Berlinger & Co. AG Clinical material and medical device temperature indicator to reduce waste at the last mile

On Wednesday, October 7th 2015 Berlinger & Co AG began a soft launch of the Mini-­‐tag® 3 alarm temperature indicator at the Annual IQPC Cold Chain Global Forum in Boston. “Numerous manufacturer’s and clinical supply directors at the conference explained that they need a product like this to reduce waste due to lack of data in normal handling situations,” says Patrick McGrath – GM at Berlinger USA LLC. There is ever increasing regulatory scrutiny on manufacturers of high priced commercial biologics and narrowly focused biological materials in clinical trials. Regulators want more and more proof that the cold chain has been maintained. Manufacturers often have temperature stability budgets and qualified packaging systems to manage the cold chain, but regulators and supply chain partners need proof to validate that materials were held within specification. Data loggers are expensive and the data is difficult for clinical patient to understand.

The Mini-­‐tag® from Berlinger has miniaturized the temperature sensing and processing technology to insure accurate temperature measurements for up to 3 years. Three (3) independent time/temperature alarms enable manufacturers to leverage temperature stability budgets without compromising product integrity. These specifications allow clinical and commercial materials to follow their normal distribution path with a reliable and inexpensive electronic indicator on-­‐-­‐-­‐board. Because they are custom programmed they will alarm according to the specific products temperature profile. If a patient calls with a normal handling issue, manufacturers will be able to say, “press the button and if the light is green it is OK.”

The Mini-­‐tag® was designed in conjunction with manufacturer’s who have temperature sensitive clinical materials that are administered by patients outside of the clinic. Currently, clinical patients make handling mistakes like leaving the 2nd syringe on a counter for a few hours. Without an indicator to validate that the materials are within their temperature stability range sponsor companies must scramble to replace materials that are most likely OK for use. With data the materials can remain in the field for proper consumption. Additionally, some medical implant devices require temperature monitoring through their life cycle. A device like the Mini-­‐tag@ enables manufacturers to place electronics closer to the product at a reasonable cost.

Transport qualification and stability budget two key focus areas in peer-to-peer workshops led by ELPRO

Boston MA – Two workshops held by ELPRO revealed a number areas where pharmaceutical manufacturers can easily solve

common shipping problems by using temperature data correctly; but also highlighted a number of more complex issues including

transport qualification and stability budget.

Many pharmaceutical manufacturers rely on carriers for proof that their shipments are in control, and also more recently how to

carry out their qualification approaches. But participants at the peer-to-peer workshops were asking, how much carrier’s data

should be used to develop such risk-based approaches?

Some manufacturers today consider lane qualification picking the worst case as the rationale, but most agreed rather you need

ongoing empirical data to have a statistically valid approach that regulator’s will accept. With the worst case ‘shot gun’ approach,

you don’t consider new lanes, it’s a moving target. Everyone agreed whatever your approach, the rationale must be documented

and risk-based.

ELPRO’s ‘Supply Chain Mapping & Transport Qualification’ workshop presented a benchmarking study based on input from five

different pharmaceutical manufacturers and how they approach setting up shipping qualifications and how they use stability budget

in their cold chain.

It was revealed that Stability Budget is a complex issue for most pharma companies. In planning stages, some companies use

Distribution approach, but live in transit use a Manual approach.

Technical Report 53 of the Parenteral Drug Association (PDA) recommends calculating a stability budget for each product as a best

practice approach by “using scientific data and rationale necessary to determine an appropriate stability budget for a drug product

over the entire lifecycle of a product”. This becomes very important when a product is questionable for release at destination.

Takanori Aasberg Miyashita, System Manager Temperature Control, at Novo Nordisk explains “We manually calculate total time out

of refrigeration against allowable excursion time. The challenge is adding up all deviations across the different supply chain ‘levels’

(manually)”.

In ELPRO’s recent release of elproVIEWER Professional, a temperature data analytics platform, the "join" functionality has been

enhanced and offers now two reports, the Joint Report and the Joint Assessment Report. It allows combining temperature data of

up to 16 PDF reports, e.g. to calculate the life-cycle stability of a batch or to determine the annual MKT. elproVIEWER automatically

calculates the statistics over the life cycle or the entire year and outputs the result in a Joint Report.

the supply chain, without the manual work. Hence it is possible for QA to check the availability of remaining stability budget of

former transport ‘legs’ and assign unspent stability budget to remaining transport ‘legs’ using elproVIEWER. From a QA perspective,

avoiding using spreadsheets eliminates the possibility of data manipulation because the data is embedded in the PDF and cannot be

altered in the software”.

Two years ago, at Cold Chain Global Forum 2013, Sofrigam revealed

the iBox, a high-technology cold chain container equipped with a

temperature regulating system. This ingenious device controls the

payload temperature and is triggered to release or inhibit the cold source

depending on variations in the outside temperature, so as to stabilise the

internal temperature at +5 °C (±2 °C). This system ensures that a

constant temperature is maintained, with traceability of the data, for up to

5 days.

Improved thermal performance

in order to improve resistance to leaks and reduce the risk of thermal bridges. These modifications have

resulted in a more constant temperature – for up to 5 days – of the products in transit, even when exposed to

highly stress-inducing outside temperatures.

A new, more accurate measuring system

The iBox is fitted with a sensor which measures the temperature in the payload area. This sensor checks that

the product temperature is maintained at between +2 and +8 °C throughout the journey. The new sensor

incorporates a more accurate measuring technology. It can be corrected or replaced in the event of a

malfunction.

An e-connected container

With a Bluetooth connection for communicating with tablets and smartphones and for retrieving data via an

Android and Apple application, the latest version of the iBox will be highly connected. Now it will be possible to

follow the container in real time from a mobile phone. The programming software has also been completely

redesigned to be more ergonomic.

SilverSkin Quattro™ incorporating SilverSkin BLAK™ technology, a new thermal protective material

Real-time Temperature Monitoring in the Cloud

On October 7th through October 8th, the global provider of cloud temperature monitoring

solutions, TSS, sponsored and exhibited at the Cold Chain Global Forum in Boston. During

the event, TSS showcased one of their most recent additions to their solutions portfolio,

i.e. the real-time monitoring and tracking device, which was fully integrated into TSS’

monitoring cargo/environment and global wireless communication capability. The

device’s on-board sensors provide temperature monitoring (calibrated to NIST standards)

to protect and certify sensitive cargo. Other on-board sensors can be used to monitor

shipments for excessive shock, tilting and package tampering. The ILC2000 has been

approved for air cargo by the FAA and EASA using a patented “airplane safe mode”

feature to automatically deactivate the radio transmitter when the device is airborne,

yet it still continuously tracks and logs location and cargo/environmental data. Similarly,

the radio transmitter is automatically reactivated when the plane lands.
