Dr. Hansen retired from the US Air Force in 1995 aſter teaching prosth- odontics in several of the air force advanced training programs. He directed the graduate program in prosthodontics at the University of Missouri — Kansas City from 1995-2000. Dr. Hansen developed and ran a private practice in Kansas City from 2000-2007. He is currently the Director of the Prosthodontic section at the University of Nebraska College of Dentistry. Dr. Hansen has a private prac- tice in Lincoln, Nebraska. Happy Holidays! What a great year to be your President of iaGD! We just completed our fi rst

Mastership track course. Th e participation part of our course was oversold a few weeks

before it began! We had more sign up during the weekend. it is great to be in a participation course where you get to have one on one with a great speaker. i want to thank Dr. Bob Margeas for

putting on an excellent program. i want to thank Dr. Margeas also for providing us with such an excellent facility to use. i can now do a class iV composite as taught and you can not see where the tooth ends and composite begins.

your board also voted to have iaGD be a part of a Coalition for Community Water Fluoridation along with the iDa,

iowa Public Health Dept., iowa section of the american Water Works association, Dr. levy from the University of iowa, and others. it is the beginning stages of organizing and other stakeholders are being invited to join. it is inspiring to hear stories from our own board members who have worked to keep fl uoride in the public drinking water. it is one of the top 10 public health measures taken in the last 100 years. if you have a story to share about what you have done in your community, please contact Dr. Jon Hardinger or myself.

We have several board members attending the aGD leadership conference in Chicago in november. you can be proud of your board representatives. Objectives • The ideal occlusion. What are the param-eters that we use to determine the ideal occlusion. We will discuss the joint position that we will use to evaluate the ideal interoc-clusal relationship. We will discuss alternative methods of observing the interocclusal rela-tionship and discuss what is evidence based.

• Fabrication of provisional restorations that will provide the ideal skeleton by which we will reconstruct the patient. We will review how to make a stable interocclusal relation-ship using provisional restorations.

• non carious tooth structure loss. We will look at the means by which we lose tooth structure. This will include abrasion, erosion and attrition. How do we evaluate wear pat-terns and interocclusal wear. Multiple cases will be shown and discussed.

• restorative materials for full mouth recon-structions. We look at different materials and their wear resistance and strength. We will discuss precious metals, non- precious metals, porcelain fused to metal restorations, lithium disilicate ceramics, and zirconia based ceramics.

The hands on session will assist you with:• Fabrication of impressions and pouring casts. • Vertical bruxism and the resulting tooth structure loss. We will look at the definition of bruxism and study the differences between vertical and horizontal bruxism. We will apply the knowledge of the ideal occlusion to the difficult clinical case. We will discuss how to treatment plan the difficult wear case and cre-ate a predictable final result.• The diagnostic mounting. What it is, and what we are looking for?• Hands on mounting the casts, evaluating the accuracy of the mounting, and equilibra-tion of the casts.• The importance of vertical dimension of occlusion.• Hands on fabrication of the indirect pro-visional restorations. We will learn how to make indirect provisionals which will provide us with individual provisional restorations to evaluate esthetics, function, and vertical dimension.• Cases. We will review different difficult cases and how to restore them. We will make this an interactive session where we all will

have discussions and input. Do we need to remount fixed cases? Will it help with esthet-ics and function?

• review anatomy of the tMJ and associated structures

• review occlusal anatomy. Hands on exercise.

• The equilibration of a patient how can we make it predictable? is the equilibration of a patient necessary? We will review more cases. We know the ideal occlusion for our patients, but how do we apply this knowledge to the clinical setting, and restore the difficult wear and difficult occlusion cases?

his deceased associate Paul Polydoran, DDs, while presenting a characterization case. an elderly lady presented with a chipped central incisor that was characterized with dark lines. Dr. Margeas illustrated his technique by mimicking the exact anatomy—a prominent stain line. He was so pleased with the outcome that he brought Dr. Polydoran to take a look and guess which tooth he had restored. Dr. Polydoran happened to guess another tooth, meaning that the restoration was undetectable. Dr. Polydoran continued, however, and addressed the lady, saying, “Would it be worth $10,000 to have your upper teeth made bright and natural again?” she said, “yes, it would, and let’s get started!” so, Dr. Polydoran had been fooled by the perfect restoration, but he had his mind on the patient’s desire for better dental health. Dr. Margeas relished this story, and he was certainly glad that Dr. Polydoran had made the proposal.

