What is Synthesized ECG ?

Synthesized 18-lead ECG uses the 12-lead ECG waveforms to mathematically derive the waveforms of the right chest leads (V3R, V4R, V5R)

and back leads (V7, V8, V9). The measurement procedure is the same as the standard 12-lead ECG but more information can be obtained.

18-lead synthesized ECG is expected to be useful in detecting right side and posterior infarction.

The most common ECG exam is the standard 12-lead ECG. It is simple to measure, has low burden on the body, and observing the heart from these 12 directions provides a lot of information which has a wide range of clinical applications.

However, some areas, especially pathological change in the right ventricle and the posterior wall cannot be observed from the 12-lead ECG. In order to actually measure the right chest (V3R, V4R, V5R) and back (V7, V8, V9) areas, it is necessary to use di� erent elec-trode positions than the standard 12-lead ECG. In particular, electrodes must also be attached to the patient’s back so that normal suction cup electrodes cannot be used. Also, the pati-ent must be turned over in some cases and in an emergency it is often di� cult to use back electrodes. This complicates the exam pro-cedure. Synthesized 18-lead ECG uses the 12-lead ECG waveforms to mathematically derive the waveforms of the right chest leads (V3R, V4R, V5R) and back leads (V7, V8, V9).

It is important to prepare the skin before attaching the electrodes

by cleaning it and removing chest hair. After this please attach

the lead wire to the electrodes before placing them on the skin.

This avoids pain to the patient. Secure the lead wire to the skin

(stress loop) preventing moving of the lead wire and therefore

noise on the ECG signal.

FactsThe NIHON KOHDEN Fastclip ECG Electrodes allow a fl exible application for routine work and emergency on every paediatric and adult patient thanks to a simple adjustability of the clip pressure (clip size). A more comfortable handling is guaranteed due to di� erent con-nectors for any existing adapter type, a big contact area for reliable ECG measurements and easier cleaning after the measurement because of less asperity of the clip. In addition to this IEC colored label sets are delivered with NIHON KOHDEN Fastclip ECG Electrodes.

By following the next steps, optimized derivations can be realized with NIHON KOHDEN Fastclip ECG Electrodes:

1.) Clean the according skin areas with alcohol in order to remove traces of dirt and oil on the skin.2.) Apply a thin layer of CardioCream F010 on the cleaned skin areas and on the electrode part.3.) Clip the electrode on the wrist or ankle. Adjust the electrode positon by sliding the electrode part according to the thickness of the limb.

The result of over 30 years experience in disposable electrode manufacturing.

The ECG electrodes are the most important portions of the ECG/arrhythmia monitoring system. It is through these electrodes that the electrical signals are conducted from the patient and into the monitoring system. If the electrodes are left in place for an extended period of time (over 48 hours), or they are reused once they are removed, they do not adhere to the patient’s skin and they cause “noisy’’ ECG signals because of the poor conduction. In addition, the patient’s movement causes the electrodes to lose conducting capabilities intermittently, and this leads to a “wandering’’ ECG waveform and muscle artifact. The result of over 30 years experience in disposable electrode manufacturing.

The ECG electrodes are the most important portions of the ECG/arrhythmia monitoring system. It is through these electrodes that

the electrical signals are conducted from the patient and into the monitoring system. If the electrodes are left in place for an exten-

ded period of time (over 48 hours), or they are reused once they are removed, they do not adhere to the patient’s skin and they cause

“noisy’’ ECG signals because of the poor conduction. In addition, the patient’s movement causes the electrodes to lose conducting

capabilities intermittently, and this leads to a “wandering’’ ECG waveform and muscle artifact. The standard monitoring practice is

to change these electrodes at least every 48 hours.

hair. After this please attach the lead wire to the

A Pulse Oximeter is a non-invasive device used to percutaneously measure oxygen saturation

in the blood by use of a clip-on fi nger probe. Pulse oximeter works with the integration of a

probe placed onto patient’s fi ngertip or toe, obtains signals from the probe and the main

device calculates oxygen saturation in the body.

Oxygen saturation measured by a Pulse Oximeter is SpO2. “P” stands for “pulse”, and the

measurement principle is based on beat-by-beat changes in blood volume. In general, arteri-

al oxygen saturation measured by spectroscopic analysis is called “SaO2”, and oxygen satura-

tion measured by pulse oximeter is called “SpO2”.

The method for determination of arterial oxygen saturation, based on the photoelectric ple-

thysmography, was conceptualised and implemented in 1972 by the Japanese bio-engineer

Takuo Aoyagi (NIHON KOHDEN Corporation). This idea came about while using an ear den-

sitometer for recording dilution curves for dyes (pigments). The fi rst commercial oximeter

“OLV-5100” was presented by NIHON KOHDEN back in 1975.

Dr. Aoyagi’s innovative technology, pulse oximetry, has rapidly spread worldwide and become essential in critical care. In

2002, Dr. Aoyagi was awarded the Medal of Honor in Japan, in recognition of his great contribution to clinical medicine.

