Shoes protect and support feet during daily and sports activities. At various orthopedic and systemic diseases, footwear modifications are indicated to reduce mechanical stress at the foot and the ankle joint. Effective orthopaedic shoe modifications are provided by different rocker sole modifications. But no internationally accepted terminology and method exists to describe the devices used in the field of footwear modifications. The clinicians who prescribe these devices, the manufacturers who produce them and the pedorthist / orthopedic shoemaker who supply them to their patient, in different countries, apply widely differing terms to describe the devices and the construction details. This situation creates problems for clinicians, manufacturers and researchers attempting to compare the treatments used in different countries and their outcomes. The German guideline specifies the clinical objectives for the use of these devices and describes their functions and constituent components.

The aim of the talk is to present the German guidelines for footwear modifications e.g. rocker sole constructions and to discuss the international differences of the construction details and the used terminology to improve the results in daily practice.

This presentation will focus on the results of an applied research study funded by the Pedorthic Association of Canada (PAC). The research was conducted by an external consultant and the findings will be of great interest to the pedorthic community. The study, referred to as the PAC Patient Outcome Measure Evaluation

Strategy (POMES), is based in part on a survey of The Patients’ Perspectives on Custom Made Foot Orthotics. In addition to the patients’ perceptions, standardized information from Pedorthists themselves on their perspectives of the patients’ orthotic care will also be presented.

The survey data were collected from October 2014 through September 2015, and information on over 220 cases will be discussed. Major findings related to perceived foot pain pre- and post- orthotic use, as well as other important findings related to the impact of orthotics on activities of daily living, cost and insurance coverage will be discussed.

FRIDAY, APRIL 8 - 10:15 AM - 11:00 AMBreakout: Toward a Gender-Targeted Approach to Reducing Running-Related Pain: Insights From a Statistical Model of 154 Runners Michael Ryan, C. Ped (C)

It is widely understood that as an industry we must confront the challenge of run injury prevention through tools that can consider multiple potential risk factors concurrently. Contemporary statistical models allow researchers and clinicians to gain better insights on the relative weighting of one factor in the presence of all other potential factors. Every study that has developed a statistical model has used the presence or absence of a running injury as the dependent variable in the model – and in essence examines ‘injury-risk’. Despite the outcomes from these models being available now for over 30 years, our industry has made little ground at developing a definitive strategy for injury prevention. We need to re-consider our direction and base our models on the physiological phenomenon that underlies all running injuries: pain.

In 2013, our research group completed a large cohort study that served as a basis for collecting a comprehensive baseline dataset from male and female runners that examined running related pain across anatomical regions. The goal of this breakout session is to discuss the outcomes from this risk factor analysis, and to outline a gender-specific strategy to reduce running related pain. A secondary goal is to review the functional

joint and muscle testing used in the baseline study that can give insights into the cause or prevention of running related pain across gender.

"Each conference I attend, there appears to be a similar chatter when leaving a ‘researchers’ presentation: “I have no idea what he was telling us”, or “great presentation, but I can’t use that information in day-to-day practice”. There are large differences between research presentations and clinical presentations, and the means of interpreting the information are quite different.

The goal of my presentation “Transferring Research to Practice” is to educate pedorthists in two areas: 1) how to determine if research is answering a clinical question, and 2) tips/suggestions on interpreting researcher presentations.

This presentation is designed for pedorthists with minimal research knowledge. The presentation will include a review of study designs, hierarchy of evidence, appraisal of quantitative and qualitative research, bias, and how to understand results. In other words, how do we determine a clinical significant result? The process of evidence-based practice is often preached by clinicians, however few seem to understand the difference between strong and weak studies.

Anatomy has been fundamentally based on an “ isolationist “ model. We often focus on origin, insertion and action of individual muscles. As a result, many sports and rehabilitation taping methods are based on outdated concepts rather than applying what we now understand about movement, functional neurology and the somatosensory / fascial system. Instead of focusing on individual muscles, the primer for Rocktape is a focus on optimizing human movement and motor control. The FMT model ( Fascial Movement Taping ) is based on the idea that the body’s myofascial architecture functions as an integrated system or chain. Using tape, we can tap into the brain with the intent to reduce pain and improve motor control.

