Passive Acoustic Monitoring (PAM)Use: Whales and dolphins. Conventional is an independent system, alternatively they are built into the existing seismic receiver system or source. Personnel may be independent operators or existing seismic crew members. Pamguard is the industry standard for software. PAM has some limitations, e.g. need vocalization, low Hz calls masked by ship noise, high Hz clicks attenuate, accurate location difficult, only as good as equipment and operator. Improvements are in hardware, software, and training. API develops PAM guidelines.

Statistics aren’t open to public.Are companies expected to use this system for incident reporting? (WG) – No, submit annual statistics as currently done. Road Safety (in USA)Electronic Logging Device (ELD) Mandate: Dec 18, 2017 ELD’s required for commercial motor vehicles. Only for interstate federal regulations. 2000 or older engines not in play, some people putting in older engines. Hours of service have not changed, only the logging. In Texas, Dec 16, 2019. Use dot website checklist before you buy one to verify you have one that meets requirements.Q: Does ELD record speed or speeding like IVMS device? No. This is likely a natural progression of the program, and may include speed limiting in the future. Drug testing panels are also changing to include opiates. One day data could be useful in incident investigation. Legislation about Speed Limiting Devices has been postponed by current administration.

Rescue at Height and Confined Space

There has been little improvement in the last ~decade in reducing the fatalities from falling in the USA, around 4500-5000/year. Most deaths are from relatively low heights ~10-15 ft.

400-500 deaths per year due to confined space (mostly asphyxiation). There were two cases presented regarding people entering a confined space that had bad air.Suspension trauma can start after 2 min.There are many additional casualties particularly in confined space that happen during the rescue. A rescue plan is important. Sharing lessons from accidents to learn is also key.

Question/comments from audience: The use of self-rescuing equipment to avoid

harness/compartmentalization injuries from hanging in a harness. These systems work but are not widely known about. Also expensive to implement and replace on a regular basis.

Step ladders: no clear way to avoid this risk, try to use a different method if possible. Always tie off anyway.

Several different standards to define confined space. Key characteristics - no natural access, any place you have trouble entering/exiting. Can be a container and so on. Permit vs non-permit required definitions.

Issue brought up from audience: getting security plan prior to / included a bid/RFP. Vague - “Security contractor shall work up to the requirements of the location.” HSE and security have not been well linked up historically – a place to improve. Specify security requirements as much as possible in the RFP. Takes time, often pressure is on the contractor post award, which is not correct. Cost of security should not ever be a commercial differentiator. The cost of doing it properly is the same for everyone.

Camp may be often secure but weak areas are often the logistics / supply trips etc. The question should be “could the camp be attacked by armed bad guys?” if so, then must be hardened all the way, or will be attractive.

Comment from audience: local nationals often provide the best information as warning for an attack, but the opposite is also true. Insiders may provide the attackers the information about security.

Comment from audience: attractiveness. Maybe not about nice vehicles or equipment but often having expatriates to take hostage may be the biggest attractor. Using locals decreases the attractiveness. Crews that are cash paid attract attacks, try to avoid. Robberies from camp cash of $100k+ example given.

Basic Aviation Risk Standard (BAR)

BARS is meant to avoid the multiple, redundant audits of aviation providers by every end user. So, a standard is developed for the audit content, and each user agrees to the standard. A single audit is performed by BARS on each aviation provider annually. The audit results are shared in a central database to which the users have access.

BARS started in 2010. Issues found during audit are categorized in P1, P2, P3. P1 is a safety / regulatory violation. P3 is ‘nice to have’ but not required.

The majority of fuel non-conformities are from West Africa.Difference between IOGP and BARS, IOGP are recommendations, BARS is a standard. There are different standards for different categories of aviation activity, air born geophysical.

Question from audience: why haven’t more IOGP members participated? IOGP members already have an established standard and it would be a big deal to switch. Discussion continues and has been suggested to the IOGP Air Committee.Q: Pilots are ageing and not being replaced. Standards expect or demand a certain level of experience, which soon won’t be available in the industry. Comment from audience: Audit content is good but the problem is the old, young or generally unqualified pilots.

Comment from audience: Union from EU recommending not to take the SuperPuma. Presenter notes that it is ok to ride in the aircraft after the changes/updates that have been made.

Question from audience: SuperPuma has been in service a long time, why seeing problems now? Answer – it’s the newer models that have the big problems.

Comment from audience: there are a lot more marine near misses, but more incidents on land. Difference between land/marine near misses definition, near miss on land has to be closer to a HiPo. This is not strictly defined by IAGC. Inconsistent reporting.

