Het gepatenteerde en integrale zitconcept AKTIVLINE bestaat uit een zit unit en rugleuning met biomechanische draaipunten die met behulp van glijstangen bevestigd zijn aan een aangepaste rolstoel. The patented, integral seating concept AKTIVLINE con-sists of a seat and back unit with physiological pivotal points, connected by means of sliding rails on an adapted wheelchair. AKTIVLINE offers patients physiological and dynamic seating with simultaneously low strain on body and material. AKTIVLINE contributes to both an improved sense of well-being on the part of patients as well as to their integration into society. AKTIVLINE is used wherever the previous form of care for children, young people and adults with cerebral movement impediments is unsatis-factory and problematical.

In particular children, young people and adults with infan-tile cerebral paresis, combined with spasms in extension or with tetraplegia and dystonic spasms, can benefit from AKTIVLINE. With patients within this framework of indi-cations, it is frequently possible to observe that no good sitting position is achieved, resulting pain reduces the sitting time or involuntary movements render for instance eating and swallowing or the operation of certain devices impossible. Frequently, the seat shell or the wheelchair also breaks, because the forces occurring cannot be absorbed.

Vooral kinderen, jonge mensen en volwassenen met in-fantiele cerebrale parese die gepaard gaat met spasmen in extensie of met tetraplegie en dystonische spasmen kunnen gebaat zijn bij de AKTIVLINE. Bij patiënten die binnen dit kader van indicaties vallen is vaak te zien dat er geen goede zithouding wordt bereikt, dat pijn die ontstaat de zitduur verkort of dat onwillekeurige bewegingen onder meer het eten, slikken of het bedienen van bepaalde appa-raten onmogelijk maken. Vaak breekt ook de zitschaal of de rolstoel, omdat de optredende krachten niet geabsor-beerd kunnen worden.

Fysiologisch, dynamisch zittenDoor het individueel aanpassen van de zitunit en rugleu-ning kan de patiënt, op basis van de eigen mogelijkheden, een orthopedisch juiste zithouding aannemen. In ont-spannen toestand bevinden zithouding, knieën en voeten zich ieder in een hoek van 90°. De anatomisch individuele polstering van de zitschaal houdt rekening met de fysio-logie van de patiënt. Het volledige bewegingsverloop van de schaal wordt aangestuurd met behulp van biomecha-nische draaipunten bij de heupen en knieën. Doordat de zitunit en rugleuning op glijstangen zijn bevestigd, kan de heuphoek geopend worden, afhankelijk van het bewe-gingsverloop van de patiënt. Tevens staan de kniehoek- en voetplaatunit de dynamische strekkingstendens van de patiënt toe. Een gasveer-rugverstelling in zowel de zitting als het beengedeelte brengt de patiënt automatisch terug in de fysiologische uitgangspositie.

Door ruimte te geven aan de natuurlijke bewegingsdrang en het eigen bewegingsverloop kunnen de beweeglijke li-chaamsdelen mobiel gehouden worden. Ongecontroleer-de strekbewegingen van de patiënt worden gecontroleerd toegestaan.

Physiological, dynamic sittingThe assumption of an orthopedically correct sitting posi-tion in line with the patient’s capabilities is made possible through the individual adaptation of the seat and back units. When relaxed, the sitting angle as well as the foot and knee angles are at 90°. The anatomically-individual upholstery of the seat shell takes account of the phys-iology of the patient. The entire movement sequence of the shell is controlled via physiological pivotal points in the hips and knees. Seat and back units are mounted on sliding rails and enable the opening-up of the hip angle in accordance with the movement sequences of the patient. Knee-angle and footrest unit simultaneously permit the dynamic extension tendency of the patient. A readjusting spring system in both the seat as well as the leg system automatically returns the patient to the physiological starting position.

By allowing the natural urge for and sequence of move-ment, it is possible to keep the moving parts of the body mobile. Uncontrolled extension movements of the patient are permitted in a controlled manner. The position of head and shoulders in space will change minimally with maximal stretching of the body and chair. The orientation will be preserved. Activities like watching, drinking or operating a joystick aren’t disturbed. Vestibular and visual perceptions are stable.