What is the advantage of Rocktape kinesiology tape

over more traditional taping methods? Rocktape, as a second skin that can stay on for 5 days, promotes both joint stability and movement congruency without limiting range of motion. Even though the main focus of the FMT model is movement taping, we can build on this model to include taping strategies and practices that reduce pain, improve motor control, reduce inflammation and enhance performance and recovery from exercise. We have collaborated on a research project with BCIT that has opened up new orthotic design ideas based on foot biomechanics and bony landmarks. Individuals were cast using two different methods: 50% weight bearing plaster slipper and non weight bearing foam box, creating two plaster moulds for each person.

In addition, subjects had their medial arch photographed while in 50% weight bearing. Adjustments were made to the plaster moulds in equal increments under the subtalar region of the foot. Each of these increments were digitized using a laser scanner and each individuals foot anatomy and location of the subtalar joint were discovered in the CAD program. Using these data, numerous insoles were generated for each individual. Each individual had to fill out a qualitative survey on insole comfort similar to a Modified PEQ developed by Dr. Silvia Rachke (BCIT Principle Investigator).

The survey results, 3D data and foot anatomy were used to identify new computer vision methods for landmark identification with the end goal of producing insoles with similar comfort to those generated using 3D point cloud data."

This presentation will provide an overview of the Great-West Life Special Investigations Unit (SIU) and its role both internally and externally in Canada. The costs of fraud incurred by the industry will be addressed as well as types and causes. Common “Red Flags” of fraudulent activity will be discussed to provide an awareness to the audience. Our investigators are in the field and make concerted efforts to identify and eradicate rogue and fictitious paramedical service providers. Time will be allotted for questions.

FRIDAY, APRIL 8 – 4:15 PM - 5:00 PMPlenary: The Search for the Perfect Diabetic OrthoticRobert Phillips

Diabetes has many biomechanical consequences on the body tissues that those who treat nondiabetic biomechanical dysfunction do not have to address. These include changes in tissue mechanics due to glycosylation and peripheral vascular disease as well as muscle changes and postural changes created by neuropathy. These are compounded by common biomechanical aberrations that lead to abnormal pronation and/or supination of the forefoot and rearfoot during gait. The perfect diabetic orthotic is one that minimizes abnormal pressure and shear, and also promotes maximum stability and foot function.

This presentation will direct itself to the evaluation process when the patient comes for the fabrication of a diabetic orthotic. It will discuss evaluation of soft tissue mechanics, as well as assessment of important postural and joint malalignments. It will then discuss methods of incorporating both the soft tissue problems and the foot kinetic and kinematic problems into an in-shoe orthotic.

Although many children with cerebral palsy (CP) rely on ankle-foot orthoses (AFOs) to address their gait and mobility limitations, clinicians lack evidence to guide selection of the most beneficial AFO design for each child. While current research suggests that AFO design and prescription are based on general factors like CP subtype, we expect that individual factors, such as the child’s physical impairments, are also considered. To

identify the factors that clinicians consider, we conducted focus groups in five Canadian pediatric rehabilitation facilities. This presentation will describe the categories and themes that emerged from these discussions, including: information that affects the choice of AFO design, processes for obtaining and monitoring orthoses, and factors that impact or challenge treatment outcomes. The session will also review the background literature and methodology for our current project, which examines the effects of a key component in the AFO prescription – the angle of the ankle in the AFO – on gait mechanics. Exploring these factors is a first step toward the development of guidelines to help clinicians improve AFO prescription for children with CP.

Falls are the second leading cause of accidental death. 75% of all falls occur in the people aged 65 and older. Even more frightening is the fact that 1 out of 3 older people who fall and are hospitalized with a hip fracture end up dying within 1 year.

The importance of falls prevention cannot be understated and we, as O&P professionals are in a unique position to play a major role in helping our at risk patients. Many of the conditions we treat elevate the risk of falling – arthritis, diabetes, peripheral neuropathy, vascular issues, amputations. Given the nature of our profession, we see a

disproportionate number of elderly patients.

This session will present attendees with many methods and modalities that O&P practitioners can provide for their patients to help decrease postural sway and increase balance and stability. Shoes, shoe modifications, foot orthoses, SCFOs and AFOs can all be utilized.