HiPo presentations:Redacted company – workboat engines stopped after launch. Immediately after launch while moving away from seismic vessel both engines stopped. Drifted into leadin and stuck ~1min. Engine restarted and recovered safely. It was due to deactivation of fuel solenoid due to lack of batter power. Conclusion – improve inspection and charging of batteries, drill for scenario.Discussion – at what point should the coxswain jump from the boat.

Redacted company – explosion of nitrogen accumulator. North Africa joint venture. Vib sent back to camp for maintenance including refilling accumulators with nitrogen. Checked sweep quality. 10 mins after shutdown, exploded. Noise trauma. No one seriously injured. Debris up to 145m away. Conclusion – nitrogen tank was 38% oxygen from local source. Under pressure can cause explosion with hydraulic fluid. Actions - changed gas supplier. Improve supplier selection process.Discussion – not the first time this has happened around the world with the same cause. Findings same, actions the same. How to prevent again in the future? Enclose them in a guard? Test the bottles?

Redacted company – Auto accident 8-13-17. F350 jug truck side swiped by F150 (local person). No injuries but damage. 3rd party guy was looking at his girlfriend. Conclusion – you have no control over 3rd parties, drive defensively. Lots of traffic these days in Permian Basin, be aware.

Redacted company – UTV accident. 10-15-17. Driving UTV – Gator - (like a big gas-powered golf cart) spotting out equipment for layout. Didn’t see a ravine and drove off 5 ft drop. Minor injuries. Conclusion – in dense brush, ravine marked on map but not flagged off. Driver using handheld GPS. Needed to advise driver of hazard and flag, driver distracted by GPS.Discussion – should he be using a handheld while driving? Should stop to look at it, not while underway.Was cargo in the bed secured? No.

Redacted company – Trailer sway incident - Queensland – mobilizing from Oakey 5-9-17. 2014 Toyota Land Cruiser towing box trailer. McHitch being used. Trailer started swaying at 80km/hr, accelerated to stop it, sway increased, brake, roll over.Conclusions – poor weight distribution in trailer, speed too high for configuration. Issues identified by worker earlier but work was not stopped. Actions – trailer is no longer in use. Re-training sessions.Discussion – does trailer have brakes – yes electric.

drill ship DS-8 on KAOMBO field Block 32 offshore. Crane completely boomed up to lift gas rack. Block swung out of control hitting lower walkway. Aborted and went down to inspect walkway. Stepped on a piece of walkway that flipped up and grate fell 17.6m. Operator grabbed walkway but still fell 4.5m, broke leg.Discussion – grating is a problem in marine as well. They have maintenance of the rivets/screws as part of the 3 month scheduled work.Operator did not have enough training or handover.

Redacted company – Crewmember lost during hurricane. 3 vessels at same location in Bahamas for Hurricane Matthew. 2 vessels broken away from moorings. 3 crewmembers on the way to vessel were stranded in Miami. Once in Nassau, they had no money, ended up in a cabin drinking rain water, no electricity. After 4 days, they managed to rejoin vessel.Conclusion – lack of procedure to deal with situation.Discussion – similarly no one looked after Filipino marine crew guys during Katrina.

Redacted company – Man Overboard – crew member 3rd party on charter fell through an open pilot door. Was working alone at night. Warm water. Swam toward guard vessel, shouted and bridge door was open. They heard him and rescued, ended up in 1st aid case, but serious HiPo. Conclusion – had not identified lack of lone-worked procedure. Crew member not following known procedure (was throwing away garbage overboard), wasn’t wearing proper PPE to have pilot door open, no life jacket or harness. Whatever he was discarding caught on his clothing pulling him through.Discussion –

Were there issues prior to this incident with the company? No seems to be individual.

Was there lone-worker in the audit? It was not covered by the WGC audit content. In bridging document, there was no lone standard, I think.

What were consequences to worker? Not to be hired again, it is thought.

Goal to revolutionize land seismic design and operations with drones. Note that 15% of Total estimated reserves left to find are in foothills, tough to operate seismic. Papua New Guinea – extremely difficult to operate seismic. Currently 100% heliportable with heavy tree cutting. High HSE exposure. Envisioning 3D HD seismic using airships with reduced personnel. Drone dart deployment for planting receivers, which are disposable and biodegradable. Wireless transmission of the data back to base camp.

What is payload? Now 1kg, aiming to reduce by 75% What is battery life for dart recorder? 60 days. Comment – technology exists to fly drone for 24 hours. Are drones deployed from air ship? No, air ship does what

helicopter did. Drones are separate, go back to smaller basecamp to get more darts.
