AFFIDAVIT

I, DELLY P. NARVASA, of legal age, Filipino, widow and a resident of J Village, Visayan Village, Tagum City, Davao del Norte, Philippines after having been duly sworn to in accordance with law, hereby depose and say:

1. That I am the same person affiant of this affidavit;

2. That I am driver of a motor vehicle registered in my name, which vehicle is more particularly described as follows:

a. Make : TOYOTAb. Type of body : Wagonc. Plate no. : LHE 996

3. That on or about second week of March 2014 at about 9:30 Am, while the above described vehicle was parked beside New Bantayan Elementary School, Asuncion , Davao del Norte, it was accidentally scratched by the kids playing around the said vehicle;

4. That I am executing this affidavit to inform the authorities concerned on the veracity of the foregoing facts and for whatever legal purpose this may serve.
