program is canceled after students have enrolled and instruction has begun, The Aesthetic Science Institute, LLC. will make arrangements for students in which the arrangements for the teach-out is to be per-formed, by an institution in the same geographic area as the original school and will provide the program or course; or shall receive a pro-rated refund of tuition. A list of all enrolled students at the time of closure, including the amount of pro- stated refund, will be submitted to the accrediting agency.

The Aesthetic Science Institute, LLC. Policies Enrollment Policy and Regulations Enrollment dates are based on completion of a required number of hours. Therefore, they vary on actual starting day within each designated enrollment month. Potential students must hold a valid high school diploma or GED and be 17-years of age or older upon entrance into The Aesthetic Science Institute, LLC.. A prospective student must display a genuine desire to become part of a learning atmosphere with the ability and aptitude to succeed in the classroom.

Previously Earned Credits

The Aesthetic Science Institute, LLC. does not accept clock hours or transfer credits from other institutions in any programs except for the Advanced Esthetics. Six hundred hours with a valid state esthetics license is acceptable for transfer.

Re-Entry Policy

In the event that a student is dismissed or willingly discontinues a program at The Aesthetic Science Institute, LLC. they must re-apply, as a new student starting at the beginning of the program. Previous hours earned and tuition paid will not be applied.

Student Conduct and Dismissal The Aesthetic Science Institute, LLC. has a very high level of expectations for its students and instructors. As a representation of the profession that we teach, it is imperative that professional standards and expectations be set and maintained upon entrance into the educational environment.

The Aesthetic Science Institute, LLC. has a set policy for all students that must be read and signed prior to enrollment. In the event a student is sent home due to a policy issue a written notice will be placed in their file. After three notices a student will be dismissed form the program. Failure to comply with THE AESTHETIC SCIENCE INSTITUTE, LLC. policies, procedures, attendance and academic progress expectations can result in program termination. Students who withdraw or terminate prior to course completion are charged a cancellation or administrative fee of $150.00.

Dress Code Your presentation is the first impression that a future client or employer will have of you. Students are expected to always present a crisp, professional image. Proper dress code is expected upon arrival and entry into the building. • Uniform is required attire during school hours. • Long hair must be pulled back in a tight knot. • Artificial nails and nail polish are not permitted. • Facial piercing of a stud .25mm or smaller only. • White tops may be worn under the uniform. • White shoes are worn with uniform • Arrive at school in uniform You represent the school when wearing your uniform; please remember that when out in public. Uniform must be covered if outside smoking or otherwise behaving in an unprofessional manner.

Attendance Your attendance to class is as important as your attendance to work. Students are expected to arrive prior to class time starting. In the event that a student will be absent from class, AESTHETIC SCIENCE INSTITUTE MUST BE NOTIFIED before classes begin in the morning of that day. Students have committed to attending class based upon their chosen schedule, and are expected to be present at each session, aside unforeseen circumstances.

In the event of a maximum of 7 day non- attendance/contact; The Aesthetic Science Institute, LLC. will attempt contact via e-mail, mail or telephone to determine status. If confirmation of status is not determined by day 14 of non-attendance the student will be dismissed form the program. An excess of 60 Esthetics/90 Advanced/30 Nail and 7.5 Waxing total hours of absenteeism may result in dismissal from the program. Late starts are permitted only up to 35 hours into any program/ 7 hours in the waxing program. Tardiness Policy Regular attendance is essential to the successful performance of each of our programs and to the development of our future leaders in the Beauty Profession. Tardiness is considered unexcused time, as it is deemed unprofessional behavior. All tardy hours are calculated into the total allowable absence hours.

All students are expected to be clocked in and ready to study at their regular starting time and remain in the classrooms/clinic until their regular scheduled quitting time, except when clocked out for lunch break. Students not physically present in class, per their class start time, are considered tardy.

When a student shows up late for school, they cannot attend the given session. Tardy am session arrivals will be permitted to clock in after lunch. Tardy afternoon arrivals can return the next scheduled day. Tardy students will not get hours for that session of the day.

Make-up Hours All missed hours must be completed within maximum timeframe of scheduled graduation. Make-up hours are scheduled in 4 hour increments and are available two weeks prior to SAP date for SAP1, and two weeks prior to graduation plus maximum time frame for SAP 2/SAP3. Hours must be made up before a certificate is awarded. Failure to makeup hours within the allowed time will result in a non-completion of the program.

