As a leading cause of death in the United States and across the world, stroke is an important service to consider for healthcare leaders. With states passing new legislation for stroke care, an efficient and effective approach is more important than ever for this population.

After completing a survey in partnership with the National Stroke Association (NSA) on the usage trends of the National Institutes of Health Stroke Scale (NIHSS), Corazon found that significant opportunities exist to improve stroke care in hospitals across the country. The need for the industry’s first stroke patient managment solution was clear.

CEREBROS™ was developed to assist clinicians with delivering the highest quality of care to an already large and growing acute patient base. Since then, CEREBROS™ has grown into a true Neurovascular Information System (NVIS). System users are already changing practice to better manage stroke patients by providing seamless care through their entire hospital experience. Nationally, the average administration rate of t-PA to eligible patients is only 10%. “Best-in-class” providers reach rates of only 40%.

Through a focused effort to increase institutional capabilities relative to stroke patient care, these rates and other critical measures of best practice care can improve when using CEREBROS™.

Along with improved patient outcomes, the administration of t-PA yields a higher reimbursement rate than strokes that do not receive t-PA.

On average, the reimbursement for a stroke patient increases by

up to $6,000 when t-PA is used.

Increasing t-PA treatment rates and reaching other clinical or operational program goals has the potential to improve the stroke program’s overall performance, allowing for increased growth of the entire neuroscience

Will the CEREBROS™ Application interface with our existing IT system/s?

Yes. The application is HL-7 compliant and uses a back-end, middleware platform to integrate CEREBROS™ with Admit/Discharge/Transfer (ADT) systems and other internal systems, such as Electronic Health Record (EHR), with minimum effort.

Will using CEREBROS™ require clinicians enter information into multiple systems?

No. Information is pushed or pulled between CEREBROS™ and hospital systems as patients move through the standard course of care. Data entry fields are mapped to fields within hospital systems, and CEREBROS™ tabs can be embedded within the current EHR for seamless availability, if supported.

What resources are available to our staff once CEREBROS™ is installed?

The CEREBROS™ Team is available for questions and troubleshooting remotely. Additionally, the implementation process creates a hospital-based group of “super-users” to answer basic staff questions that emerge during patient care. Other documentation and resources are provided within the application, such as a detailed user guide.

Our organization already reports to other certifying organizations or entities (e.g. TJC, AHA, HFAP, DNV, State agency). Will using CEREBROS™ be duplicative?

No. CEREBROS™ data elements are matched to nationally recognized data definitions. Once the certifying body’s file structure is defined, standard reports are created to efficiently export the necessary information.

Can we use CEREBROS™ across multiple system hospitals?

Yes. Additional licenses may be purchased for muliple stroke programs within a system. This will lend to standardization of care across the system.

In November of 2012, Corazon was engaged to assist with the implementation of a Primary Stroke Center at Memorial Hospital in York PA. As in many communities, recently passed legislation mandated that patients suspected of having a stroke, be transferred only to certified stroke centers. This change meant that patients within the community served were potentially bypassing Memorial, and receiving care many miles from home. As a recent study outlined in the American Heart Association’s journal Stroke, outcomes can be vastly improved for certain types of stroke if treated as soon as possible - something hampered by the need to transfer away from the local hospital. Given an above national average incidence of stroke in the community, development of a stroke center was a strategic imperative. Corazon was engaged to implement a plan that would incorporate best practice, evidence-based protocols, as well as to develop the infrastructure necessary to achieve certification through the Healthcare Facilities Accreditation Program (HFAP). As part of the implementation plan, Corazon incorporated CEREBROS™, a technology solution to assist in the proactive management of the stroke patient population. In addition to the system’s reporting and program management value, Cerebros was a resource to Memorial’s Stroke Program Coordinator to access additional neurology expertise as they grew their program.

Additional benefits of the CEREBROS™ software include reference materials and reminders for several key care factors that often prove to be “pain points” as hospitals pursue certification – such as consistent provision of patient education, performance of swallow screens, and determination of t-PA eligibility. A major benefit of the system is removing the need for retrospective data collection, as data is collected real-time through the course of patient care, additionally saving time and effort that may otherwise be spent specifically on data abstraction separate from the patient care process. The Memorial team integrated CEREBROS™ into the entire episode of care for stroke patients, making it a valuable tool as they continue to provide successful outcomes for their stroke population.

The marriage of pro-active technology, with standard performance metrics and best practice care delivery, within this implementation model means success for both Memorial and the patients that they serve. Memorial was successful in achieving certification through HFAP during the summer of 2013. The Stroke Program Coordinator and front-line clinical providers continue to utilize CEREBROS™ every day, for every patient, to ensure a high standard of care is met. The data collection is now real-time, and course corrections with respect to the care of an individual patient are effected when they are the most beneficial - during the acute hospital stay.

Any clinical program is more than an IT system. There are key clinical considerations that must be determined and prepared for prior to go live: order sets, processes and procedures, coverage schedules, staff education, staff buy-in, and many more.
