Risk Assessment & Managing Complications in the Esthetic ZoneMany variables influence the implant treatment outcomes in the esthetic zone. Patient factors such as periodontal biotype and tissue display are critical in determining the potential for success when treating patients with implant dentistry. Additional considerations, including implant choice, surgical design and restorative design, are among other critical factors that are in the control of the treating dentist. Teeth may be extracted and implant placement may be delayed, immediate or even loaded. Surgical design may be flapless or involve a minimally traumatic design if grafting is desired. This presentation will outline the risk factors which most significantly influence esthetic prognosis in the esthetic zone. While many techniques offer excellent results in most situations few can be said to guarantee success. A series of cases have been gathered to illustrate risk assessment and predictable management of implant treatment in the esthetic zone.

Increasing Case Acceptance in a Change Economy 91% of US dentists feel they are being impacted by the current economic situation. Of these, 68% are noticing a decline in their case acceptance as a result. The dentists who reported they are NOT impacted report three reasons why: their enhancements to their patient communication, marketing and team development. — CRA, special report on the economy, Dr. Gordon Christensen

Course Synopsis: Dentistry is undergoing some of the biggest changes in history, and your choice to adapt will determine the success of your future. One of the biggest changes is how patients make decisions about spending their money with you at this time. 85% of your patients’ decision to say “Yes” is a direct reflection of how the dental team understands and meets their needs. Only 15% of their decision is related to your intellect and technical ability. It is critical that case presentation involves all members of the dental team and what better time to invest in providing them the tools and training.

Clinical Uses of the Waterlase MD and EZLase Diode LasersLaser dentistry has evolved since its humble beginnings in the 1960’s. Current all tissue lasers manufactured by Biolase can be considered “7th generation lasers”. Dr. Rubinger uses a Biolase Waterlase MD for entire or part of procedures on every patient in his practice. The uses of the Waterlase MD range from cavity preparations, composite removal, soft tissue removal and incision, bone removal and shaping, periodontal debridement and reattachment procedures, root canal preparation and disinfection and more… Recently, this technology has made some inroads into North American dental schools. Their use for a variety of procedures has been studied and the superiority of their results documented. This lecture will touch on laser science and show a multitude of everyday clinical uses of the Waterlase MD and EZLase diode lasers.

Dr. Tzvi Rubinger is a graduate of the University of Toronto Faculty of Dentistry 1985. He has been in a private practice since. Dr Rubinger practices all aspects of dentistry and provides a full range of “specialty” procedures. Dr Rubinger adopted Biolase laser dentistry in 2006 and has trained with many internationally renowned dentists and dental specialists. Dr Rubinger received a Mastership certification from the World clinical Laser Institute and is certified as a trainer in

Waterlase dentistry. He is currently the only dentist in Ontario to hold the above certifications.

Predictable Bone Grafting for Future Implant Placement One of the main criteria for having successful surgical results in implant dentistry is to have adequate bone. When inadequate bone quantity and quality is present certain forms of bone augmentation are required in order to facilitate future implant placement. This seminar will discuss predictable bone grafting techniques for different situations and areas of the maxilla and mandible. Clinical situations and cases will be shown to demonstrate these grafting techniques.

Mastering Success for Implant Supported Restorations for the Edentulous Patient With the successful introduction of implant dentistry the edentulous state can be remarkably improved, which has inspired a renewed interest for the edentulous patient to seek retreatment. Depending on the number and location of implants, there are a variety of restorative options that can now be considered. If properly diagnosed and efficiently treated these restorations can provide patients with extraordinary improvements over their conventional denture experience. They can also prove to be a tremendous financial incentive for both the clinician and the dental lab. Unfortunately “dentures on implant” restorations can be extremely challenging if not properly diagnosed and treatment planned. The restoration may not meet the patient’s expectations and may end up costing the clinician and laboratory time, money & frustration. In order to maximize the success of overdenture restorations, it is essential that the clinician engage the technologist in the early stages of treatment. Together the restorative team develops accurate methods of transferring patient information and records to the lab.

This program will emphasize four areas of treatment for the implant overdenture restoration:

1. Restorative options:Depending on the number and location of implants there are many restorative options available. Although there are many factors that determine the number & location of implants the major consideration is whether the prosthesis will be implant supported or implant/tissue supported. This will significantly influence the prosthesis design and its future success.

