The 8th Annual ENT for the PA-C CME conference is provided by the American Academy of Otolaryngology - Head & Neck Surgery Foundation (AAO-HNSF) and the Society of Physician Assistants in Otorhinolaryngology / Head & Neck Surgery (SPAO-HNS) in collaboration with The Mayo Clinic.

The ENT for the PA-C Conference provides unparalleled CME opportunities for PAs and other medical professionals interested in or specializing in otolaryngology. Our workshops maximize hands-on learning, with concise content and small group sessions. This conference is also the ideal setting to network with ENT professionals from across the country. Lecture topics have been selected based on relevancy, practice need and audience requests from over eight years of lecture series. This program has been reviewed and is approved for a maximum of 116.00 hours (a maximum of 38 earnable by an individual) of AAPA Category 1 CME credit by the Physician Assistant Review Panel. Physician assistants should claim only those hours actually spent participating in the CME activity. This program was planned in accordance with AAPA's CME Standards for Live Programs and for Commercial Support of Live Programs.

Steve Bansberg, MD Dr. Stephen Bansberg is an ENT-otolaryngologist in Phoenix, Arizona and is affiliated with Mayo Clinic Phoenix. He received his medical degree from Rush Medical College and has been in practice for more than 20 years. Jamie Bogel, AuD, PhD Jamie M. Bogle, AuD, PhD is an Assistant Professor of Audiology at Mayo Clinic Arizona. Her primary clinical focus includes evaluation of children and adults with neurological conditions, including concussion and migraine. Dr. Bogle's research interests include evaluation of children at risk for dizziness and imbalance and the development of novel diagnostic techniques to better evaluate complex conditions. Susan Broquard, PA-C, MS Susan graduated from A.T. Still University in 2004 with a Master of Science degree in Physician Assistant Studies. She has practiced in a variety of areas of medicine, including Family Practice, Emergency Medicine, and Occupational Medicine. During the last 6 years of her career, she has worked in ENT and Allergy/Asthma. She joined Mayo Clinic in the Allergy, Asthma, and Clinical Immunology division in 2017. In these roles, she frequently evaluates and treats patients with allergic rhinoconjunctivitis. Mike Cevette, MD Michael J. Cevette, Ph.D. Chair and Professor of Audiology, Department of Otorhinolarygology, Co-chair Aerospace Medicine and Vestibular Research Laboratory (AMVRL), Mayo Clinic Arizona. The research interests of Michael J. Cevette, Ph.D., span several areas that involve the function of the vestibular system and human hearing. Over the past several years, Dr. Cevette and the AMVRL team have investigated vestibular illusions underlying spatial disorientation in the aerospace environment, a universal listening system for enhanced speech understanding, and the development of eye tracking technology for rapid assessment of cognitive performance. Wenli Chen, SLP Wenli Chen, CCC-SLP, is a speech-language pathologist under the department of Otorhinolaryngology at Mayo Clinic Arizona. Her practice predominately includes rehabilitation of dysphonia, and swallowing/TEP management in the head and neck cancer population. She is also involved in voice research, and published several peer reviewed papers on the topic of high-speed videoendoscopy in normal and pathological vocal folds. I have worked alongside our laryngologist, Dr. David Lott, since 2011 seeing and treating many complex laryngotracheal disorders addressing aspects of voice, swallowing and breathing complaints. I obtained my Bachelor's degree in Kinesiology from Arizona State in 2008, Master's in Physician Assistant Studies from A.T. Still University in Mesa, AZ in 2010 and completed a post-graduate fellowship in Otorhinolaryngology/Head and Neck Surgery from Mayo Clinic Arizona from 2010-2011. Jason Fowler, PA-C After spending the better part of the last 10 years working in Otolaryngology in northwestern Pennsylvania, Mr Fowler recently opted for a career change and has transitioned into the world of emergency medicine. He currently works for US Acute Care Solutions at St Vincent Hospital in Erie, PA. Mr. Fowler is active in the educational efforts of USACS and is assisting in the development of core otolaryngology curriculum for all APP personnel within the USACS system. Currently USACS serves 180 sites in 22 states. Mr Fowler is also a past-president of SPAO and serves as adjunct