Proactive Health Management Plan…Why ACLHS?

A Charitable Life Health Solutions (ACLHS) prides itself in creating the most unique

products to improve workforce health while assisting businesses with

compliance. ACLHS is proud to announce its new Proactive Health Management Plan

(PHMP). The PHMP is a voluntary limited medical plan designed to provide preventive,

diagnostic, and continuing services for all employees while creating FICA savings for

employers.

By providing the PHMP to employees, businesses benefit from payroll savings, a healthier

workforce, and compliance with current ACA mandates. The PHMP begins with IRC Sec.

125 (cafeteria plan) which reduces taxable income, creating payroll tax savings for

employers and employees. Utilization of the services provided through the PHMP

qualifies participants for a fixed benefit payment. The plan benefits provide a wide range

of offerings for the diagnosis, cure, mitigation, treatment, or prevention of disease. All of

Nutritionists Available on the Plan Effective Date PHMP Certified Nutritionists are experts in the field of food and nutrition. They may provide dietary

plans specific to each employee promoting a healthier lifestyle or achieve a specific health-related goal.

Biometric Screening Available 3 Months (90 days) After Plan Effective Date Bio-metric Screening can identify risk factors including diabetic and

cardiovascular disease up to ten years before full onset. Baseline data and analytics assist our health management coaches to provide customized plans for each employee.

DNA Screening Available 9 Months After Plan Effective Date Knowing you have a mutation in one of the 19 genes analyzed —including BRCA1 and

BRCA2 —enables you to work with your healthcare provider to create a personalized plan designed to prevent or detect breast and ovarian cancer at an earlier or more treatable stage.

Mammograms Available 1 Year After Plan Effective Date Regular mammograms can often help find breast cancer at an early stage, when treatment is

most likely to be successful.

Prostate Available 1 Year After Plan Effective Date Prostate cancer screening can help identify cancer early on, when treatment is most effective. And a

normal PSA test, combined with a digital rectal exam, can help reassure you that it’s unlikely you have prostate cancer.