Dr. Margeas offered comments on his office, which he said is lean. He follows a rule he learned from his grandmother, who said, “if you see work to do, do it.” He said that his office is cross trained and that he doesn’t have job descriptions that might limit the staff ’s assisting, hygiene, and receptionist duties.

issue, but the bone sets the tone”• to the patient in need of treatment: “i am

only the referee, and i call them like i see them.”

the iowa academy of General Dentistry (aGD) Fall Meeting was held on Oct. 5 at the Hilton Garden

inn in Johnston, iowa. This was the lecture part of the Mastertrack program, and it continued on through the weekend at the office teaching facility of Bob Margeas, DDs, FaGD. He is a former president and longtime friend of the iowa aGD.

His lecture program began with the diagnosis of various types of tooth wear, including attrition, erosion, abrasion, and abfraction. He defined them and gave clinical examples. He said that the diagnoses of acid reflux and sleep apnea are often missed by the medical community but often seen in our patients. He recommended the selective use of GC Fuji iXtM extra and Ketac nanotM glass ionomers, along with composite resins. He also presented diastema closure with direct resin.

Dr. Margeas also discussed his management of worn dentition and gave

us indications for opening the bite or using orthodontic bite opening to allow for restorative material. He demonstrated how he achieved this with composite resin, especially when staging a case over time. He told us about an online course at www.occlusiondesignvideos.com that can provide specific help with this subject.

He also presented porcelain veneers and distinguished between the feldspathic and the pressed, saying that the former was very thin and subject to breakage before being seated. He reviewed indications, diagnoses, preparations, and delivery of the restorations.

He said he asks bulimic patients how long they’ve had their problem and checks whether his patients have fingernails, as they might be chewing them. He also recommended the Benex extractor for esthetic extraction.

The program was outstanding from a clinical perspective, and from a personal one as well, as Dr. Margeas talked freely about

He introduced his daughter, an iowa state University architecture student, who was fresh from a school landscape presentation.

i also met a couple of runners at this meeting. Phelan Th omas, DDs, talked about it at lunch, and the youthful Karla Becthold, DDs, who is also a runner, was at the reception from Hinton. later, we were talking with roberto Moreno, DDs— son of iowa aGD President Carol Moreno, DDs, MaGD—who as a student athlete at iowa state University ran a mile in just more than four minutes.

i also met executive Director Julie Berger-Moore’s mother, Carol, who had come along to assist Julie for the weekend and see relatives.

Th e iowa aGD Fall Meeting was followed by a reception sponsored by Dental Prosthetic services (DPs), and many dentists lingered around aft er the course.

i spoke to Don Good, DDs, who had recently added an associate to his ames practice. He said he liked that his agreement includes a $25,000 payout for each party if they don’t wish to continue. He also said that he saw Bob shirley, DDs, at a recent craft sale and gave him a big hug. Dr. Good said that Dr. shirley really missed his eldora practice!

i also met rick nelson, DDs. Our paths had last crossed in nashua, where his late father practiced veterinary medicine and i practiced a block away at that time.

i heard about it on sept. 10, and i remember where i was. i was sitting at my desk, and Henry schein representative Brett Drea said, “i am not an ambulance chaser in telling you this, but my manager said that iowa Dental supply (iDs) was purchased by

Patterson Dental, effective immediately!” i was shocked, but i did not want to show it. i thanked him for the warning and let

him know that i excused him for passing on information that might benefit him if iDs was indeed no more. after he left, i checked the iDs website and saw the official announcement redirecting visitors to the Patterson website. it read that business would continue as usual.

i was still in shock—speechless, in fact! i could not fathom how to express myself, so i showed the website announcement to my office staff so they could read it for themselves. everyone was shocked—except my ordering assistant. she said she had expected it, explaining that everything small gets eaten up by big companies. Haven’t we seen the trend ourselves?

i remember having a conversation about the sudden death of a man, and the person to whom i was speaking said, “Who is going to do what he did for me now?” He then corrected himself: “i am ashamed to say that all i am thinking about is myself!”