clinical faculty in the Phys Assistant department at Mercyhurst University. He has lectured at numerous state and national meetings and has extensive list of journal articles and book chapters to his credit. Marie Gilbert, PA-C Emeritus, DFAAPA, CPMA, CENTC SPAO CME Chair Marie Gilbert has retired from private practice Otolaryngology after 37 years as a PA-C and office administrator. She has been a preceptor, instructor, and lecturer for PAs and NPs. She has served eight years as SPAO’s and AAPA’s Liaison to AAO-HNSF. She has directed previous years of the ENT for the PA-C Conference, and serves on the AAO-HNSF APP Education Task Force. She continues to teach, and is available as an ENT auditor & consultant. Catherine Hart, MD Catherine is the newest addition to the faculty at Cincinnati Children’s. She has specialized training in pediatric airway reconstruction through her fellowship at Cincinnati Children’s Hospital Medical Center under the guidance of Dr. Robin Cotton. She is pursuing additional training in quality improvement and outcomes research through the Quality Scholars Program at the James M. Anderson Center for Health Systems Excellence. Hines, MD is a practicing Internist in Scottsdale, AZ. Dr. Hines graduated from University of Mississippi School of Medicine in 2013 and has been in practice for 4 years. She completed a residency at Mayo Clinic Scottsdale. Dr. Hines also practices at Khaled M Saleh MD in Phoenix, AZ. Mike Hinni, MD Dr. Hinni is a fellowship trained head and neck surgeon with extensive experience treating head and neck cancer and oropharyngeal cancer in particular. In the 20 years since joining the faculty at Mayo Clinic, he has pioneered new surgical techniques that reduce the morbidity of the surgical resection of oropharyngeal cancer. This included the design and introduction of new surgical instrumentation and the creation of one of the world's largest databases of patients with oropharyngeal cancer. Now as the chair of the department, he is supporting several research and educational initiatives regarding improving the value of treatment for oropharyngeal cancer and the causative and evolving role of HPV. He enthusiastically supports the proposed research and will be an active participant in the execution of the project. David Holley, PA-C David Holley currently works as a Physician Assistant in the department of Otolaryngology - Head & Neck Surgery at the Mayo Clinic in Arizona. Currently, he practices primarily in the General ENT outpatient clinic. He graduated with his MPAS from the University of Texas Medical Branch in Galveston, Texas. Upon graduating he completed a one year postgraduate fellowship in Otolaryngology - Head & Neck Surgery at the Mayo Clinic. Brittany Howard, MD Brittany E. Howard, MD is a practicing Ear, Nose & Throat (ENT Specialist) in Scottsdale, AZ. Dr. Howard graduated from University of New Mexico School of Medicine in 2010 and has been in practice for 8 years. She completed a residency at Mayo Clinic and Medical School. Joe Hoxworth, MD Joseph M. Hoxworth, M.D. is the Chair of the Neuroradiology Division in the Department of Radiology at Mayo Clinic in Arizona. He received a Bachelor of Science in Chemical Engineering from Case Western Reserve University where he was a summa cum laude graduate with induction into the Tau Beta Pi engineering honor society. Dr. Hoxworth completed his medical degree at the same institution where he was elected into Alpha Omega Alpha, during which he also participated in a clinical research training program at the National Institutes of Health where he received a

He then completed his residency in diagnostic radiology at the University of California San Francisco where he served as chief resident. Following completion of a 2 year fellowship in neuroradiology at the University of California San Francisco, he joined Mayo Clinic in 2007 where he is currently an Associate Professor of Radiology with a joint appointment in the Department of Otolaryngology – Head & Neck Surgery. Dr. Hoxworth’s primary research focus within the discipline of Neuroradiology is head & neck radiology with a particular emphasis on sinonasal imaging and head & neck cancer. Stacey Ishman, MD Stacey Ishman is the Surgical Director of the Upper Airway Center and Director of Otolaryngology Outcomes Research at the Cincinnati Children’s Hospital Medical Center and a Professor of Otolaryngology—Head and Neck Surgery and Pulmonary Medicine. She earned her medical degree from Rush Medical College and completed her Residency in Otolaryngology