Lost or Missed Hours Given hours are mandated by the State of New York, and by The Aesthetic Science Institute, LLC.. Accumulation of daily hours will be maintained and must be made up in order to complete the educational program and be eligible for participating in the state exam. All tests, quizzes, assignments and home-work must also be made up prior to graduation.

Over Hour Fee Policy Each course/program has been scheduled for completion within an allotted time frame. A grace period of approximately ten percent has been added to the calculated completion date for each program. It is not realistic to expect to receive an education for free. The school has reserved space, equipment, and licensed instructors for each student and course/program.

Additional training will be billed at the rate of $15 per hour, payable in advance or prior to graduation. Students making up hours beyond the scheduled graduation will not be allowed to clock in until applicable payments are made. Sexual Misconduct Sexual misconduct of any kind is not permitted. Any accusation deemed valid is grounds for termination.

Meals and Beverages A student lounge is provided for lunches and breaks. Food is not permitted in the classrooms or the clinic areas at any time.

Payment Students are expected to fulfill their agreed upon payment plan for tuition. Tuition must be paid in full before a certificate is granted. Half of tuition is due prior to the first day of class in order to begin.

Leave of Absence Policy (LOA) Any interruption of program time is discouraged. In certain emergency situations, if an interruption is unavoidable a leave of absence shall be granted and will extend contract period and maximum time frame, by the number of days taken during the leave of absence. Changes to a contract period must be noted on the enrollment agreement and initialed by all parties; or an addendum must be signed and dated by all parties.

An LOA implies that there is an expectation that a student will return within thirty days.

All requests for leaves of absence must be submitted in writing, including the reason for the request as well as the students signature. Written requests for LOA’s must be submitted in advance of the actual leave, unless unforeseen circumstances prevent this from happening. An LOA may be granted without a prior written request if The Aesthetic Science Institute, LLC. documents the reason for such a decision and receives the written request at a later date.

Only one leave of absence will be granted during the time that a student is enrolled and shall not exceed thirty class days. If a student takes a leave at any time during the program, they shall be responsible for making up all class time, tests, assignments and quizzes and homework missed during their leave prior to graduation.

A student granted an approved LOA is not considered to have withdrawn, and no refund calculation will be performed. There will be no additional institutional charges as a result of an approved LOA.

In the event that a student does not return from an LOA, an official withdrawal date will be determined as of the last actual date of attendance.

Grading and Evaluation New York State requires completion of all required hours, a complete physical, application fee of $40.00, a $15.00 NYS practical exam fee; and a pass rate on State board written and practical examinations of 70%/ for state licensure. Students will receive a variety of grades while attending classes. All test grades will be recorded as a percentage of correct, 70% is considered a passing grade. Any grade below 70% will be equally recorded and may not be retaken. An overall 70% is required in order to pass the enrolled course. An average score that falls below 70% may result in a probationary period and possible dismissal. In curriculum with few test grades, such as waxing and Nail Specialty, test grades below 70% may be retaken, but will only allow for a maximum of 70% to be recorded.

Each curriculum also has a required amount of practical services that must be completed prior to graduation. Ample time is given to achieve these requirements.

Students who complete with a grade point average of 90% or above will receive a gold seal of achievement on their diploma.

Complaint procedure If a student has a suggestion or a complaint they are urged to contact their instructor. If they are not satisfied with the results they are urged to contact the director of The Aesthetic Science Institute, LLC.. Complaints should be addressed in writing to the Director in order to be kept on file. You may file a complaint with the New York State Education Department.

Discrimination Policy The Aesthetics Science Institute is an equal opportunity learning facility we do not discriminate based on sex, age, race, color, religion, sexual orientation or ethnic origin.

Policy on Accusation of Breach of Policies and Procedures In the event of breach of policy and procedure accusation the accusee must agree to provide details, sign a statement and pursue the issue. If accusee does not agree, the accusation is deemed unfounded. If agreed, the school will proceed with an internal investigation and accuser may be suspended for three days. If found valid, appropriate authorities will be notified. If unfounded accuse returns to position with no loss of hours. If found valid accuse will be dismissed from position at The Aesthetic Science Institute, LLC..

Financial Assistance Monthly payment plans are available with payment of 50% of tuition of a given curriculum. Students must be aware that available financial options are private loans, payment options and Title IV aid if qualified. Students are therefore responsible for repayment of such loans regardless of academic success. For more information please contact the Director .

Returned Checks If a tuition payment is made with a personal check and the check is dishonored at the payer’s bank, the student is subject to a returned check fee of $30.00.

