Request for Laptop Replacement for Full-time Faculty/Staff

Requestor: _________________________________ Department:________________________________ Campus:__________

Phone:______________________________ Email:____________________________

This form is intended for requesting a laptop computer as part of the regular three-year computer replacement cycle. If replacement is desired outside of this cycle, please explain the nature of the emergency replacement under “Justification”. Reason for Laptop instead of a Desktop Computer: (See second page explanation of categories.) 1. Replacement of existing laptop 3. Required to perform college work at multiple sites 2. Documented use of computers or instructional 4. Special need technology in classroom. Please be sure that your justification fits one of the reasons described. Laptops for use in classrooms can be checked out from Media Services at your campus. Flash drives are relatively inexpensive and may be purchased for instructors by their department. If the “need” for a laptop can be accomplished by using a flash drive, your request will not be approved. Unlike a regular desktop replacement, any peripherals to accompany a laptop must be purchased by the individual or their department.

By signing the form below, you accept responsibility for having a laptop computer should it be approved. Under most circumstances, it is your personal responsibility to replace the laptop should it be damaged or lost. A stolen computer containing student or personal information might also result in violations of the Family Educational Rights and Privacy Act for which you could be held liable. Justification: ________________________________________________________________________________________________ ___________________________________________________________________________________________________________ ___________________________________________________________________________________________________________ ___________________________________________________________________________________________________________

___________________________________________________________________________________________________________ ____________________________________________________

Requestor Date

____________________________________________________ ____________________________________________________ Department Chair Date Dean Signature Date

Deans and Department Chairs: If reason #2 above is checked, then by signing this form you are acknowledging that the faculty member has regularly used computers in the classroom.

Submittal Instructions: The Dean or Dept. Chair may submit this form through intercampus mail to both Richard Smith (at HBC) and Roy Mosier (at SVC). Contact Roy Mosier at 223-1177 with any questions. A hard copy of this form must be submitted with signatures.

EXPLANATION OF CATEGORIES

1.) Replacement of existing laptop — Check this reason if you already have a laptop and are

requesting its replacement as part of the regular 3-year cycle. If you currently have a laptop and would like a desktop instead, simply call Roy Mosier at 223-1177. But chances are you would like a new laptop.

2.) Documented use of computers in classroom — Choose this reason if you already regularly use

computers in your classroom instruction. If you use specialized software, please include this in your justification. Admittedly, getting a laptop and projection cart from media services and writing your presentations to incorporate such technology is a significant undertaking. The CWTC is satisfied that those who are sufficiently motivated to have already incorporated such technology in their instruction will make good use of a personal laptop. However, such usage needs to be documented by the department chair and dean.

3.) Required to perform college work at multiple sites — Select this reason if you regularly

teach at multiple campuses or do college work, such as outreach programs, at multiple locations. This justification also specifically includes those who are effectively “on-call”, such as computer support personnel or those in administrative positions where they may be expected to get into the system to address a problem regardless of where they are. It does not include simply wanting to do schoolwork at more than one location, such as writing lecture notes at home.

4.) Special need — This category would include any other pressing need for a laptop that is not

covered by the categories above.
