Allstate Benefits with Taylor Insurance Enrollment Assistance Guide

To access your Voluntary Benefits with Allstate, log in to the SGMC Benefits Portal by visiting https://my.adp.com. On the Home

Page of the SGMC Benefits Portal, click on Additional Benefits.

On the Additional Benefits page, you will see all of the offered Allstate benefit plans, including the plans you are currently enrolled

in with your deduction amount(s).





To access these benefits to modify, cancel, or add benefits, click on the Change or Learn More link on any one of the options.

If you do not want to make any modifications to your current coverage, you do not have to make any changes to these benefits.





You will be redirect to an external site to enroll in these benefits. Click OK.

The Benefits Selection site will open in new window. Please ensure your pop-up blocker allows this pop-up.

Your Login ID is your Employee ID Number and your PIN is the Last 4 Digits of your Social Security Number (SSN) + the Last 2 Digits

of your Birth Year.

Click Log In.

Select the appropriate Plan—South Georgia Medical Center 2016.

You will be able to see any enrollments that you have on the home page after logging in to the Allstate Benefits Selection system.

You will also see all of your Available Enrollment Options—Accident, Cancer, Critical Illness, SHOP, Disability, and Universal Life.

Click Next to begin enrollment for your Allstate Benefits.





You will be able to see any enrollments that you have on the home page after logging in to the Allstate Benefits Selection system.

To add, decline, or modify a benefit, click Review.

Click Next after choosing your benefit elections to complete your Allstate Benefit Enrollment.





After selecting or modifying your Allstate Benefit selections, you will need to enter your PIN to sign off on the enrollment.

Click Next after choosing your benefit elections to enter your PIN and submit your Allstate Benefit Enrollment.





Enter your PIN to sign the form.

Click Sign Form after choosing your benefit elections to enter your PIN and submit your Allstate Benefit Enrollment.





When you arrive at this page, your Allstate Benefits Enrollment is complete and has been submitted.

You can print your confirmation by clicking on Enrollment Confirmation.

Click Back to modify your Allstate Benefits Enrollment, or click Return to go back to the home screen of the Allstate Benefits Selec-

tion Portal.