Additional Fees There are no additional education fees while in school aside from over hours. Students are required to purchase a kit for state board practical exams. The average cost runs approximately $90.00. Three mock state board examinations are given during school time. Students are expected to have their kit prepared for mocks. Placement and Add’l Services Information While placement service may be provided, it is understood that the school cannot promise or guarantee employment to any student or graduate. Students will complete an actual job interview in which they are provided feedback. Resumes and cover letters will also be completed. A job placement information site is available to students and graduates in the school student lounge and all job postings are placed on the school web site.

Career Counseling Career counseling is stressed throughout the course in personal hygiene and posture. Also stressed throughout the course is communication with others both verbal and non- verbal. Areas that are discussed both in class and on a one-to-one basis are the different specialized fields; one may go into, such as an esthetician, nail technician or waxing specialist in a spa or salon; training, sales and education for a product manufacturer.

Physical Demands There are many factors to consider when deciding upon choosing an esthetics career, such as physical health. Estheticians usually work on an appointment basis, and as a clientele builds, days may become busy and hectic. Not only are you working with your hands and arms, but may be sitting or standing in the same position for great lengths of time.

Conflicting Information The Aesthetic Science Institute, LLC. understands that all conflicting information must be resolved before any Title IV funds can be disbursed. Students with conflicting information in their files or paperwork may be selected for verification by the school or its servicer in order to resolve the information in question.

Code of Conduct The School is committed to ensuring the integrity of its employees and students with respect to all aspects of its schools and operations. Compliance with all applicable laws, regulations, company policies & procedures, and performance of duties according to the highest standards of honesty and integrity, is expected of all students. Employees assisting students in the financial aid process are required to affirm that they will 1) be ethical and conduct themselves with integrity, 2) avoid any conflicts of interest and comply with all Codes of Conduct required by the school, 3) provide prospective and enrolled students with accurate and complete financial aid and policy information, 4) keep student information confidential and comply with the Family Educational Rights and Privacy Act (FERPA), and 5) comply with federal and state laws and regulations, accreditor rules, and school policies & procedures.

Record Disclosure and Information Release We are required by NY State Law to maintain accurate records of our students even after they've completed their course work with us. By providing your email on this form, you agree to allow us to communicate with you via email as needed in order to obtain necessary information. This communication may, from time to time, include surveys which we are required to have you complete in order to maintain accurate records.

Policy on Releasing Students Information Any request for releasing information on an individual student must be made by the student or parents of dependent minors by filling out an advisement/referral form through the schools director. All students records are made available to the accrediting agency. Records will be released to a third party ONLY with written consent of the student. The Aesthetic Science Institute, LLC. complies with all FERPA/The Family Education Rights and Privacy Act. A release of information form must be completed with each request.

Voter Registration The school encourages its students to be registered voters and to exercise their right to vote. Students can register to vote in New York at www.rockthevote.com, at www.londistancevoter.org, or at www.sos.wa.gov/elections.

Vaccination Policy While The Aesthetic Science Institute, LLC. encourages its students to lead happy healthy lifestyles and stay disease free it does not have any specific vaccination policy. Vaccinations are not a requirement to be licensed in the State of New York.

Student Services, Counseling and Housing The school conducts an orientation program on the first day of class which provides information about the instructional programs, goals of each course, policies affecting students and services available to students. Students have access to advising from members of the school staff regarding their program of study, progress, financial affairs, housing, placement or other areas in which the student may need assistance including referral to professional assistance when necessary. Students whose progress in the course is unsatisfactory are advised and provided any additional assistance available. When requested, the school may provide assistance and advisement in acquiring adequate housing facilities. A student requiring housing assistance should contact the school prior to enrollment. Housing is not provided by the school. Handicapped Access The School and its facilities are adequately equipped to meet the needs of handicapped students and individuals in need of the assistance of a wheelchair or crutches.

Gum Chewing Gum chewing is not permitted in school clinic.

Smoking Smoking is not permitted in the school, around the entryways or on the front walk way. Students who smell of smoke when returning after a break will not be permitted to work on clients in the clinic.

Policy On Evaluating The Validity Of High School Diplomas Regulation 34 CFR 668.16 (p) requires Title IV schools to establish policies and procedures to confirm the authenticity of high school diplomas in the event that the school or the Secretary of the U.S. Department of Education has reason to question the validity of a student’s high school diploma.

The Aesthetic Science Institute, LLC. may require further documentation in the form of a certified copy of final high school transcripts for the high school in question or information from a company that evaluates foreign diplomas (in the case of a foreign diploma). Student self-certification is NOT considered sufficient proof of validity.