Verminderde belastingDoor de dynamische bevestiging van de zitschaal op de rolstoel worden de optredende krachten geabsorbeerd. In tegenstelling tot de gebruikelijke vaste zitschalen met een rigide zithouding verandert de AKTIVLINE de zit- en beenhoekpositie als er kramp optreedt en past het sys-teem zich automatisch aan de strekking van het lichaam aan. De krachten die ontstaan worden niet rechtstreeks op het lichaam en het materiaal overgebracht, maar door de toegestane individuele bewegingsradius opgevangen. De gedetailleerde, stabiele constructie van zitsysteem en onderstel is eenvoudig te bedienen en kan in velerlei opzichten aan de patiënt en de begeleider aangepast wor-den. Door de uitgebreide techniek heeft de rolstoelunit een niet gering totaalgewicht, maar dat kan op individuele basis verlaagd worden door het weglaten van diverse afzonderlijke onderdelen. Het gebruik van de AKTIVLINE heeft evenwel een belangrijk voordeel voor verzorgers en ouders: de zithouding van de patiënt hoeft niet meer voortdurend handmatig gecorrigeerd te worden.

Verbetering van het welzijnDoor de AKTIVLINE kunnen patiënten doelgerichte handelingen verrichten, zoals spreken, slikken of zelfs het bedienen van een computer. Onwillekeurige bewegin-gen, bijvoorbeeld als gevolg van plezier, schrik of ande-re verstoringen van het natuurlijke bewegingsverloop, worden door de beweeglijkheid van de zitschaal geabsor-beerd. Hierdoor kan de concentratie gericht blijven op de gewenste doelmatige beweging. Stevig vastzetten en een starre zithouding leiden tot een verhoogde spierspanning en daardoor tot een gebrek aan welzijn. De zitduur wordt korter. Bewegingen zorgen ervoor dat spierspanning zich niet verder opbouwt en maken zodoende een langere zitduur mogelijk.

Reduced burdensThe dynamic bearing arrangement of the seat shell on the wheelchair absorbs emerging forces. In contrast to conven-tional, rigid seat shells with a rigid sitting position, AKTIV-LINE alters the seat and leg-angle position in the event of cramp situations arising and automatically adjusts to the body extension of the patient. Emerging forces are not trans-mitted directly to body and material, but rather are absorbed by the admissible individual movement radius. The detailed, stable construction of seating system and undercarriage is simple to operate and can be adapted to the patient and accompanying person in many ways. Due to the comprehen-sive technology, the overall weight of the complete wheel-chair unit is not inconsiderable; nevertheless, this can be reduced individually through the omission of various individ-ual components. For carers and parents, however, the use of AKTIVLINE offers one decisive advantage: there is no need for permanent correction of the patient’s sitting position.

Verzorging met een gebruikelijke, vaste zitvoorziening is door gebrek aan acceptatie vaak niet mogelijk of zeer moeilijk te verwezenlijken. Met name de hoge drukkrach-ten die bij een conventionele zitvoorziening ontstaan, zijn onverdraaglijk voor de patiënt.

Daarnaast verhoogt een gewone zitvoorziening de kans op het optreden van: heupluxaties, pijnlijke heupen, spierver-korting, secundaire schade ten gevolge van immobiliteit. Spiercontracties of afname van de beweeglijkheid van gewrichten.

De dynamische zitschaal van het AKTIVLINE-systeem maakt het automatisch wisselen van verschillende lichaamshoudingen mogelijk en bevordert zodoende de ademhaling, de bloedsomloop, de vertering en de beweeglijkheid van de lichaamsgewichten en helpt het ontstaan van drukplekken voorkomen.

Care through the provision of a conventional, rigid seat shell is frequently not possible or very difficult to achieve, due to a lack of acceptance. In particular, the high pres-sure forces, resulting from conventional sitting care, are impossible for the patient to bear.

In addition, conventional sitting care increases the risk of the occurrence of: hip luxation, painful hips, shortening of the muscles, secondary damage as a result of immobility, muscle contractions or reduced mobility of the joints.