2. Restorative space:This is where communication with the dental lab is critical “prior” to the placement of implants. The expectations of the patient may not be compatible with the available restorative space (inter-ridge distance).

3. Materials selected:Again engaging the dental laboratory in the decision making process for superstructures, denture teeth and processing materials and systems is very important depending on the function of the prosthesis.

4. Occlusion:Probably one of the most destructive factors that accounts for the majority of restorative failures can be associated with improper choice and/or equilibration of occlusion.

Do Small Diameter Implants Have a Place in the Fixed Restoration World? Since their introduction in dentistry in 1999, small diameter implants have proven their efficiency in the stabilization of full removable prosthesis. Even though their first use was for stabilization of full dentures, the use of small diameter implants rapidly spread to stabilization of partial removable prosthesis, in the upper or lower jaw, bilateral or unilateral, with free saddles or not. Due to several reasons, some practitioners started to look at the option of using small diameter implants for fixed cases. Sometimes the lack of bone bucco lingually or space mesio distally may be the reason, or sometimes it is the refusal for bone grafting and often it is a pure financial reason that will bring the dentist to choose the small diameter for fixed cases. How can small diameter implants be used for fixed restorations? What is the protocol to use them for fixed restorations? Can they be only single or are multiple fixed cases also viable? What is the long-term success rate for small diameter implants for fixed restorations? Are there products on the market that are more suitable to achieve success with fixed restorations? What about the small diameter 3.0 mm one piece implant?

All these questions and more will be answered in this seminar so every participant will know if small diameter implants can be used efficiently for cases of fixed restoration.

Dr. Bruno Lemay was introduced to the small diameter implants at UCLA. In 2000, he started practicing in Palm Springs, California where he perfected his technique with small diameter implants and developed his own protocol to finally reach incredible success. In 2009, he started the Canadian Mini-Implant Institute specializing in the teaching of the mini-implant technique all around Canada. Since then he has been giving hundreds of weekend comprehensive training and has written several

articles in denturist and dental journals.

New Opportunities for Enhancement in the Esthetic Zone The esthetic implant rehabilitation of patients with esthetically and functionally compromised dentition frequently involves a multidisciplinary approach. The achieving of esthetic objectives in multidisciplinary cases represents a considerable clinical challenge. During recent years , treatment modalities and options have changed. From a surgical point of view, less invasive procedures are used in order to preserve soft tissue anatomy and minimize recession. Also the design of implants and prosthetic components has changed in order to minimize bone resorption and soft tissue alterations. This lecture will focus on a new surgical and prosthetic approach in difficult esthetic implant rehabilitations.

Dr. Eric Van Dooren opened a private practice in Antwerp, Belgium, after graduation, which is limited to periodontics, fixed prosthodontics, and implants. Currently, Dr. Van Dooren is a Visiting Professor at the University of Liege (Belgium) and an active member of the European Academy of Esthetic Dentistry. In addition, he is a member of the editorial staff of Teamwork (Germany) / PPAD (US) and a member of the Belgian Dental Society. Dr. Van Dooren lectures nationally and

internationally , mainly on aesthetics, implants and aesthetic periodontal surgery.

The Occlusion – TMJoint and Restorative Connection: Predictable Procedures for Predictable Results Over the years medicine has embraced technology and the use of physiologic measurements and parameters to assist formulating a diagnosis. The role of Biometrics in dentistry, physiologic assessment of muscle function and occlusion has been underused in dentistry. There is a fundamental role played by the neuromuscular system in the pathogenesis of dysfunctions of the stomatognathic apparatus. We are now able to measure and control the biophysical and biochemical phenomena which determine mandibular movements: “If it has been measured, it is a fact; If it has not, it’s an opinion”.

This presentation will focus on the diagnosis and process before the preparation of the teeth, the addition of the porcelain or the start of any orthodontic or prosthetic treatment plan. This discussion will include diagnosis of the smile, the dentition, soft tissue and occlusion then it will take the clinician through predictable steps to ensure aesthetic and functional success. Introduction to the concepts of Biometrics in dentistry and how technology can assist us in the diagnosis of the case will be reviewed. Specifically the discussion will focus on Joint Vibration Analysis, Jaw Tracking and the use of Tek Scan.