In addition to checking online for further information about the school issuing the diploma and its accreditation, the school may also contact the Department of Education in the state in which the diploma was issued to determine if the school listed on the diploma is on the state list of recognized schools.

The school maintains a list of known diploma mills for the admissions staff to check when receiving a diploma from an unknown and questionable source. It is understood that this list may not be all inclusive as there are hundreds of diploma mills some known and some not currently known. It is also understood that the list of schools in the FAFSA drop down box online also may not be all inclusive.

The school makes every reasonable effort to verify the validity of questionable high school diplomas. On-line GED/High School Diplomas are not accepted.

The Aesthetic Science Institute, LLC. does not offer a Comprehensive Transition Postsecondary Program for students with intellectual disabilities.

Cost of Attendance Budgets (COA) In order to determine a student’s level of loan funding, the Department of Education requires us to develop annual cost of attendance budgets. These budgets include an average allowance for room and board, transportation, miscellaneous, loan fees and if applicable child care and expenses related to disability.

Drug Abuse Prevention The school actively supports the prevention of drug abuse. Upon enrollment, students are provided the school’s policy on drug and alcohol abuse. A list of agencies and counselors is also available on the state website for personal assistance and is available to any student requesting assistance.

OSHA Requirements In compliance with United States Department of Labor Occupational Safety and Health Administration requirements, the school advises its students of the chemicals used in cosmetology/related training. During the course work the student learns about the importance of safety in the work place and how to use and follow the Material Safety Data Sheets (MSDS) for chemicals used in cosmetology or related training. During each unit of study, students are apprised of the various chemicals used and safe practices that apply. A complete file containing Material Safety Data Sheets for the chemicals used is available in the administrative office. The school endeavors to facilitate a safe environment for staff and students by

teaching the proper and safe use of equipment, tools, and products. The school does not assume responsibility for injuries resulting from improper or unsafe use of equipment, tools, or products.

Policy for Safeguarding Student Information The Aesthetic Science Institute, LLC. is committed to implementing and maintaining a comprehensive information security program, to maintain and safeguard your non-public personal information against damage or loss. The policy covers all student records in whatever form (hard copy, electronic).

The school Director/administrator shall be responsible to coordinate the school’s information security program. The Director shall, at least once every 3 years, assess foreseeable internal and external risks to the security, confidentiality, and integrity of student information that could result in the unauthorized disclosure, misuse, alteration, destruction or other compromise of the information. The risk assessment shall cover every relevant area of school operations, including employee training & management, network & software design, information processing, storage, transmission and disposal, and ways to detect, prevent and respond to attacks, intrusions, or other system failures. The Director shall design and implement safeguards to control identified risks and shall monitor the effectiveness of them, recommending changes when warranted.

Records for prospective students who are not accepted or who do not enroll in the school will be held for 3 months then destroyed in a secure manner. Records of enrolled students shall be maintained in accordance with federal and state law and accreditation requirements. Students shall receive notice of this policy at the time they enroll. The Aesthetic Science Institute, LLC. shall only enter into servicing agreements with service providers who also maintain appropriate safeguards for customers’ non-public personal information.

Veterans Credit for Previous Education and Training Students must report all education and training. The Aesthetic Science Institute, LLC. will evaluate and grant credit, if appropriate, with the training time shortened, the tuition reduce proportionately, and the VA student notified.

Satisfactory Academic Progress Satisfactory Academic Policies are provided to all students prior to enrollment.

Evaluation Periods Students, in all programs are evaluated, and receive a copy, for Satisfactory Academic Progress as follows: Esthetics 300 Scheduled Hours Nails 150 Scheduled Hours Waxing 37 Scheduled Hours Advanced Esthetics 450 Scheduled Hours

Evaluations will determine if the student has met the minimum requirements for satisfactory academic progress. The evaluation ensures that students have opportunities to meet both the attendance and academic progress requirements by midpoint in the course.

Attendance Progress Evaluations Students are required to attend a minimum of 90% of the hours possible based on the applicable attendance schedule in order to be considered maintaining satisfactory attendance progress. Evaluations are conducted at the end of each evaluation period to determine if the student has met the minimum requirements. Evaluations are based on the cumulative attendance percentage as of the last day of the evaluation period. The attendance percentage is determined by dividing the total hours accrued by the total number of hours scheduled. The students start month in school will be counted as the first month of the evaluation period. For partial months, scheduled hours will be calculated by multiplying the number of scheduled days by the number of hours scheduled each day. At the end of each evaluating period, the school will determine if the student has maintained at least 90% cumulative attendance since the beginning of the course which indicated that, given the same attendance rate, the student will graduate within the maximum time frame allowed.

