BeiGene strives to bring you a choice of comprehensive medical plans, as well as the many other

plans and options that make up your benefits program. We remain committed to maintaining a

strong benefits program in 2021 and beyond. The following chart summarizes the benefit options

available to you and your family.

01

Eligibility

2021 Benef i t Guide p.7

ELIGIBILITY

Active, regular US employees working a minimum of 30 hours per week are

eligible for medical, dental, vision, life insurance, and disability insurance on

date of hire. Medical, dental and vision benefits end on the last day of the month

in which employment is terminated.

Eligible Dependents

▪ Your spouse;

▪ Your domestic partner;

▪ Your dependent children up to age 26 (regardless of marital status), including

a natural child, stepchild, a legally adopted child, a child placed for adoption

or a child for whom you or your spouse are the legal guardian;

▪ Your unmarried children age 26 or older who are mentally or physically

disabled and who rely on you for support and care; and/or

▪ Children of a domestic partner relationship, up to age 26 (regardless of

marital status).

Domestic Partner Coverage

You may cover your same-sex or opposite-sex domestic partner for certain

benefits. IRS regulations mandate that the value of the health care benefits for

your domestic partner are considered taxable income to the employee.

Contributions for the domestic partner and domestic partner's children need to

be made on an after-tax basis.

You will be required to complete an Affidavit of Domestic Partnership. Please

contact the Benefits Department for more details.

changes

MAKING BENEFITS CHANGES

In most cases, you may only make changes to your benefits during Open Enrollment in

November of each year, for a January 1st effective date. However, if you have a “qualified

life event,” you may make changes to certain benefits, as defined by the plan documents,

related to that event.

Qualified Life Events

You MUST complete the steps below within 30 CALENDAR DAYS of the qualified life event

to make changes to your benefit elections. For most events, changes are effective on the

first of the month following the date of the event:

Your marriage Your divorce or legal separation Birth, adoption or placement for

adoption of an eligible child

Death of your spouse or covered

child

Change in your or your spouse’s

work status that affects benefits

eligibility

A significant change in your or your

spouse’s health coverage

attributable to your spouse’s

employment

A change in your children’s

eligibility for benefits

Becoming eligible for Medicare or

Medicaid during the year

Becoming eligible for domestic

partner status in accordance with

BeiGene’s Domestic Partner policy

Gather supporting

documentation of the life

event.

Go online to your ADP

event, or email BeiGene Benefits

department at

COBRA

Under certain circumstances, you and your enrolled dependents can choose to continue

coverage under your health and welfare plans including the healthcare flexible spending

account, beyond the time coverage would have ordinarily ended. You may elect

continuation of coverage for yourself and your dependents if you lose coverage under the

plan because of one of the following qualifying events:

▪ Termination (for reasons other than gross misconduct)

▪ Reduction in employment hours

▪ Retirement

In addition, continuation of coverage may be available to your eligible dependents if:

▪ You die

▪ You and your spouse divorce or legally separate

▪ A covered child ceases to be an eligible dependent

If you experience a COBRA qualifying event you will receive a COBRA election notice from

our COBRA administrator with information regarding your cost and instructions on how to

apply for COBRA continuation.

If you and your spouse legally separate or divorce or your covered child ceases to be an

eligible dependent while you are on COBRA you must notify the COBRA administrator

within 60 days of the event.

CONSIDER YOUR CHOICES

Your Medical Plan Type Options

BeiGene provides access to high-quality, comprehensive care. The plans differ

largely in the way the cost of care is structured and in the strategies you can use to

control your expenses:

HMO Plans PPO Plans HDHP HSA Plans

Available to California

employees.

Available to all employees. Available to all employees.

Integrated health care that

offers a comprehensive,

convenient approach, but

require that you seek care

within the HMO network.

Allows you to choose care

inside or outside of the plan

network you selected, but

you will incur a higher out-

of-pocket cost for services,

if you use out of network

providers.

High deductible medical

plans that help protect

against large claims and

cover preventive care at

100% (deductible is waived

for preventive services). If

you enroll in this plan, you

are eligible to open and

contribute to a Health

Savings Account (HSA)

which can be used to help

pay for qualified medical

expenses. In addition, the

HSA also has an employer

contribution.

2021 Benef i t Guide p.13

CHOOSING A PLAN – KNOW YOUR

OPTIONS

We know you want the best benefit coverage with the fewest obstacles between you and

your healthcare.

Kaiser Permanente HMO – California Employees

Kaiser Permanente HMO offers an all-in-one approach to healthcare, with predictable

copays with no deductibles. Preventive care is covered at 100%.

Under the Kaiser Permanente HMO you and your covered dependents must receive all care

and prescriptions from Kaiser physicians at Kaiser Permanente facilities. With the exception

of emergencies, coverage is not available at non-Kaiser facilities. You must call Kaiser

immediately (or as soon as possible) after you are admitted to a non -Kaiser hospital for

emergency services.