When i got the news about iDs, the central vacuum line in my office’s fourth operatory was broken—which meant we could not use the room—and the Pelton & Crane part had been ordered before the big acquisition. it seemed strange that i was concerned about my operatory—but that concern was necessary.

so, i called central operator at Patterson, and a male voice answered:“Patterson Dental supply.”

“Hello, this is Dr. Hardinger from Mason City. i need the service department.”“i will get them for you right away, Bro.” “Uhh … what did you say? Did you call me Bro?” “yeah!” i laughed out loud, amazed. i have been doing business with iDs for almost 30 years,

and they always called me Dr. Hardinger. early on in my career, soon after dental school graduation, Dick Cacciatore of iDs greeted me as Dr. Hardinger and said, “it took a lot of hard work to earn that degree. you deserve to be called doctor.” it will take some time to get used to this phone operator, whom i have never met, calling me Bro! nevertheless, Patterson said the parts would be located and installed soon.

When i asked 3M sales representative Carla stallman about the sale of iDs, she said, “These two companies have been my working partners for many years, and i think they are both great. i like them both equally. it is like a marriage of my best friends.”

“That is the best way to think of it,” i said, “the coming together for good.”i can’t say goodbye without acknowledging a few of the many at iDs. Carl and noche

Cacciatore founded the company in 1935. Carl’s daughter, Mary, and noche’s sons, Dick and lou, were the second generation. lou’s sons are Mike and rick. Dick helped me draw up plans for remodeling my office in nashua. We cut through a wall to make the traffic flow better for the staff, and the changes served me for many years.

sales representative Don Graziano helped me get started in assuming a practice in nashua in 1979, and i was touched when he got me a little bouquet of flowers after my daughter rachel was born in 1980. tom tice was my sales representative in Mason City, and i felt like he was teaching me a continuing education (Ce) class as he cycled through the various disciplines of dentistry over the years.

i will always remember sept. 10, 2012, and where i was when i first heard this news. Goodbye, iowa Dental supply—it has been great doing business with you. They were my dental family.

advocacy Update

senator stops by Iowa Dental Practice

On Oct. 11, 2012, U.s. sen. Chuck Grassley (r-iowa) visited the Des Moines, iowa, office of stephen r.

Thies, DDs, legislative chair for the iowa aGD. at the time of the visit, Grassley was the ranking member of the senate Judiciary Committee and had been conducting an investigation into abuses of the Medicaid system by corporate dental chains.

as an attendee of the aGD Government relations Conference, a Great Dentist Goes to Washington, Dr. Thies understands the importance of good relationships with legislators. He first met Grassley during the aGD 2011 Government relations Conference as part of a delegation that discussed repealing the McCarran-Ferguson act.

after meeting Grassley’s staff at the aGD 2012 Government relations Conference, Dr. Thies invited the senator to visit his practice while Congress was in recess. While Grassley was in Des Moines for other legislative activities, he made time for an office tour and some interaction with patients. although he had been to dental offices for treatment, the senator said he had never before seen “behind-the-scenes” activities like sterilization and laboratory procedures.

During the visit, Dr. Thies presented the senator with copies of two aGD white papers—“White Paper on increasing access to and Utilization of Oral Health Care services” and “Barriers and solutions to accessing Care.” He explained the dental profession’s concern about patients’ access to quality care from dental team led by a trained dentist. Dr. Thies also raised the issue of burdensome student loan debt, which may limit practice decisions for dental graduates. Grassley was receptive and concerned about the issues facing dentistry.

academy of General Dentistry Joint Council Meeting

i would like to report on the biannual academy of General Dentistry (aGD) Joint Council Meeting, held from

sept. 27 to 29, at the renaissance Chicago Downtown Hotel. i currently serve on the Communications Council, which covers marketing, technology, and publications.

