Scope of this Addendum

Addendum Number Five includes modifications to the following Contract Documents issued October 20, 2014. These modifications are deemed necessary by the City and County of Denver.

PART I – PROJECT REQUIREMENTS

BID PROPOSAL 1. Replace the original bid proposal with the attached revised bid proposal noted

as Addendum #5 and dated November 20, 2014.

PART II – DIVISION II – TECHNICAL SPECIFICATIONS

VOLUME NO. 1 – CIVIL 1. Appendix A, 015525 TRAFFIC CONTROL

Add the following work description and measurement and payment requirements for Temporary Detour Pavement Construction and Removal: “015525e - Construction and removal of the temporary pavement section for detour roads will be paid for once per each detour location required. This will include placement of 3 inches of hot-mixed asphalt in accordance with Item P-401C on top of 8 inches of compacted asphalt millings. Compaction of asphalt millings shall be tested and accepted via proof roll in the presence of the DIA project manager. Compaction of the hot-mixed asphalt surface shall be to a 90 percent level as monitored by the QA testing firm, but all remaining testing requirements under the P-401C specification will not be required for this temporary pavement construction. This item shall include any excavation and preparation of subgrade necessary to complete the item. The subgrade shall also require proof rolling for acceptance by the DIA project manager. This work shall also include the removal of temporary detour roads and remediation of areas on which temporary detour roads are placed, which shall include revegitation of disturbed areas as required under the T-901 specification. Payment for temporary pavement construction and removal for each detour location will be at the contract unit price per each detour location. This price will shall be full compensation for placement of 3 inches of hot-mixed asphalt in accordance with Item P-401C on top of 8 inches of compacted asphalt millings. This item shall include any excavation and preparation of subgrade and the removal of temporary detour roads and remediation of areas on which temporary detour roads are placed,

as well as revegitation. This item shall also include all labor, equipment, tools and incidentals necessary to complete the item for each detour location.” Add the following pay item for Temporary Detour Pavement Construction and Removal: “015525e TEMPORARY DETOUR PAVEMENT CONSTRUCTION AND REMOVAL-PER EACH” The quantity will be 2 Each.

VOLUME NO. 2 – ELECTRICAL 1. L-125 – AIRPORT LIGHTING SYSTEMS

Section 2.02, section D, change “… five thousand volt, nineteen strand.” to read “… five thousand volt, seven or nineteen strand.”

2. L-125 – AIRPORT LIGHTING SYSTEMS Section 2.03, second paragraph, first sentence, change “The contractor shall remove and reinstall the existing quartz fixture.” to read “The contractor shall remove and reinstall the existing quartz or LED fixture, as shown in the plans.”

PART III – CONTRACT DRAWINGS VOLUME NO. 1 – CIVIL PLANS 1. Volume No. 1 Sheet C0003 – SUMMARY OF APPROXIMATE QUANTITIES

Add the following bid item “015525e TEMPORARY DETOUR PAVEMENT CONSTRUCTION AND REMOVAL-PER EACH” The quantity will be 2 Each.

2. Volume No. 1 Sheet C0004 – GENERAL NOTES AND LEGEND Note 17 is hereby revised from “100-Foot separation from the AOA fence.” to “10-Foot separation from the AOA fence”. Note 19 is hereby revised from “15-Feet above existing grades.” to “30-Feet above existing grades.”

3. Volume No. 1 Sheet C0100 – ARFF RESPONSE PHASING PLAN Emergency Vehicle Access Notes, Note 1 is hereby revised from “shall be delineated with reflective cones” to “shall be delineated with construction delineators”. This is in accordance with Detail 4 on Sheet C0150. The maximum spacing is increased to 25-feet for the designated ARFF access route, which deviates from the normal spacing of 10-feet as shown on Detail 6, as noted for other construction areas.

