PROGRAM DESCRIPTION

Overview:

• The University of Florida Head and Neck Surgical Oncology program sees more than 3500 outpatient cancer visits per year with 350-500 new patients treated surgically. This consists of the majority of head and neck oncology patients referred in North Central Florida. The head neck surgery team performs over 100 major head and neck ablative – free flap cases per year in addition to a robust number of endocrine, salivary gland and trans-oral cases.

• The fellowship is designed to train Otolaryngology-Head and Neck Surgery graduates in the art and science of head and neck surgical oncology and microvascular reconstruction. Fellows will be given responsibilities to maximize clinical skill and knowledge acquirement for advanced tertiary surgical treatment of head and neck cancer patients. Additional skills will be developed in research and teaching to produce well-rounded academic surgeons.

• The fellow will operate with the head and neck surgical oncology and microvascular surgery faculty on advanced ablative and reconstructive cases. For basic cases, the fellow will act as the supervising/teaching physician to the chief and junior residents. The fellow will be expected to work with the residents on peri-operative care for the head and neck patients on a daily basis.

Objectives:

The fellow will gain expertise in:• Multidisciplinary management of head and neck cancer patients• Head and neck mucosal tumor ablation• Head and neck skin cancer ablation• Skull base surgery• Endocrine surgery• Trans-oral surgery• Regional flaps• Microvascular free flap reconstruction

COMPONENTS OF THE FELLOWSHIP

A. Surgery:

Microvascular Free Flap / Reconstructive Surgery• The fellow will operate with faculty and receive increased independence as their

skill level increases. It is expected that the fellow will participate in a minimum of 80 free flaps including radial forearm, anterolateral thigh, fibula, scapula, latissimus dorsi as well as other less commonly used flaps.

• The fellow will become facile with regional flaps such as pectoralis major, submental and supraclavicular flaps.

Ablative / Skull Base Surgery• Fellows will participate in at least 300 major ablative and skull base cases. As the

skill level of the fellow increases they will be given opportunities to take residents through cases.

Transoral Robotic Surgery (TORS)• The fellow will participate in TORS cases for oropharyngeal and supraglottic

malignancies as well as carcinoma of unknown primary.

Endocrine Surgery• Fellows will participate in simple and complex thyroid and parathyroid cases.

Trauma• To enhance bony reconstructive skills, fellows will participate in and teach

maxillofacial trauma cases to residents.

B. Clinical:

Head and Neck Tumor Board• Fellows will attend a weekly multidisciplinary head and neck oncology tumor

board. This will facilitate complete treatment planning for head and neck cancer cases.

Fellow’s Clinic• The fellow will run an independent clinic ½-1 days per week. He/she may book

surgical cases from this clinic.

C. Research: • The fellow will be expected to complete a minimum of 2 projects with the

presentation and publication of results. The fellow will also have the opportunity to supervise resident and student run projects. Dedicated research time will be provided.

• It is expected that the fellow will present his/her work at a regional and a national meeting and to submit at least 2 first author papers for publication.

D. Teaching:• The fellow will be involved in the clinical and surgical teaching of residents and

medical students. He/she will also be involved in 2 formal didactic courses: (1) Head and Neck Anatomy for Residents and (2) Head and Neck Surgery for residents. He/she will also be given the opportunity to participate in AO Trauma courses.

The University of Florida Head and Neck Surgical Oncology/ MicrovascularReconstruction fellowship will provide fellows with robust and diverse training in tumor ablation, endocrine surgery, transoral techniques, regional flaps as well as a high volume of free flap reconstructions. Fellows can expect to graduate with over 80 free flaps with comfort in multiple types of soft tissue and bony flaps. Moreover, research and teaching opportunities will shape a well-rounded academic head and neck / reconstructive surgeon.
