2) Part 1: Chapter Leadership From: E-mail by December 16 to Chapter.Accord@afpnet.org

3) Part 2 – Check boxes

4) Part 3: Chapter Activity Report- Must be completed through online survey

5) Part 4: E-mailed to Chapter.Accord@afpnet.org

6) Click Save button

Deadlines• December 16 – Chapter Leadership Form

• January 15 – All other materials

• Must be RECEIVED by close of business

• Required for 2014 Ten Star/Ten Star Gold Awards (awarded in 2015)