Determination of Progress Status Students meeting the minimum requirements for academics and attendance at the evaluation point are considered to be making satisfactory progress until the next scheduled evaluation. In order for a student to be considered making satisfactory academic progress as of course midpoint, the student must meet both attendance and academic requirements on at least one evaluation by midpoint of the course. The institution will notify a student of any evaluation the impacts the students eligibility for financial aid.

Academic Progress Evaluations The qualitative element used to determine academic progress is a reasonable system of grades as determined by assigned academic learning. Students are assigned academic learning and a minimum number of practical experiences. Academic learning is evaluated after each unit of study. Practical assignments are evaluated as completed and counted toward course completion only when rated as satisfactory or better (the computer system will reflect completion of the practical assignment as a 100% rating). If the performance does not meet satisfactory requirements, it is not counted and the performance must be repeated. At least two comprehensive practical skills evaluations will be conducted during the course of study. Practical skills are evaluated according to the text procedures and set forth in practical skills evaluation criteria adopted by the school. Students must maintain a written and practical grade average of 70% to maintain satisfactory academic progress. Probation The status of probation can be granted to students Who are not considered meeting minimum Standards for satisfactory academic progress if:

- The institution evaluates the student’s progress and determines that the student did not make satisfactory academic progress during the warning or previous evaluation period; and

- The student prevails upon appeal of a negative progress determination prior to being placed on probation; and

- The institution determines that satisfactory academic progress standards can be met by the end of the subsequent evaluation period; or

- The institution develops an academic plan for the student that, if followed, will ensure that the student is able to meet the institution’s satisfactory academic progress requirements by a specific point within the maximum timeframe established for the individual student. During probation students are not considered to be making satisfactory progress unless an appeal has been granted. If at the end of the probationary period, the student has still not met both the attendance and academic requirements, he/she will be determined as NOT making satisfactory academic progress and, if applicable, students will not be determined eligible to receive Title IV funds will be dismissed from the program. If the institution determines that SAP can be achieved by the end of the probationary period, Title IV funds may be retained. A specific plan of action will be determined. In this situation, students will be notified of status and possible loss of Title IV funds.

Re-Establishment of Satisfactory Academic Progress Students may re-establish satisfactory academic progress and Title IV aid, as applicable, by meeting minimum attendance and academic requirements by the end of the probationary period.

Noncredit and Remedial Courses Noncredit and remedial courses do not apply to this institution. Therefore, these items have no effect upon the schools satisfactory progress standards.

Interruptions, Course Incompletes, Withdrawals If enrollment is temporarily interrupted for a Leave of Absence, the student will return to school in the same progress status as prior to the leave of absence. Hours elapsed during a leave of absence will extend the student’s contract period and maximum timeframe period, by the same number of days taken in the leave of absence and will not be included in the student’s cumulative attendance percentage calculation. Students who withdraw prior to completion of the course and wish to re-

enroll must re-apply as a new student starting at the beginning of the program. Previously earned hours and tuition will not be applied. If a student wishes to withdraw they need to complete a withdrawl form, or notify via a written statement to the Director. A status request form is submitted to third party processor and refund calculation form is completed and the student is notified. Any refunds due are paid within 30 days. Appeal Procedure If a student is determined to not be making satisfactory academic progress, the student may appeal the determination at that time. The student must submit a written appeal to the school on the designated form with supporting documentation of the reasons why the determination should be reversed, such as: death of a relative, an injury or illness of the student, or other allowable special circumstances. Appeal must include documentation supporting non-compliance with SAP and changes in the students’ situation to allow them to meet SAP at the next evaluation period.

Termination The school may terminate a student prior to the completion of their program for one or more of the following: 1) Insufficient academic progress. 2) Not fulfilling financial obligations to the school. 3) Failure to comply with the rules and regulations of the school. 4) Failure to comply with the Tardy/Attendance policy of the school. 5) Insubordination. 6) after 14 consecutive school days of absence or 7) in the case of a leave of absence on the documented date of return if the student does not return.