at the Medical College of Wisconsin. She then completed a fellowship in pediatric otolaryngology and underwent advanced sleep training at Johns Hopkins before joining the faculty from 2005 through 2013. She earned her Master’s in Public Health from the Johns Hopkins Bloomberg School of Public Health and is Board certified in Otolaryngology – Head and Neck Surgery and Sleep Medicine. While at Johns Hopkins, Dr. Ishman founded and directed the Center for Snoring and Sleep Surgery. She joined the divisions of Pediatric Otolaryngology and Pulmonary Medicine at Cincinnati Children’s in 2013 where her clinical practices are focused on the management of children and adults with obstructive sleep apnea. She also serves as past-chair of the Board of Governors and as a member of the Board of Directors of the American Academy of Otolaryngology – Head and Neck Surgery as well as President of the International Surgical Sleep Society. Laura Kirk, MSPAS, PA-C, DFAAPA Laura Kirk, PA-C is the Senior PA Supervisor in the University of California at San Francisco Department of Otolaryngology - Head and Neck Surgery. Laura serves on the UCSF AHP Executive Advisory Board, the UCSF Committee for Interdisciplinary Practice, and the Strategic Planning Committee for the UCSF Director of Advance Practice. She is the only non-physician educator and facilitator in the UCSF Center for Enhancing Communication in Healthcare. Laura also serves as a Director at Large for SPAO-HNS. She has a strong interest in advocacy, from everyday communication, to the office of the department chair, to the institution C-suite. Currently she is an Instructor in Otolaryngology at the Mayo Clinic in Arizona. Her primary work is in Head & Neck cancer surgery in a variety of cases spanning from TORS to complex microvascular reconstructions. In addition, she has established and continues to run a chapter for SPOHNC (support for people with oral head and neck cancer) cancer survivors throughout the Phoenix, Arizona area. She is an avid runner and triathlete and enjoys traveling with her husband. Kimberly Lakhan, PA-C Dr. Kimberly Lakhan is an assistant professor with the College of St. Scholastica’s PA program and a PA in Otolaryngology at Essentia Health-Duluth Clinic where she specializes in vestibular disorders and migraine manifestations in OHNS. David Lott, MD David Lott, M.D. holds the academic rank of Associate Professor of Otolaryngology at Mayo Clinic College of Medicine. He is a consultant in the Department of Otolaryngology Head and Neck Surgery and serves as the Director of the Voice and Swallowing Program at the Mayo Clinic in Arizona. His practice includes voice and swallowing restoration, laryngeal cancer, and laryngotracheal reconstruction. Mike Marino, MD Michael J. Marino, MD is Senior Associate Consultant at the Mayo Clinic in Phoenix, AZ. He completed fellowship training in rhinology and endoscopic skull base surgery at the McGovern Medical School at the University of Texas at Houston. He was a medical student and otolaryngology resident at the Tulane University School of Medicine in New Orleans. After undergraduate, he completed his medical training at Rutgers University-Robert Wood Johnson Medical School and completed a surgical internship and residency at the University of Miami Miller School of Medicine and the Jackson Memorial Hospital. After completing his fellowship in Pediatric ENT at Children’s National Medical Center in Washington, DC, Dr. Mancuso was an assistant professor of Pediatric Otolaryngology and Broncho-esophagology at St. Christopher’s Hospital for Children in Philadelphia. After moving to Arizona, Dr. Mancuso was a past chairperson of the department of Pediatric Otolaryngology and the department of Pediatric Surgery at Cardon Children’s Hospital-Banner Desert Medical Center from 2008 to 2014. Dr. Mancuso has since joined Phoenix Children’s Medical Group and the Division of Otolaryngology at Phoenix Children’s. Throughout his career, he has published numerous clinical publications and presentations and has been named a Phoenix Magazine “Top Doc” for several years. Jodi McCollum, AuD Jodi McCollum, Au.D., a clinical audiologist at Mayo Clinic in Scottsdale, AZ. I have worked at Mayo Clinic for 6 years. I perform diagnostic testing for hearing disorders, fit amplification and assistive listening devices, teach aural rehabilitation for patients with hearing loss, provide counseling for patients suffering from tinnitus, and am an instructor who supervises anywhere from 2-5 audiology students, each year.
