to the office before 15-3-2014. (Maximum 1500 words).
* Candidates should not write their names or addresses on the essay. They should not reveal their identity in the essay. Candidate should write the name and complete address on a separate piece of paper.

(For Family Physicians only).
* The last date for submission of abstract is 15-3-2014. Complete text of the paper will have to be submitted on the day of presentation.
* All papers accepted by the Scientific Committee will be read few days before our Annual Day function.
* The paper must be original and must not have been published or read previously.
8 minutes will be allowed for presentation and 3 minutes for discussion.

Tumor BoardEssential concept for cancer care
In this era of advanced research culminating to increased cure rates & better quality of life for cancer patients,it requires a clear obligation on Cancer Specialists to offer all their patients not only an option for cure but an option for possibility for maximum chance for cure at minimum deterioration in quality of life. There are clinical situations that are beyond the realms of standard guidelines or there are situations for which guidelines are not defined as level 1 evidence where a clinician needs consensus from a multi-disciplinary team. World over in leading cancer centers, 'Tumor Board' is such an arrangement which guides the clinicians & ultimately patients to the optimum cure option with minimal morbidities. 1) Basics of Cancer Treatment:There are mainly three types of cancer treatment modalities: surgery, chemotherapy and radiotherapy. Each type of cancer is treated with these three types of treatment as single modalities in early stages & in various combinations in later stages based on evidence based national and international guidelines. 2) Tumor Board Definition (what is Tumor Board?): A cancer treatment planning with multidisciplinary approach, in which a number of expert cancer doctors from different cancer specialties (disciplines) review and discuss the clinical condition and treatment options of a cancer patient. 3) Members: Each tumor board includessurgical oncologist (who provides cancer treatment with surgery), medical oncologist (who provides cancer treatment with medicines) and radiation oncologist (who provides cancer treatment with radiation), pathologist, radiologist, hematologist, nuclear medicine specialist and researchers. Generally, depending upon institute – organization size and expertise available, there may be more or less specialist in tumor board. It is important to understand that all experts have gone through the structured super-specialist training in various oncology disciplines.

4) Need of Tumor Board (Why Tumor Board is needed?): It should be noted that advancement in diagnostic technology, operative procedures and cancer research has led to exponential evidence growth for clinical management. Just like all specialties in medicine field, It is ever evolving science, thus treatment options for each cancer patient are changing based on available evidence. Generally, based on Tumor, Nodal status and Metastasis (TNM) status of the patient, each cancer patient is treated as per existing national and international guidelines. We also need to consider the patient factors as well as infrastructure facilities & expertise available.However, for few cancer patients (cancer conditions) there is no available evidence in terms of what is the best treatment option. It may also include the preference for treatment option. For example, what should be the preferred treatment for various recurrent tumors, should chemotherapy/radiotherapy given first then surgery option or vice versa. Same is true for various advanced local cancer tumors. All these kinds of cases, where lack of evidence (of treatment options) exist are discussed in tumor boards. Some early cancers are equally curable by two different modalities & guidelines also suggests the use of any one. In these kind of situations tumor board can suggest the best possible modality for that particular patient as there will be unbiased discussion by both modality specialists discussing the pros & cons of the treatment option. E.g. Early vocal cord cancer is equally curable by radiation as well as by endoscopic laser treatment. Which patient should receive what can best be discussed in tumor board and the pros & cons should be presented to the patient for his active participation in choosing the modality for cure.5) Functioning:Generally each tumor board meets once in a week. The tumor board meeting lasts for 1 to 2 hours, depending upon number of clinical cases discussion. The cancer specialist (either of surgeon, medical oncologist, radiotherapist) presents the detailed clinical status of cancer patient. That includes clinical features including specific clinical pictures or endoscopy videos, investigations work up including images of X ray, CT, MRI, PET CT etc, confirmed histo-pathological diagnosis, treatment history and current clinical status and general condition of patient. A through detail of previous patient treatment if given is presented to all members.Thenafter, detailed discussion regarding the further treatment options are discussed by all experts. options are discussed.Based on discussion, further treatment plan chalked out and options are given to patient, which is not only the best available option but also multidisciplinary evidence based decision. 6) Advantages: It works best not only for the patients but also for the busy oncology doctors. The tumor board treatment plan is best for patient because it is based on combined clinical expertise of surgical oncologist, medical oncologist and radiation oncologist, radiologist, nuclear medicine specialist,pathologist, hematologist and researchers. It is helpful to doctor also because they can refine and continuously expand their knowledge base, promotes teamwork based on unique expertise of team members. The opinion of second specialist doctor for cancer treatment is advisable and it is standard procedure in many of developed countries. The second opinion is given by one specialist while cases where evidence of best treatment options is not known, are discussed in multidisciplinary tumor board.