Appeal documents will be reviewed and a decision will be made and reported to the student within 30 calendar days. The appeal, decision documents and academic plan will be retained in the student file. If the student prevails upon appeal, the satisfactory academic progress determination will be reversed and federal financial aid will be reinstated, if applicable. If the appeal is NOT reversed the student will be dismissed from the program. Any refunds will be calculated under the Refund Policy Guidelines. Graduation Requirements Graduation requirements for all courses are as follows: 1) Completed the required hours of training within the maximum time frame allowed. 2) Maintain satisfactory progress requirements per the criteria of the Satisfactory Progress Policy. 3) Pass a final written and practical exam with 70%. 4) Fulfill all financial obligations to the school. 5) Completion of an exit interview and required exit paperwork. UPON GRADUATION A DIPLOMA WILL BE AWARDED. Certification of hours will not be provided until all tuition charges have been paid in full. Students who complete with a grade point average of 90% or above will receive a gold seal of achievement on their certificate.

Previously Earned Credits The Aesthetic Science Institute, LLC. does not accept clock hours or transfer credits from other institutions in any programs except for the Advanced Esthetics Course. Six hundred hours with a valid state esthetics license is acceptable for transfer. For transfer hours accepted in the Advanced Esthetics Course such hours accepted are counted as both attempted and completed hours for the purpose of determining when the allowable maximum timeframe has been exhausted. SAP evaluation periods are based on actual contract hours at the institution.

Federal Assistance Programs The school is approved as an eligible institution by the U. S. Department of Education to participate in Title IV grant and loan programs. The packaging of financial assistance is determined according to guidelines set by the US Department of Education. A variety of programs are available for students qualifying for assistance:

Federal Grants: Federal Pell Grant: Intended to be the basis of the financial aid package and may be combined with other aid to meet the full cost of attendance. The Federal Pell Grant is a need based aid program in which an eligible recipient does not have to repay the funds received.

Federal Student Educational Opportunity Grant (SEOG): An additional grant available only to Pell eligible students who meet the schools predetermined criteria. 75% of the award is contributed by the Federal Government and the other 25% is contributed by the school.

Federal Direct Loan Program: These are low interest loans for undergraduate and graduate students that are made available through the Federal Government. This program includes the Federal Direct Subsidized, Federal Direct Unsubsidized and Federal Direct Plus loans. There are grade level progressions and loan limits used for the administration of these loans.

The following Direct Loans are made through the William D. Ford Federal Direct Loan (Direct Loan) Program, which is administered by the U.S. Department of Education:

Federal Direct Subsidized Loan: This is a need- based-loan for which the Federal government subsidizes the interest until repayment begins and during any period of deferment. This is a loan and recipients must begin making payments at the end of their six-month grace period.

Federal Direct Unsubsidized Loan: This is a non- need-based loan for which the Federal Government does not pay the interest subsidy. Interest accrues during all periods. The recipient has the option to pay the interest or to defer payment of the interest for the grace period. This is known as capitalization. This is a loan and recipients must begin making payments at the end of their six-month grace period. Federal Direct Plus Loan: This loan is available to parents of dependent undergraduate students to help pay for the cost of the dependent’s education. Borrowers of PLUS Loans are required to undergo a credit check by the lending institution. The definition of a “parent” for PLUS Loan eligibility is a student’s biological, adoptive or step-parent in the event that person’s income would have been taken into consideration when calculating the student’s expected family contribution (EFC). Interest is charged during all periods.

Entrance and Exit Counseling: Students receiving Federal Direct Loans must complete a Student Loan Entrance Counseling prior to disbursement of funds and a Student Loan Exit Counseling within 30 days of graduating or withdrawing from school.

Title IV Credit Balances: Tuition and fees are charged by academic year. Federal student aid is received by payment periods. Generally there are two payment periods in a full academic year. When Title IV funds received for the academic year are greater than the charges for the academic year, a credit balance is created on the student’s account. This credit balance will be refunded to the student (or parent in the case of a Parent PLUS loan), within 14 calendar days of the credit balance being created on the student’s account.

Verification Students selected by the U.S. Department of Education for the process of verification are frequently required to submit additional information and/or parents’ financial & household information to the finance office. The verification procedures will be conducted as follows:

1. When selected by the U.S. Department of Education for the process of verification, the student must submit all required documentation to the finance office within 10 days from the date the student is notified that the additional documentation is needed for this process.

2. If the student does not provide all of the required documentation within the 10 day time frame, the student will be required to make other payment arrangements until the documentation is received and the student’s eligibility for federal student aid has been established.