Anthem Blue Cross PPO – All Employees

The Anthem Blue Cross PPO plan is a traditional plan that gives you the greatest flexibility,

combined with lower costs when you choose a provider within its broad national network.

Choosing health professionals who participate in the Anthem Blue Cross network keep your

costs lower and eliminate paperwork. How the plan works:

▪ It is recommended, however not required, that you choose a primary care physician

(PCP) to serve as your personal doctor and help coordinate all your healthcare needs.

Keep in mind; no referrals are necessary to see specialists in the Anthem Blue Cross

network. Just choose a participating doctor and make an appointment.

▪ Visit any licensed physician inside or outside the network. You will pay more when you

get care outside the network. Either way, most routine services require a copay or

coinsurance, and may involve meeting an annual deductible as well. Preventive care is

paid at 100% in network.

▪ All outpatient surgeries and hospital stays require pre-treatment authorization. Network

doctors may take care of this for you. If you go to a non-network provider and do not

obtain pre-treatment authorization, you may be subject to a non-compliance penalty, and

services determined not to be medically necessary may not be covered.

2021 Benef i t Guide p.14

Anthem Blue Cross HDHP (High Deductible Health Plan) – All Employees

The Anthem Blue Cross HDHP also provides flexibility to choose a provider within its broad

national network. When you are covered by a HDHP, you are eligible to participate in a

Health Savings Account (HSA). An HSA is an investment tool that helps you save for health

care expenses, including deductibles and coinsurance. Contributions to your HSA account

are pre-tax (federal and most states), and any interest earned on the account is tax -free.

In 2021, you may contribute via payroll deduction up to $3,600 [including employer HSA

contribution], if you have individual coverage, or up to $7,200 [including employer

contribution] if you are covering yourself and at least one additional family member.

If you are age 55 or older, you may contribute an additional $1,000 to your account.

Contributions to your HSA roll over from year to year and accumulate if not used. You may

use the funds to pay for any qualified health expenses incurred after the account is opened.

You may pay the bill directly via the HSA debit card, or you may use the HSA to reimburse

yourself for payments that you make. Payments and withdrawals made from your HSA to

cover qualified health care expenses are tax-free.

Watch the HDHP HSA video (available on https://app.strivebenefits.com/beigene under

Your Benefits, HSA) to learn about the ‘Things you need to know about HDHP and HSA

Am I Eligible To Participate In an HSA?

To be eligible you must meet a few criteria:

NOTE: HSA participants cannot Participate in the Healthcare Flexible Spending account

(FSA). However, you can participate in a Limited Purpose Healthcare FSA. Eligible

expenses with a Limited Purpose Healthcare FSA include most unreimbursed dental and

vision expenses (including expenses for your dependents).

*Some exceptions apply.

HSA Withdrawals

• Qualified medical expenses include anything from doctor’s office visits to dental or vision

care, prescription medications, over the counter medications, and many other health

care products.

• You can also use HSA funds to pay COBRA and long-term care insurance premiums,

though health insurance premiums are not qualified unless you are receiving

unemployment benefits.

• Withdrawals for non-qualified expenses are taxable and carry a 20% penalty unless you

are age 65 or older. If you are age 65 or older, you can withdraw funds for non -qualified

expense and pay ordinary income taxes.

How Do I Access My HSA Funds?

DIRECT PAYMENT: When you use your HSA debit card, your expense is automatically

paid from your account

PAY YOURSELF BACK: Pay for eligible expenses with cash, check or your personal credit

card, then withdraw funds from your HSA to reimburse yourself. You can even have your

payment deposited directly into your checking or savings account. For more details visit

www.naviabenefits.com

use your HSA debit card where applicable.

Summary of Benefits and Coverage (SBC) and Uniform Glossary

Group health plans are required to provide you with an easy-to-understand summary about

a health plan’s benefits and coverage so that you can better evaluate your choices.

The SBC form also includes details, called “coverage examples,” which are comparison

tools that allow you to see what the plan would generally cover in two common medical

situations. The SBC’s standardized and easy to understand information about health plan

benefits and coverage allows you to more easily make “apples to apples” comparisons

between our insurance options.

You are encouraged to read and explore the SBC’s and Uniform Glossary available for your

medical plans posted on https://app.strivebenefits.com/beigene.

Important Terms

COINSURANCE: The amount you pay for covered services after you pay the deductible.

For example, if your plan has coinsurance of 0% and you have already paid the deductible,

the plan pays 100% of the costs and you pay 0%.

COPAY: The amount you pay up-front for your visit.

COVERED SERVICES: Those services deemed by your plan to be medically necessary

for the care and treatment of an injury or illness.

DEDUCTIBLE: The amount you pay before the plan starts to pay. For example, a PPO plan

requires a $500 deductible for an individual using in -network services. This means that you

pay the first $500 in medical care you use (please note, the deductible is not applicable to

all services).