The dynamic seat shell of the AKTIVLINE seating system enables automatic changing between various body pos-tures and thus encourages breathing, the cardiovascular system, digestion, mobility of the body joints and helps prevent the development of pressure sores.

Als we een groep kinderen gadeslaan die tijdens een schoolles zit of een bioscoop bezoekt, dan wordt het ons duidelijk dat het begrip “rustig zitten” meer een wens is dan de werkelijkheid. Iedereen heeft ook bij het zitten wat beweging nodig om de eigen houding te verbeteren, de te lang belaste lichaamsdelen te ontlasten en de gewrichten beweeglijk te houden. De AKTIVLINE is dé intelligente oplossing voor alle kinderen en volwassenen met een cerebrale bewegingsstoornis die belangrijke bewegings-mechanismen niet zelfstandig kunnen beheersen. Door de dynamische verbinding tussen de verschillende onderde-len van de zitunit biedt dit systeem een eveneens beweeg-lijke zitschaal. What does “dynamic seat shell” mean?

If we observe a group of children sitting during a school class or in a cinema, we realise that the term “sitting qui-etly” is more of a wish, which in no way corresponds to re-ality. Everybody needs some movement even when sitting, in order to improve their own posture, to relieve parts of the body subjected to an excessive period of strain and to keep body joints mobile. AKTIVLINE is the intelligent solu-tion for all children and adults with a cerebral movement impediment, who cannot control important movement mechanisms independently. Through the dynamic linking of the various parts of the seat unit, it offers correspond-ing movement of the seat shell. This mobility is adapted to the individual requirements of the user by means of pull and push suspension, with the result that a good sitting position in the interests of therapeutic care in no way interferes with pleasant sitting.

Welke voordelen heeft de AKTIVLlNE voor de gebruiker?

Veel mensen die zich moeilijk doelgericht kunnen bewe-gen of van wie de houding grotendeels door ongecontro-leerde strekbewegingen worden beïnvloed, kunnen baat hebben bij verzorging met de AKTIVLINE zitschaal. In het verleden heeft men geprobeerd de onwillekeurige bewe-gingen, die het zitten om verschillende redenen negatief beïnvloeden, door middel van een vastgezet systeem te beteugelen. Een dergelijke oplossing heeft het pro-bleem echter alleen maar weggestopt. De spierspanning, vooral in het gebied van heupen en romp, kon zelfs hoger worden. Iedereen weet wat de gevolgen zijn: de zitschaal brak vaak of vervormde, de patiënt gleed uit de zitschaal, het gebruik van klossen of zitbroeken tussen de bovenbe-nen leidde tot extra druk op de spieren die verantwoorde-lijk zijn voor luxatie van de heup. De AKTIVLINE wil deze ongewenste gevolgen verhelpen en zelfs de mogelijkheid bieden de beschreven bewegingen deels te controleren of voor een nuttig doel te gebruiken. De druk- en trekvering laat de onwillekeurige bewegingen in beperkte mate toe. Als de spieren weer in ontspannen staat zijn, keert de zitschaal vanzelf in de beginpositie terug. Zo wordt tege-lijkertijd een fysiologische en dynamische zithouding be-reikt. Bovendien is de weerstand tegen de strekking niet - zoals bij een conventionele verzorgingsmethode - meteen maximaal, maar pas tegen het einde van de beweging.

Dat betekent dat de spieren de maximale kracht niet in verkorte toestand opbouwen (gevaar voor contracties). Gevolg: de waarneming van de eigen lichaamshouding wordt merkbaar beter en de kans bestaat deze gedeelte-lijk te verinnerlijken en te beheersen.

What advantages does AKTIVLINE offer the user?