3. The finance office reserves the right to make exceptions to the policy stated above on a case by case basis for extenuating circumstances

The finance office will notify the student of any changes to their financial aid award resulting from corrections made due to the verification process. An adjustment will be made to the student’s financial aid award as required by federal regulations and an addendum to the existing award letter or a new award letter will be issued Return of Title IV Funds The law specifies how your school must determine the amount of Title IV program assistance that you earn if you withdraw from school. The Title IV programs offered at this school are: Federal Pell Grants Federal Direct Stafford Loans (Subsidized and Unsubsidized) Federal Direct PLUS Loans

Calculation of the Amount of Title IV Funds That a Student Has Earned Upon Withdrawal: When you withdraw from school during a payment period the amount of Title IV program assistance that you have earned up to that point is determined by a specific formula. The amount of assistance that you have earned is determined on a pro-rata basis. For example, if you completed 30% of the scheduled hours in your payment period, you earn 30% of the assistance you were originally scheduled to receive. Once you have completed more than 60% of the scheduled hours in your payment period, you earn all the assistance that you were scheduled to receive for that period. The Condition Under Which Title IV Program Funds Would Be Required To Be Returned: If you received more assistance than you earned, the excess funds must be returned by the school and/or you. If you receive (or your school or parent received on your behalf) excess Title IV program funds that must be returned, your school must return a portion of the excess equal to the lesser of: your institutional charges multiplied by the unearned percentage of your funds, or the entire amount of excess funds. The school must return this amount even if it didn’t keep this amount of your Title IV program funds. The Condition under Which a Student Would Be Owed a Disbursement of Title IV Program Funds upon Withdrawal from the Institution: If you received (or your school or parent received on your behalf) less assistance than the amount that you earned, you may be able to receive those additional funds as follows: If you did not receive all of the funds that you earned, you may be due a post-withdrawal disbursement. If your post-withdrawal disbursement includes loan funds, your school must get your permission before it can disburse them. You may choose to decline some or all of the loan funds so that you don’t incur additional debt. Your school may automatically use all or a portion of your post-withdrawal disbursement of grant funds for tuition, fees, and room and board charges (as contracted with the school). The school needs your permission to use the post-withdrawal grant disbursement for all other school charges. If you do not give your permission (some schools ask for this when you enroll), you will be offered the funds. However, it may be in your best interest to allow the school to keep the funds to reduce your debt at the school. There are some Title IV funds that you were scheduled to receive that cannot be disbursed to you once you withdraw because of other eligibility requirements. For example, if you are a first-time, first-year undergraduate student and you have not completed the first 30 days of your program before you withdraw, you will not receive any Direct Loan funds that you would have received had you remained enrolled past the 30th day.

How the Withdrawal Date for a Student Who Withdraws Officially or Unofficially is Determined: For clock hour schools the actual last date of attendance is the last day the student was physically in attendance. This is the withdrawal date. The determination date of the withdrawal date on a student who had been previously attending could be up to, but will not to exceed 14 calendar days from that student’s actual last date of attendance. An active student is determined to be officially withdrawn on the day they notify the school’s administrative office of their intention to withdraw from school or to not return from an approved leave of absence. An active student is determined to be unofficially withdrawn when they have been absent for 10 consecutive school days (14 calendar days) from their last date of physical attendance without notifying the school’s administrative office. A student who does not return from an approved leave of absence is determined to be withdrawn on the day they were scheduled to return but did not. Time Frame for the Return of Title IV Funds: The school has 30 days from the last date of attendance to complete an R2T4 calculation. The school then has 45 days from the date of determination of the withdrawal to return any funds due to the Title IV programs and no more than 60 days from the date of withdrawal or last date of attendance. The Order in Which Title IV Program Funds Must Be Returned: The school and or the student if applicable must return Title IV funds to the programs from which the student received aid during the payment period or period of enrollment as applicable, in the following order, up to the net amount disbursed from each source:

1. Unsubsidized Direct Stafford loans (other than PLUS loans).

2. Subsidized Direct Stafford loans. 3. Direct PLUS loans. 4. Federal Pell Grants for which a return of funds is

required.