While some people may be justifiably critical of reductionist science there is no denying the scientific process is responsible for the remarkable health care

advances over the last 200 years that have resulted in, among other things, a doubling of our life expectancy and relative control of infectious disease.

However, in the pedorthic field, indeed as in most fields in medicine, we still require considerable progress to fully understand important elements of epidemiology, pathomechanics and treatment efficacy/effectiveness for the scope of injuries seen in our profession. We must also reconcile this need for information with current diminishing returns regarding the increase in clinical utility from each new scientific publication.

The goal of this talk is to review the importance of staying current with relevant research and discuss common ways clinicians can access contemporary research material that can provide new insights on advancing clinical innovations in pedorthics.

Donna Alteen is the founder and President of Time+Space Media, Atlantic Canada’s leading media services firm. The T+S team of professional consultants serve a growing roster of leading brands

Donna’s career began in the advertising sector in the early 80’s. She founded Time + Space Media, Atlantic Canada’s first specialized media agency, in 1988. Since that time, digital technology has changed the industry dramatically and today Donna actively works with clients and internal teams to provide strategic counsel grounded in measurable outcomes. Her professional experience spans three decades and a broad scope of industry sectors nationally and internationally, including Tourism, Energy, Communications and Professional Services.

Donna received her Business degree (Marketing Major) from Acadia University and currently serves as a member of the Board of the Christian Children’s Fund of Canada and the International Women’s Forum. Dr. Tyler Amell is a well-known and respected thought leader specializing in evidence-informed health strategies for insurers and employers. Dr. Amell is a Partner at Morneau Shepell, and in the

past has been the Chief Executive Officer of StaffRelay and Vice President of LifeMark/Centric Health.

Dr. Amell has significant experience in musculoskeletal injuries and illnesses, return to work, stay at work, disability management, mental health in the workplace, workplace health surveillance, biomechanics, ergonomics, whiplash associated disorders, occupational cancers, micro-gravity induced muscle disuse atrophy, occupational biomechanics, work physiology and other similar areas.

Dr. Amell has authored over 20 peer-reviewed publications, as well as book chapters and technical reports for industry. He has also given seminars and presentations at over 75 national and international conferences in areas of his expertise. He is a former instructor at the University of Calgary, University of Alberta, University of Ottawa and Queen’s University in physical therapy, anatomy, kinesiology, continuing education as well as occupational health programs.

Dr. Amell currently serves on the Executive Board of Directors of the Canadian Association for Research on Work and Health, and the College of Pedorthics of Canada. In the past Dr. Amell worked with the Conference Board of Canada on their attendance and disability management initiatives, contributing to several publications as well as speaking engagements.

Another of his areas of expertise is in applied allied health research, and using evidence, data and technology to make better, more informed business and health decisions when it comes to strategy, funding, prevention, assessment and treatment.

Graham Archer has been a C. Ped (C) for 14 years and is the President of Kintec in Vancouver, BC as well as the chief developer at Kiwi Software Solutions. He works with a team of people at

Kintec to consistently innovate new orthotic designs, source new materials and keep orthotic designs relevant to today's footwear, activity and workplace demands.

Graham is a past president of the Pedorthic Association of Canada and currently serves on the IGR committee.

Carly Fennell@wiivvit

Carly Fennell attended the University of Guelph and completed a bachelors of science and engineering in Biological Engineering. She then did her Masters in Mechanical

engineering focussing on the electromyography and biomechanics patterns in children with upper limb loss with and without their prosthetic limb. She worked at the University of Guelph for 4 years as an Engineering Lab Technician and cofounded a company in Guelph that was developing a pressure sensing insole for athletic and recreational activities. Carly started at Wiivv in March 2015 as the lead Biomechanical Engineer and is responsible for the validation research initiatives and implementing product development based on this research.