FactsNIHON KOHDEN reusable SpO2 fi nger-clips probes have parallel mounted upper and lower sensor shells, thus, providing an exact alignment of the op-tical sensor components and markedly enhanced comfort during long-term attachment. In addition, fl exible lateral fl aps shield the measuring signals from the external light, thus resulting to minimize the interference by additional artifi cial light sources! Bright LEDs provide more accurate measure-ment even for thicker fi ngers or dark skin tones. The fi nger-clip reusable probe is the most popular type of SpO2 probes. Instead of the typical hinge near the fi nger tip, P225F/P225H have a unique sure fi tting and comfortable mechanism using two sliding pieces for the front and back of the fi nger with two rubber fl aps along both sides of the fi nger. When you press the button, the two halves of the probe slide directly open, parallel to each other. No additional fi xation tape is needed. Disposable SpO2 Probes NIHON KOHDEN disposable probes, from neonate to adult sizes, place special emphasis on the exact positioning of the LEDs and detector. For this reason the two optical components are designed separately and can thus be brought into precise alignment with each other by the user. All probes have breathable material and can be remained up to 8 hours on one attachment site. Paediatric probes have a cute cartoon character printed on the tape to reassure children.

Self-adhesive, soft and thin tape provides a secure fi t. The special design allows an easy application to the attachment site. Handling of the probe a� ects measurement accuracy and patient safetyFor the proper placement of the probe on the fi nger, it is advisable that the light emitter and detector are placed just opposite to each other. When using a fi nger clip probe, it is necessary to ensure that the probe covers the fi nger properly, otherwise some portion of the light never passes through the body tissues and may reach the light-receiving part directly, leading to incorrect measurement results.

The more distant the light emitter and detector gets, the weaker the transmitted light intensity becomes. Additionally, a thinner body part tends to have small amount of blood, which makes it di� cult to extract the arterial blood signal component. The most suitable thickness of the probe site is about 10 mm.

Too loose probes may interfere with alignment of LEDs and detector, come o� easily by body movement, cause larger motion artifacts or be e� ected by venous pulse.

Too tight probes may cause venous pulsation, cause skin injuries such as burn and necrosis or unstable measurement because of a lower pulsatile signal.


Reasons to measure ETCO₂ (the level of carbon dioxide in the air exhaled from the body –

normal value is about 38 mm Hg): Capnography helps to detect life-threatening problems in:

• Breathing

• Circulation and

• Metabolism

Capnography is a fast and reliable help to determinate respiratory di� culties, wrong connections of airway

adapters, leaks in the ventilator circuit due to its placement in the respiratory circuit. An early detection by

capnography might help to avoid inadequate oxygen supply of the body.

Blood pressure (BP) is the pressure exerted by circulating blood against the artery walls. During each heartbeat, the blood

pressure varies between a maximum (systolic) and a minimum (diastolic) pressure. The variation indicates the condition

of the pumping function of the heart as an important vital sign.

A non-invasive indirect method to measure the blood pressure is the oscillometric blood pressure measurement. This me-

thod is using an infl ated blood pressure cu� which is applied to the patient’s arm or thigh. The blood pressure is detected

by partial occlusion of the artery by the cu� . The so called Korotko� sound is generated by the distension of the arterial

wall with each cardiac impulse. Systolic and diastolic blood pressures are determined by an algorithmic interpretation of

the device based on the mean arterial pressure and changing amplitude of the Korotko� sounds.

For a proper non-invasive measurement it is important to use the correct cu� size due to the fact that a cu� too small will

lead to faulty high pressure, while a large cu� can lead to wrong low pressure readings.

Blood pressure (BP) is the pressure exerted by circulating blood against the artery walls. During each heartbeat, the

blood pressure varies between a maximum (systolic) and a minimum (diastolic) pressure. The variation indicates the

A strong Chain of Survival improves chance of survival

The “Chain of Survival” is a proven approach to the early treatment of sudden cardiac arrest, heart attack, stroke and any other emergency. It consists of four crucial links:

1. Early Access - to activate the Emergency Medical Services (EMS).

2. Early Cardiopulmonary Resuscitation (CPR) – to slow the rate of deterioration

of the brain and heart, and buy time to enable defi brillation.

3. Early Defi brillation – to restore to a normal heart rhythm.

4. Early Advanced Cardiac Life Support (ACLS) – to stabilise the patient.

Any delay in any of these links could potentially be fatal or decrease the chance of survival.

Most people who die of heart attacks experience irregular heartbeats known as ventricular fi brillation (VF); a condition where the heart stops circulation of blood. lf left untreated, the victim will die within minutes. By far, defi brillation (electric shock) is the only known treat-ment to revert a troubled heart from VF to its normal rhythm.

Importance of AED in the Chain of Survival

Automated External Defi brillator or AED is perhaps the most important link in the chain to improve the SCA victim‘s survival chances.

An AED is a device that is used to diagnose and treat potentially life thre-atening abnormal heart rhythms by providing the means to apply con-trolled electrical shocks (known as defi brillation shocks) to the heart in an attempt to stop these abnormal rhythms, thereby allowing the heart to establish again an e� ective rhythm.

It will also provide guidance by simple and clear vocal instructions to the rescuer during the rescue sequence.

Studies have shown that the time to fi rst defi brillation shock is the most crucial factor in determining survival rates for SCA*. When an AED is available, it should always be applied to the SCA victim at the earliest.

For over 90% of SCA cases, death usually occurs if without intervention. However, as many as 30-50% would likely survive if AED and CPR was administered within fi rst 5 minutes of collapse. Survival chance can be signifi cantly increased with early defi brillation.

For every minute of delay in giving CPR and defi brillation following collapse, the survival rate decreases by 7–10%. So, why let tragedies happen where there is prevention available now? The result of not having an AED may well be another fatal case.
