JOINT AFFIDAVIT OF TWO (2) DISINTERESTED PERSONS

We, JULIO Q. CAGADOC and DIOSDADA E. PEDRIGAL, both senior citizens, Filipinos and long time residents of Avenida Veteranos, Tacloban City , after being first sworn according to law, hereby depose and state: THAT---

1. We personally know a female person by the name of ANA MARIE MONTHERMOSO ALMERIA of 248 Avenida Veteranos, Tacloban City;

2. We also know that said ANA MARIE is the eighth child of the Sps. CRESCENTE C. ALMERIA and LEONOR MONTHERMOSO ALMERIA, and was born in Tacloban City on 26 October 1964;

3. We also know that ANA MARIE MONTHERMOSO ALMERIA was baptized in the Bishops Chapel in Palo, Leyte, evidenced by a copy of her Certificate of Baptism & Confirmation; we later learned that she has been married to Alfredo Lozano in 1994;

4. We were made aware that her FIRST/GIVEN NAME was not supplied in her Certificate of Live Birth. Also, we observed an innocuous clerical error in her MIDDLE/MATERNAL SURNAME, which should be spelled as MONTHERMOSO instead of MONTERMOSO.

5. We know that said person has always used the first/given name ANA MARIE in all her school records and official dealings, evidenced further by her SSS ID CARD No. 03-8759041-1 and PRC (MEDICAL TECHNOLOGY) ID No. 0016854.

6. We are not related by blood or affinity with the MONTHERMOSO-ALMERIA, but we fully know the said facts of the birth, baptism and schooling of ANA MARIE since we were close neighbors in Avenida Veteranos, Tacloban City at the time said ANA MARIE was born until she grew up to be a lady, and we were often invited as friends in their important family affairs;

7. We are executing this Joint Affidavit to attest to the truth of the foregoing facts of birth and personal data about ANA MARIE MONTHERMOSO ALMERIA for all legal purposes and intents.