We had a lot of activity at the session, working on the exciting new Web page that will simplify the current cumbersome site, and discussing various articles to be published in AGD Impact and General Dentistry. We also met the managing editors for General Dentistry and AGD Impact, tiff any slade and Beth Garcia, respectively. additionally, aGD editor roger Winland, DDs, Ms, MaGD, reported that we are looking for an associate editor.

Th e 2012 July/august issue of General Dentistry featured articles submitted by the american academy of Periodontology (aaP). Th e staff is currently working on a future issue with the american association of Oral and Maxillofacial surgeons (aaOMs). Further, there will be a bonus issue of General Dentistry in July 2013 and a separate issue for august 2013. Th e Communications staff has reached out to an organization of dental offi ce managers about contributing to articles to AGD Impact.

in regards to using the fl agship publication of the aGD, General Dentistry, i would like to refer you a few years back to a July/august 2008 by James Pavlatos, DDs, “Oral care protocols for patients undergoing cancer therapy.” it happened that one of my patients developed oral squamous cell carcinoma, and i remembered this article and used it to explain to her the practical day- to-day management of her oral needs. an added bonus occurred when i related this to my Joint Council colleagues. Th ey referred me to the author directly, and we had several informative conversations about cancer management. another more recent General Dentistry article i shared with my dental hygienist was on lasers in periodontics, “eff ectiveness of lanaP over time as measured by tooth loss” (March/april 2012). Th is was useful because my offi ce is looking into laser therapy.

Th e marketing part of our council also discussed successful public promotions, including the ad Council’s recent launch of its campaign for the Partnership for Healthy Mouths, Healthy lives. Th e accompanying videos are available at www.2min2x.org. We also had a presentation on our organization’s growing social media activities, including Facebook, twitter, and linkedin.

aGD spokespersons and leaders respond

to current issues that are carried forward by the print and visual media. Th e value of this can be understood by comparing it to the cost of advertising the same information. Plus, it is a great value to the profession.

Th e publications and technology parts of our council also heard reports about aGD bloggers and podcasts. We discussed the use of current technology to promote our publications as well. One suggestion was to make aGD publications available on iPads and Kindle Fire readers like other newsmagazines. We hope to off er this for our members who rely on these devices for current publications. George Boyle, our staff technology specialist, quoted some vendors in this fi eld. We can be highly sophisticated and include live links and videos in the electronic issues, and each of these features would add to the cost per issue. Mr. Boyle also told us about the redundant computer backup system that is now used to prevent more losses

like the one that occurred last year with a couple of podcasts. Our publications include self-instructional exercises found in General Dentistry, which has continued to grow in subscribers.

Our council also had combined sessions with the annual Meetings and Dental education councils. We also met in Joint Council, where interim executive Director larry rosenthal, PhD, gave a presentation on strategic planning, and a representative from Kantar Media presented her fi ndings from a recent survey on the value and usage of aGD membership services.

i urge my readers to use the resources available to them through publications and other services to keep current with their dental practices.

AGD Headquarters Purchases New Building

the academy of General Dentistry (aGD) has purchased its own building in the West loop neighborhood of Chicago, at 560 W. lake st. according to aGD President Jeff rey Cole, DDs, MBa, FaGD, the purchase will eventually result in savings that

will be used to fund member services.Th e academy of General Dentistry (aGD) has found itself a new home.earlier this year, the organization purchased its own building in the West loop

M. Cole, DDs, MBa, FaGD, the aGD has operated out of a rented space since it was founded 60 years ago. Th e lease on the current aGD Headquarters, located in the american Dental association (aDa) building at 211 e. Chicago st. in Chicago, expires in 2014.

Th e 560 W. lake st. property was recommended by the Board of trustees’ real estate task Force, which was formed in 2007. Th e aGD settled on the property on aug. 3, 2012, and the task force is working with aGD staff , as well as a team of interior designers, to plan a work space in the loft -style building.

Th e aGD will realize a positive cash fl ow of $500,000 seven years aft er the purchase date, Cole says, and those savings will help the organization fund its member services.

He says the move will also benefi t the aDa, which can now secure a new tenant for the aGD’s current space at a higher rate.