FRACTIONAL FLOW RESERVE Current imaging modalities for quantification of Coronary Artery Disease have many shortfalls and hence the need for newer modalities like FFR which can cover the grey zonesLimitations of Coronary Angigraphy include :
• Intermediate lesion (40-70%) significance cannot be accurately determined
• Does not provide intraluminal detail or vessel wall details ; • Diffuse disease, angiographic artifacts of contrast streaming,
image foreshortening and calcification often complicates the interpretation ;
• Bifurcation or ostial lesion locations may be obscured by overlapping branch segments.
TMT , SPECT indicate ischemia in patients with MVD but fail to distinguish the specic ischemic territories and responsible stenosesFractional Flow Reserve (FFR) measurement developed by Pijls et al has emerged as a potent tool for overcoming the above limitations
• It is defined as the Ratio of coronary pressure distal to the stenosis to that of aortic pressure as the normal perfusion pressure (distal coronary pressure/aortic pressure) when the microvascular resistance is minimal and assumed to be constant (ie at maximal hyperemia).
• FFR model assumes that under maximum arterial vasodilation, the resistance of the myocardium is minimal and constant across different myocardial vascular beds and thus blood flow to the myocardium is proportional to the driving pressure (myocardial perfusion pressure).
• It is currently considered Gold standard for invasive assessment of physiologic stenosis signicance

Interpreting FFR :The ratio determines the physiologic significance of the stenosis and its consequences for the patient, no matter what the stenosis looks like on angiographyFFR is not influenced by changing hemodynamic conditions (heart rate, blood pressure, contractility)
• Normal value = 1.0 for every patient and every artery• An FFR of 0.90 equates to 90% of normal maximum flow.• An FFR of 0.80 equates to 80% of normal maximum flow• Ischemia detection : <0.75• Defer angioplasty : >0.80• End point of angioplasty/stenting : >0.90• FFR < 0.75 suggests inducible myocardial ischemia [ 88%
sensitivity, 100% specificity, 100% positive predictive value and overall accuracy 93%]
• FFR < 0.75 Intervention is appropriate and results in significant improvement.
• FFR > 0.75 No benefit from intervention, neither in terms of functional class nor in terms of adverse events.
Interpreting FFR during Interventions :FFR <0.75 suggestive of unsuccessful PTCAFFR 0.75- 0.89 suggests a moderate result after Balloon AngioplastyFFR >0.90 suggests excellent result post angioplastyFFR >0.94 suggestive of optimal stent deployment
What Does it mean to the treating clinician ?• FFR should be measured before a decision is made either to
perform PCI or send the patient to surgery, in patients who come to the cath lab without a prior functional test and with a stenosis(es) 50-90% by angiography. (Class I recommendation , Level of Evidence A)
• This is regardless of whether the patient has single-vessel disease, multivessel disease, or if the vessel is especially important, eg. proximal LAD or LMCA. By changing the FFR from e.g. 0.50 to 0.95 by stenting the affected lesion then you are increasing blood flow from 50% to 95% of normal maximum flow

Hormones or Hormone Replacement Therapy does not work
There are several lifestyle changes women can make to reduce their risk of
heart disease:
· Exercise 30 to 60 minutes a day on most days of the week.
· Maintain a healthy weight.
· Quit or don't start smoking.
· Eat a diet that's low in saturated fat, cholesterol and salt.
You'll also need to take prescribed medications appropriately, such as blood
pressure medications, blood thinners and aspirin. And you'll need to better
manage other conditions that are risk factors for heart disease, such as high
blood pressure, high cholesterol and diabetes. Some women at high risk of
heart disease may also benefit from the use of supplements, such as omega-3
fatty acids

ndEvery year since 2006 every 2 Thursday of March is celebrated as World thKidney Day. The aim of this 9 world kidney day is to spread awareness in
general population as well in doctors about kidney diseases; the incidence of which are increasing tremendously.Since 2006 WHO with National kidney foundation different themes have been used to spread awareness. So this time the theme is “ KIDNEYS AGE; JUST LIKE YOU!” ; mainly to spread awareness about chronic kidney disease (CKD): so let us get some knowledge about CKD.Instead of just formal discussion; let us discuss this topic in question and answer form.
1. Is chronic renal failure(CRF) and chronic kidney disease(CKD) are same?
No. Terminology of CRF is now outdated and every doctor should use CKD instead of CRF. Any disease is chronic if it is there in the body for > 3 months.
CRF means : fall in GFR (glomerulo filtration rate) to less than 60 ml/min for >3 months. Now if patient is having normal GFR e.g. in diabetic GFR may be >100 ml/min ; but urine is showing persistent protienuria ; then patient is suffering from diabetic nephropathy. So according to definition he/she is not having CRF (normal GFR); but according to CKD definition he/she falls in CKD stage I.
So either or these two criterias ; one would be sufficient to diagnose CKD.1. Is s.creatinine is suffeicient marker for diagnosis of CKD. No. s.creatinine depends on muscle mass of a person. So s.creatinine of
1.5 mg/dl may be normal for musculine patient but it may represent renal dysfunction for thin lean person. That's why always measure kidney function with GFR.
GFR can easily calculated by each and every doctor by using simple calculators with COCKCROFT –GAULT formula ( requies patients age; weight and s.creat).