Many people with problems in terms of targeted move-ment or whose posture is influenced through mainly uncontrollable extension movements, can only benefit from care with the AKTIVLINE seat shell. In the past, attempts were made to control the involuntary move-ments, which for various reasons had a negative effect on sitting, by means of firm fixing. However, this type of solution only made the problem worse. The muscle ten-sion, in particular in the hips and trunk, even increased. The consequences are common knowledge: the seat shell often broke or became deformed, the child slid out of the seat shell, the use of wedges or seat pants between the thighs caused increased pressure on the muscles, pressure which is responsible for the luxation of the hips. AKTIVLINE aims to remove these undesired consequenc-es, and even to offer the possibility of, in part, controlling the movements described, or of using them for a sensible purpose. Thanks to the pull and push suspension, the involuntary movements are permitted to a limited extent. Once the muscles have returned to a relaxed condition, the seat shell turns back to the initial position by itself. Si-multaneous physiological and dynamic sitting is achieved. Additionally, the resistance to extension does not – as is the case with conventional forms of care – take maximum effect immediately, but rather not until the end of the movement.

This means that the muscles do not develop maximum force when in a shortened condition (risk of contraction). The result: the perception of one’s own body posture is noticeably improved; the possibility exists of in part inter-nalising and controlling it.

Which children and adults will benefit most from this type of AKTIVLINE care?

Movement impediments as a result of spastic paralysis (infantile cerebral paresis) can be observed in a wide variety of forms: on the one hand, various parts of the body can be affected (hemiparesis, diparesis, tetrapare-sis) and, on the other hand, movements themselves can be deficient (akinetic and hypokinetic) or not targeted (diskinetic). The perception component is always of major importance for people in the organisation of their move-ments. Children with tetraplegia are frequently incapable of controlling their posture and quickly become “fright-ened” as a reaction to various forms of irritation. An alternative form of care should be tried if over-extension of head, trunk and hips is observed, if movement of the hips is more or less bearable and if almost exclusive use is made of a wheelchair.

Nevertheless, even children who are unable to sit for more than a short time as a result of hip pain, will also feel benefits. For diskinetic movement impediments, AKTIVLINE offers the only alternative to conventional care. The main advantage lies in the fact that movement components, which disturb target-oriented actions (e.g. speaking, operating a computer), are absorbed by the movement of the shell.

Welke kinderen en volwassenen zijn het meest gebaat bij een dergelijke AKTIVLINE-verzorging?

Bewegingsstoornissen als gevolg van spasticiteit (infan-tiele cerebrale parese) zijn in veel verschillende uitings-vormen waarneembaar. Enerzijds kunnen verschillende lichaamsdelen getroffen zijn (hemiparese, diparese, tetraparese) en anderzijds kunnen bewegingen zelf ook gebrekkig (akinetisch en hypokinetisch) of niet doelge-richt (diskinetisch) zijn. Het waarnemingselement voor mensen bij de organisatie van hun bewegingen is altijd van groot belang. Patiënten met een tetraplegie zijn vaak niet in staat hun houding te beheersen en “schrikken” snel bij verschillende prikkels. Er moet een andere vorm van verzorging uitgeprobeerd worden als er overstrek-kingen van het hoofd, de romp en de heup waar te nemen zijn, de beweging van de heup min of meer draagbaar is en er vrijwel uitsluitend gebruik wordt gemaakt van een rolstoel.

Patiënten die vanwege een pijnlijke heup niet lang kunnen zitten, zullen echter ook voordeel hebben. De AKTIVLlNE is – naast de gangbare verzorging – het enige alternatief bij diskinetische bewegingsstoornissen. Het voornaamste voordeel van dit systeem is dat bewegingscomponenten die een doelgerichte handeling verstoren (bijv. spreken, het bedienen van een computer), door de beweeglijkheid van de schaal geabsorbeerd worden.

I am not happy with my rigid seat shell!

Naturally, one should not change satisfactory care through a rigid seat shell under all circumstances. It is known just how long the route to good care can be in some cases. However, if you are not satisfied despite long years of experimenting, because the seat shell frequently breaks, a good sitting posture is not achieved, because pain shortens the sitting period or because involuntary movements have a negative effect on the operation of various devices, then a trial with AKTIVLINE is a recom-mendable step. It will very soon become apparent wheth-er the advantages of the “dynamic care” are accepted by the user and his/her environment.
