AFFIDAVIT OF ABANDONMENT

I, MA. KATRINA Q. SARILI-ESPIRITU, Filipino, of legal age, married, and with residence and postal address at Flat 3, Building No. 19, Al Ittefaq Street, Fereej Bin Omran, Doha, Qatar, after having been duly sworn hereby depose and state:

1. That my first child Mikel Antonio S. Agagas was born out of wedlock on 12 May 2006 in Balanga City, Bataan, Philippines;

2.That my first childs father, Mennard E. Agagas, abandoned me and his child and has not communicated to us nor extended any financial support since January 2007;

3.That on 06 July 2009, I married Jayson de Leon Espiritu at the Orion Municipal Hall in Orion, Bataan, Philippines;

4. That out of the marriage was born, on 10 February 2010, Jaidyn Kaethe S. Espiritu in Orion, Bataan, Philippines;

5. That my spouse Jayson de Leon Espiritu, abandoned me and my child in June 2010 and since that time has not communicated with us nor extended any financial support;

6.That I am executing this affidavit to attest to the truth of the foregoing facts, particularly the neglect and desertion of my spouse, and in compliance with the requirements for my childrens travel abroad;