4. Volume No. 1 Sheet C0151 – CONSTRUCTION SAFETY PLAN

SAFETY NOTES: Note 10, is hereby revised to read as follows: “Contractor shall maintain and keep clear of the ARFF response access route between ARFF #3 and Great Rocks Road. The route from ARFF #3 to Taxiway EC, along Taxiway P to the north shoulder of Taxiway ED, and onto Great Rocks Road, shall be delineated as shown on Sheet C0100, in accordance with the Emergency Vehicle Access Notes thereon.” Note 13, is hereby revised to read as follows: “Contractor shall establish specific haul/traffic routes to, within, and from the construction areas. Construction delineators and signage shall only be required at entrance and exit locations to assure that construction vehicles understand the prescribed areas that are allowed for construction traffic. Contractor shall present their proposed plan to the DIA project manager for concurrence prior to construction. All areas that present opportunities for vehicles to potentially stray from the approved routes shall be appropriately barricaded and signed to prevent access to unapproved areas.”

5. Volume No. 1 Sheet C0450 – PAVING DETAILS Detail 4, Type E2 – Doweled Construction Joint, change call out from “Detail 2, Sheet C0451” to Detail 4, Sheet C0451.” See attached, revised Sheet C0450.

10. Volume No. 1 Sheet C0601 – PAVEMENT MARKING PLAN AREA 1 Note 4, Change to read, “The Contractor shall cover elevated and in-pavement light fixtures before removal of curing compound. Covering elevated and in-pavement light fixtures before pavement marking operations is not the responsibility of the Contractor. The exception to this would be where lights would need to be covered by the Contractor while placing and removing temporary pavement markings at the designated detour locations.”

11. Volume No. 1 Sheet C0702 – DETOUR ROADS Add Note 4, “Temporary paint shall be Waterborne and meet the requirements of Federal Specification TT-P-1952E, Type II. Temporary paint shall be applied at a rate of 230ft2 /gal, maximum. There will be no additional payment for the placement or removal of temporary pavement markings, as these items will be incidental to Item 015525d – TRAFFIC CONTROL. Match the markings that are present along the centerline and pavement edges.” .

1. DRILLING METHOD FOR DOWELS SHALL BE CAPABLE OF MAINTAINING DRILL HOLES

PARALLEL TO THE CONCRETE SURFACE AND NORMAL TO THE JOINT LINE WITHIN

1/4" AT THE END OF THE DOWEL BAR. DRILL HOLES SHALL BE ACCURATELY LAID

OUT SO THAT THE MAXIMUM DEVIATION DOES NOT EXCEED 1". DRILL HOLE

DIAMETER BE OF SUFFICIENT SIZE ACCEPT THE TYPE AND SIZE DOWEL

REQUIRED, INCLUDING BONDING MATERIAL.

2. AFTER THE DRILLING IS COMPLETE AND PRIOR TO THE INSTALLATION OF THE

DOWELS, THE HOLES SHALL BE THOROUGHLY CLEANED REMOVE DRILLING DUST,

CONCRETE CHIPS AND ANY MATERIAL DETRIMENTAL TO DEVELOPING BOND

BETWEEN THE DRILLED SURFACE AND DOWEL.

3. BONDING MATERIAL SHALL BE APPLIED UNIFORMLY TO THE ENTIRE

CIRCUMFERENCE OF THE DOWEL HOLE SURFACE, AND SUFFICIENT MATERIAL

PLACED IN THE HOLE SO THAT A SLIGHT AMOUNT WILL BE FORCED FROM AROUND

THE ENTIRE CIRCUMFERENCE WHEN THE DOWEL IS INSERTED AND TAPPED TO

THE CORRECT POSITION. SECURE DOWELS UNTIL EPOXY HAS FULLY SET. SMALL

WEDGES MAY BE USED TO SUPPORT THE DOWEL IN CORRECT ALIGNMENT AND

ELEVATION UNTIL THE MATERIAL HARDENS. WEDGES WILL BE REMOVED PRIOR

TO PAVING ADJACENT PANELS.