Dennis JanisseDennis Janisse is President and CEO of National Pedorthic Services, Inc., a national network of full-service pedorthic facilities with eight locations. In addition to his duties at NPS, Mr. Janisse is a Clinical Assistant Professor

in Department of Physical Medicine and Rehabilitation at the Medical College of Wisconsin and Clinical Director of Pedorthics for OHI, Inc. Dennis is presently on the Board of Directors of American Board for Certification

in orthotics, prosthetics, and pedorthics. He teaches pedorthics courses at Northwestern University Medical School and the Medical College of Wisconsin. He is past President of the Pedorthic Footwear Association and an honorary member of the American Orthopedic Foot and Ankle Society. He has written extensively on all aspects of pedorthics, with over 50 publications to his credit and has lectured throughout the United States and the world.

Kyra KaneKyra Kane, BscPT, MSc received her degree in Physical Therapy from Queen’s University in 1997 and a MSc degree in Kinesiology and Health Studies from the University of Regina in 2011. She is currently a Senior

Physical Therapist in the Children’s Program at Wascana Rehabilitation Centre in Regina, and is completing a PhD in Health Sciences at the University of Saskatchewan. Her research focusses on evidence-based decision-making and individualized AFO-footwear combination prescription to improve walking biomechanics in children with cerebral palsy. Kyra’s research is funded by the scholarships from the Pedorthic Research Foundation of Canada, the Physiotherapy Foundation of Canada, and the University of Saskatchewan.

Robert PhillipsRobert D. Phillips, DPM, is a 1979 graduate of the California College of Podiatric Medicine. He is a diplomate of the American Board of Foot and Ankle Surgery and also of the American Board of Podiatric Medicine. He is recognized for

his expertise in biomechanics and has published several award winning research papers and several book chapters on foot biomechanics. He has spent the past 18 years working for the Department of Veterans Affairs and is currently stationed in Orlando, Florida. A major portion of his practice is devoted to diabetic wound and post-wound care. He is Professor of Podiatric Medicine at the College of Medicine, University of Central Florida in Orlando, Florida, directing the 4th year medical clerkship in wound care. He is currently doing research on the mechanical properties orthotic materials.

Kelly RobbKelly Robb attended the University of Western Ontario, completing her undergraduate and pedorthic degrees in London, ON. She is the owner of KAL Orthoses, a small private pedorthic clinic in Southwestern Ontario. Kelly is

currently completing her graduate studies in rehabilitation sciences, focusing research on pedorthic care and Parkinson’s disease. Research data focuses on altering somatosensory perception of Parkinson’s patients through the use of textured top covers on custom orthotics. Preliminary data from her graduate work is goaled towards a PhD in September 2017.

Michael RyanC. Ped (C)

Michael Ryan has been involved in research in sports medicine for the past 15 years, with an interest in sports injury epidemiology, running biomechanics and tendinopathy. After graduating

with a PhD in Experimental Medicine from the University of British Columbia, he worked his first post-doctorial position at the University of Wisconsin-Madison in the Department of Orthopedics and Rehabilitation. In 2011, he moved to the Gold Coast of Australia after being awarded a Post-Doctoral Research Fellowship at Griffith University. He has published 33 peer-reviewed journals, been cited 1,068 times and maintains his research activity as an adjunct Senor Research Fellow at Griffith University.

Clinically, he has spent 8 years practicing as a pedorthist in private and hospital based services in Canada and the United States, and am currently the Director of Research and Development for Kintec Footlabs.

Garry SchmidtGarry Schmidt is attached to Great-West Life’s Internal Audit Department as the Manager of the Special Investigations Unit for the past 14 years. Prior to this he spent 20 years with the Winnipeg Police Service as a Detective

Sergeant working in specialized units such as Narcotics and Commercial Crime sections. He has been designated as an expert witness in both Provincial and Federal Courts associated to narcotics and voice recognition. Garry is presently an active member of the Association of Certified Forensic Investigators of Canada (ACFI)

Thomas StiefThomas Stief is the Director of the Research and Education Network Orthopedic Footwear Technology Germany (newOST). He designs and implements of practice-oriented R & D projects and conception of modern and

innovative educational standards for Orthopedic Footwear Technology. His research focus is in motor control, influence of orthopedic devices on musculoskeletal system and motor control, multiaxial stress analysis of the lower extremities His teaching focuses are lectures, tutorials and practical exercises in technical and clinical biomechanics, electrical and orthopedic measurement technics. His educational background is an orthopedic and biomechanics engineer, orthopedic footwear technician.