2. Can raised s.creat differentiate acute kidney injury from chonic kidney disease?
No. we can differentiate acute from chronic by following means; Absolute indicators : • USG : kidney sizes <8.5-9.0 cm(adults)• High phosphorus with low calcium and high parathyroid in serology.• Fundus showing diabetic ratinopahty in diabetics with protienuria.• Previous report of s.creatinine in last 3 months.• X-ray : wrist / spine showing ostodystrophy• In children growth retardation. All others : including history and physical examination are supportive
evidence but not confirmatory.3. If s.creat is high which kidney is affected right or left ? When s.creat is raised : it means both kidneys are affected . Even if single
kidney is normal s.creat would be normal; and that's why kidney transplantation are successful surgery ; both donor and recipient will have single , normal functioning kidney!
4. What is ESRD : End Stage Renal Diesase? a. Azotemia - elevated blood urea nitrogen (BUN >28mg/dL) and
creatinine (Cr>1.5mg/dL) b. Uremia - azotemia with symptoms or signs of renal failure c. End Stage Renal Disease (ESRD) - uremia requiring transplantation

6. What is most common cause of ESRD in India?
DIABETES IS MOST IMPORTANT CAUSE.1. Which is most common symptom of CKD? Most common symptom is patient may be totally asymptomatic ! so;
screening of person at high risk (long standing uncontrolled DM/ Hypertension/analgesic abuse/family history) may be helpful. Multiple symptoms and signs can be there but nothing is specific to this disease!
2. Is chronic kidney disease : reversible ? Numbers of nephrons in each kidneys at birth are 1.0 million. Till age of
30 years it remain same and then decreases. i.e. kidney does not regrow like hair and skin. So world across available treatment for CKD is just for prevention of progression from one stage to other ; treament of complications and preparation for future definitive therapy. There are certain instances in which s.creatinine decreases : that is actually recovery of acute componant on top of chronic kidney disease! CKD patients are prone to get superadded acute kidney injury.
3. When to get tested? Type 1 Diabetes: 5 years after diagnosis, then annually*
Type 2 Diabetes: at diagnosis, then annually* Hypertension: at diagnosis and initiation of therapy, then every 3 years if
eGFR and microalbumin tests are normal Family history of kidney disease: every 3 years, as long as tests are
normal. *KDOQI Guideline 4. What as a doctor we can do at diagnosis of CKD ?· Explain patient about nature of disease· Factors responsible for progression(DM ; Hypertension and proteinuria)· Staging at dianosis by estimating GFR ; rather relying on s.creatinine.· Hepatitis B vaccination ( after polio hepatitis B should be the target for
atleast partial/ complete irradication!) 5. What are recommandation for management of early CKD stage (1-3)· Lifestyle changes
· Smoking: Recommend smoking cessation· Diet : Sodium intake <100 mmol (2·3 g) per dayo Consider oral sodium bicarbonate supplementation if acidotic· Weight: Body-mass index <25 kg/m²; waist circumference <102 cm for
men and <88 cm for women· Exercise: When feasible, 30–60 min of moderate intensity dynamic
exercise (walking, jogging, cycling, or swimming) 4–7 days per week· Hypertension· Treatment goal:• Recent guideline suggest
• Non- Proteinuric : BP goal:≤140/90mm Hg
• Proteinuric CKD:(≥30 mg/24 hours) BP goal: ≤130/80 mm Hg.• Pharmacotherapy: • Proteinuric chronic kidney disease : ACE inhibitoror an angiotensin-
receptor blocker• Non-proteinuric chronic kidney disease : any / combination of
antihypertensive can be used ; depending underlying clinical status.Diabetes mellitus• Treatment goal: HbA1c about 7%.• Pharmacotherapy:• Metformin acceptable for stage 1–2 chronic kidney disease, and stable
stage 3 chronic kidney disease• Repaglinide acceptable with no dose adjustment• Short-acting sulphonylureas (eg, gliclazide) are preferred over long-
acting agents• Sulphonylureas and insulin need dose adjustment To conclude, CKD is a problem of epidemic proportions in India, and with
an increasing diabetes burden, hypertension, and growing elderly population it is going to increase even further. In a country like India, where screening and prevention of CKD still has to take a start, screening of high-risk individuals for CKD and its risk factors is the best bet.