Repayment of Student Loans The student loans that remain outstanding in (Box R) of Step 8 of the R2T4 calculation worksheet consist of the loans disbursed to the student (Box B) minus any loans the school repaid in Step 6, Block P. These outstanding loans are to be repaid by the student according to the terms of the student’s

promissory notes. Title IV Grant Funds to Be Returned By a Student The regulations limit the amount a student must repay to the amount by which the original overpayment amount exceeds 50% of the total grant funds disbursed or could have been disbursed by the student for the payment period or period of enrollment. The initial amount of unearned Title IV grant aid due from the student in step 9 (Box S) of the R2T4 calculation worksheet is found by subtracting the loans to be repaid by the student (Box R) from the initial amount of unearned aid due from the student (Box Q). The amount of grant overpayment due from a student is limited to the amount by which the original grant overpayment (Box S) exceeds half of the total Title IV grant funds disbursed and could have been disbursed to the student in (Box F). Return of Title IV Grant Funds by the Student: The student is obligated to return any Title IV overpayment in the same order that is required for schools. Grant overpayments may be resolved through:

1. full and immediate repayment to the school; 2. repayment arrangements that are satisfactory to the school;

or 3. by overpayment collection procedures negotiated with Debt

Resolution Services. A School’s Responsibilities in The Return of Funds by The Student: A school has responsibilities that continue beyond completing the Return calculation and returning the funds for which it is responsible. Here we discuss the institution’s participation in the return of funds by the student. Grant Overpayments: The applicable regulations limit the amount of grant funds a student must repay to one-half of the grant funds the student received or could have received during the applicable period. Moreover, repayment terms for students who owe Title IV grant overpayments were established to ensure that students who could not immediately repay their debt in full had the opportunity to continue their eligibility for Title IV funds. Students who owe overpayments as a result of withdrawals initially will retain their eligibility for Title IV funds for a maximum of 45 days from the earlier of:

1. the date the school sends the student notice of the overpayment, or

2. the date the school was required to notify the student of the overpayment.

Within 30 days of determining that a student who withdrew must repay all or part of a Title IV grant, the school will notify the student in writing via U.S mail that he or she must repay the overpayment or make satisfactory arrangements to repay it. In its notification, the school will inform the student of the following five items: 1. The student owes an overpayment of Title IV funds. 2. The student’s eligibility for additional Title IV funds will end if the student fails to take positive

action by the 45th day following the date the school sent or was required to send notification to the student.

There are three positive actions a student can take to extend his or her eligibility for Title IV funds beyond 45 days: a) The student may repay the overpayment in full to the school. b) The student may sign a repayment agreement with the school. Note: Two years is the maximum time a school may allow for repayment. c) The student may sign a repayment agreement with the Department. *****The student should contact the school to discuss his or her options***** Consequences for Not Following upon the Notification: If the student takes no positive action during the 45-day period, the school will report the overpayment within a few days of the end of the 45-day period to NSLDS on the NSLDS Professional Access Web site under the AID tab, “Overpayment List” menu option after the 45-day period has elapsed. If the student fails to take one of the positive actions during the 45-day period, the student’s overpayment will be reported immediately to the NSLDS and referred to the Debt Resolution Services for collection.

When a student receives additional funds during the 45-day period of extended eligibility: Students who owe overpayments as a result of withdrawals generally will retain their eligibility for Title IV funds for a maximum of 45 days from the earlier of (a) the date the school sends the student notice of the overpayment, or (b) the date the school was required to notify the student of the overpayment. A student who receives Title IV funds within that period of extended eligibility and then fails to return the overpayment or make repayment arrangements becomes ineligible for additional Title IV program funds on the day following the 45-day period. However, any Title IV program funds received by the student during the 45-day period were received while the student was eligible. Therefore, those Title IV funds do not have to be returned (unless the student withdraws a second time). A student who loses his or her eligibility for Title IV funds at the expiration of the 45-day period will remain ineligible for additional Title IV funds until the student enters into a repayment agreement with the Department. If, at any time, a student who previously negotiated a repayment arrangement fails to comply with the terms of his or her agreement to repay, that student immediately becomes ineligible for additional Title IV funds. The school will also notify the student in writing via U.S. mail using a Refund Calculation worksheet, of the amounts of aid that were retained by the school for institutional charges and the amount(s) of aid that will be refunded by the school to each Title IV program. The student will also be notified of the amount of Title IV loans they will owe after all applicable refunds have been made, the requirement for the student to complete exit counseling, when the student will be responsible to start repayment of their student loans and who they may contact for further assistance or to report any changes to their personal information. The requirements for Title IV program funds when you withdraw are separate from any Institutional Refund Policy that your school may have. Therefore, you may still owe funds to the school to cover unpaid institutional charges. Your school may also charge you for any Title IV program funds that the school was required to return. If you have questions about your Title IV program funds, you can call the Federal Student Aid Information Center at 1-800-4-FEDAID (1-800-433-3243). TTY users may call 1-800-730-8913.