FORMULARY: A list that contains the approved medications that are part of your

prescription drug plan.

GENERIC: An FDA-approved drug, composed of virtually the same chemical formula as a

brand-name drug.

OUT-OF-POCKET MAXIMUM: The most you will pay for covered medical expenses in a

year. Once you reach your out-of-pocket maximum, the plan pays 100% of your covered

medical expenses for the balance of the year.

EXPLANATION OF BENEFITS (EOB): It’s not a bill, but a statement sent by your health

insurance company to explain what medical treatments and/or services were paid for on

your behalf.

Dental PLANS

BeiGene offers two competitive PPO dental plans through MetLife to help you maintain your oral health. If

you choose to use a dentist that is not part of the network, your benefit will be based on the Usual and

Customary (U&C) allowance for the dental procedure performed. You will be responsible for the charges

over the Usual and Customary allowance in addition to your portion of the coinsurance.

Network providers will submit claims on your behalf directly to MetLife. However, if you receive services

through non-network providers, you may be required to submit your own claims.

Please note – you will not receive a benefits card for your MetLife Dental Insurance. You may view your ID card by

account funds at

the end of the year

BeiGene’s Health Care and Limited Purpose FSA plans have a $550 carryover provision. Any

unspent funds up to $550 will be carried over to the 2022 plan year. Any unspent amounts over

$550 will be forfeited. Consult your plan summary for more details. The carryover provision does

not apply to day care. If you have not spent your day care funds by the end of the year they will be

forfeited. The amount you carry over does not count toward the annual maximum for 2022.

Here’s how the FSA plans work:

You can also visit www.FSAstore.com to purchase approved health care items.

Flexible Spending Accounts are a great way to save money because your eligible expenses are paid using

tax-free dollars. You don’t pay Federal, FICA or most state income taxes on contributions you make to the

FSA. Depending on your tax bracket, you may save as much as $40 for every $100 you contribute to an

FSA.

COMMUTER BENEFITS

Commuting to work each day can be expensive, so we work with Navia Benefits to help you save money on

your commuting costs. Through this program, you simply go online to electronically place your transit order for

your commuting needs. This benefit is strictly for the use of commuting from your house to a BeiGene office

location.

BeiGene will provide $270 a month subsidy toward your Transit and/or Parking election(s) combined.

BeiGene will provide a $20 a month subsidy toward your Bicycle expenses if you use a Bicycle to get to work.

The bicycle subsidy is a taxable benefit. If you are receiving any other commuter benefit, you are not eligible

for the bicycle subsidy.

How Does The Program Work?

Using the Navia website, you will create an account and place orders for transit and parking products. Up to

$270 will load to your Navia account (debit card or directly to the transit authority) on the first of the month

following the month for which you placed your election. For elections greater than $270, you will see a

deduction from your paycheck.

Access To Your Account/ First Time Login

2. Select “Register” from the top right-hand corner

3. Select “I’m a participant”

DISABILITY plans

Short Term Disability Insurance (STD)

If you cannot work because of an illness or injury for more than 7 calendar days, BeiGene provides you

with The Standard’s short-term disability plan (STD) that helps replace a portion your income. BeiGene

provides STD coverage at no cost to you.

After 7 calendar days of disability, you will be eligible to receive a weekly benefit based on a percentage

of your weekly salary. The amount of your benefit is based on your weekly income in effect just prior to

your disability date. Your weekly salary is defined as your base salary.

Long Term Disability Insurance (LTD)

If you continue to be disabled for 90 days, you may be eligible to receive a monthly benefit based on a

percentage of your monthly salary. The amount of your benefit is based on your monthly income in

effect just prior to your date of disability. Your monthly salary is defined as your base salary. BeiGene

provides LTD coverage at no cost to you, however, the cost of the LTD premium is added to your W2

each year (as imputed income) to provide you with a tax -free benefit when you need it most.

The LTD plan does not cover any disabilities caused by, or resulting from, a pre -existing condition. You

have a pre-existing condition if you received medical treatment, consultation, care or services including

diagnostic measures, or took prescribed drugs or medicines in the 3 months just prior to your effective

date of coverage; or you had symptoms for which you would have consulted a health care provider in the

3 months just prior to your effective date of coverage; AND disability begins in the first 12 months after

your effective date of coverage.

Critical Illness

The treatment to recovery is vital, but it can also

be expensive. If treatment keeps you out of work,

the financial worries can grow quickly, and stress

levels may rise. Critical Illness coverage helps

provide financial support if you or your loved ones

are diagnosed with a covered critical illness.

The program typically pays a specified lump sum

benefit for the initial diagnosis of conditions such

as heart attack, severe stroke, permanent

paralysis, invasive cancer, end-stage renal failure,

or major organ failure. You can use the lump sum

benefit any way you wish. When a diagnosis

occurs, you need to be focused on getting better

and taking control of your health, not stressing

over financial worries.

This plan covers you and your family for the

monthly cost as noted in table below.
