Even though costs of conducting the camp have gone up in recent years we are trying to keep your cost down because WE BELIEVE IN THIS CAMP and its ministry, so we want to keep the prices down in hopes of making it affordable for everyone who wants to attend. - Coach Barber Savings Register in February, take $ ff the total cost. Campers who have attended 5 years or more will be inducted into the PBA Hall of Fame and will receive a special gift and ceremony during camp. Each morning we start camp with prayer and a daily challenge – a motivational start for the day’s activities. Each evening we have praise and worship with a worship leader (singing and music), and a message from a Christian perspective. Although we are about lots of basketball, this sets us apart as a camp with SOMETHING MORE TO OFFER. • Offensive individual open court moves • Team defensive fundamental and drill work • Ball handling and scoring • Individual defensive skills • Post moves for scoring • Full court pressing defense and fundamentals • Team offensive fundamentals, screening, cutting, & drill work * The NCAA has a required rule regarding an educational component at all basketball camps offered on college campuses. The Panther Academy mission and goals will embrace this addition with an added session on seven statistically proven ways to improve your grades in school, and National Academic standards for athletic participation in college. I/We the undersigned (name: ______________________) parents or guardian of a minor participating in Basketball Academy at Greenville College in Greenville, Illinois, hereby authorize the participation of, and accept responsibility for the attendance of the said minor in the above camp on the Greenville campus and all activities in connection therewith, conducted under the auspices of the Greenville College Department of Athletics.

I/We request that said minor be permitted to participate in said camp, having been fully and completely informed and advised regarding the nature and purpose of said camp and the activities conducted there under.

I/We acknowledge that Greenville College, through its agents and employees, extended every effort to inform, advise, and acquaint each of us with the camp and purpose of participation in these activities by the child so named above. It is my/ our full and free decision to allow said child to participate.

I/We verify that my/our son or daughter is not academically eligible to enter college in the fall of 201 . Furthermore, I/we verify that our son/daughter is in good health, and hereby authorize the directors of the Basketball Academy to act for me/us, according to their best judgment, in any emergency requiring medical attention.

Since the law requires that parental permission be obtained for most medical procedures on minors. I/we wish to give permission for the trainer of Greenville College to perform such diagnostic, therapeutic, and operatic procedures as they deem necessary for my/our son/daughter.

I/We understand that my/our son/daughter will allow procedures to be promptly carried out so that no unnecessary delays will occur with treatment. No operation will be performed, except in extreme emergency, without parents being contacted and fully informed and their consent obtained.

I/We also understand that the Summer Conference Center Director/Staff has the right to send my/our son/daughter home without refund for damages, inappropriate activities, or misconduct and that I may be billed for room damages, lost keys, or other replacement costs resulting in theft or danger.

Neither the Board of Greenville College, or directors or anyone connected with the camp assumes any responsibility for accidents, medical, dental, or any other expenses incurred as a result of accidents while attending or participating in the Academy.
