Application Cost:

If you decide to apply to rent one of our properties all adult applicants over the age of 18 must submit a fully completed and

signed rental application and processing fee ($75.00 per adult). Applicants must submit all required and/or requested

documentation within five (5) business days of the original application date or the application will be closed as “incomplete”.

Incomplete applications or applications submitted without complete documentation or processing fees will not be processed,

applications fees are NON-REFUNDABLE. Applicant must provide a photo ID (Driver’s License, Military ID or State Issued ID

Card).

Application Processing Time Frame:

Processing an application normally takes between 2-3 days. Some Homeowner’s Associations or Condo Associations may

require an additional separate application, fee and/or HOA security deposit. In some cases, application processing by

Homeowner Associations, Condo Associations or other unforeseen circumstances may require some applications to take

longer. You will be contacted immediately upon determination of approval or denial. Approval by the Homeowners

Association or Condo Association is a necessary prerequisite to our approval of your application.

No rental property will be held vacant for longer than two (2) weeks unless approved by Management.

Background Check and Employment/Rental History Verification:

Once we receive your application you hereby authorize us to (1) check your credit report; (2) check public records for any past

convictions; (3) verify your employment; (4) verify your current and previous landlord references; (5) complete a criminal

background check; (6) screen for any OFAC positive results.

We would encourage you to not apply if you have bad credit, owe another landlord money, have a history of eviction, have not

been with your present employer at least one year (unless transferring to area with same employer or military) . Criminal

records must contain no convictions for felonies within the past 7 years involving the illegal manufacture or distribution of

controlled or illegal substances. For other felony convictions, we will conduct individualized assessments that take into account

mitigating factors, such as facts & circumstances surrounding the criminal conduct, age at time of conduct, evidence of good

tenancy before and after conduct, nature & severity of conviction and the amount of time that has passed since the conviction.

Criminal history which indicates that an applicant's tenancy would constitute a direct threat to the health or safety of other

individuals or whose tenancy could result in substantial physical damage to the property of the owners or other may result in

rejection of the application. Prior foreclosure or short sale approved if applicant has been with their current employer for a

minimum of two years or prior residency two years.

Notification:

Once you have been notified of your approval, you must pay a holding fee of one month's rent for the property in the amount

specified by the property manager within 48 hours. Due to the high demand for rental homes we will not hold the property off

the market for more than 48 hours unless you have submitted the required holding fee. Once this holding fee is paid it is NON-

REFUNDABLE. Once you have entered into the Lease Agreement the holding fee will be applied to the Security Deposit for the

home. In the event you fail to enter into a Lease Agreement or take possession of the property on or before your scheduled

beginning lease date you shall forfeit the holding fee as liquidated damages. If you do not comply with this requirement we

may rent the home you were approved for to another applicant. All initial funds, to include but not be limited to the holding

fee, security deposit, first month’s rent and pet fee must be paid by cashier’s check or money order ONLY. Personal checks will

NOT BE ACCEPTED under ANY circumstances.

Income:

Applicants must have a combined gross income of at least three (3) times the monthly rent. Income(s) must be verifiable,

applicant must provide pay stubs, W2’s, 1099’s, copies of tax returns or bank statements as requested to accommodate such

verification. Applicant must have been at their current job for at least 1 year unless they are transferring to live/work in the

Tampa area with the same employer (military are exempt). Co-signers and/or higher security deposits (if applicable to the

property you have applied to rent) are considered on an individual basis and require homeowner approval. Applicants who do

not qualify may potentially be denied if a homeowner chooses not to allow a co-signer or higher security deposit. Any requests

for an exception to these criteria will need to be submitted in writing through Florida State Unlimited Realty, Inc. to the

property owner for approval.

Credit History: A minimum credit score of 620 is required. Credit history must not contain slow pays, judgments, eviction

filings, collections, liens or bankruptcies within the past 5 years. Applicants must not owe any current or previous landlord

money for rent, fees, utilities or any other monies due. We will not provide you with a copy of the credit report or tell you of its

contents, however, we will provide you with the name and contact information of the credit reporting agency so you may

request a copy from them. All information collected for the processing of the rental application is considered confidential in

nature and for company use only.

Criminal Records: Criminal records must contain no convictions for felonies for crimes within the past 7 years involving

violence against persons, damage or destruction of property, manufacture or distribution of controlled substances and no

sexual offenses ever.

Rental History: Rental history verification from current and/or previous landlords must reflect timely payments, sufficient

notice to vacate, no complaints regarding noise, disturbances or illegal activities, no NSF payments, and no damage to the

rental property or failure to leave the property in a clean and undamaged state upon move out.

Renters Insurance Required/ Fees to Agent: Tenants are required to carry Renters Insurance. If you do not already have

renters insurance, information will be provided to you prior to move in so you may obtain a policy/declarations page.

Lease Changes: In the event we must re-order the lease from our Attorney due to last minute changes on your behalf (move-in

date, occupants, etc.) you will incur a $100 lease change fee.

Occupancy Standards: Current occupancy standards area a maximum of 2 persons per bedroom, except for children under the

age of four (4). Certain city and county municipalities and/or Homeowners Associations prohibit more than two (2) unrelated

adults to reside in a single family dwelling unit. Consequently, we also prohibit the rental of a single family dwelling unit to

more than two (2) unrelated adults.

Pets: Pets are allowed only with the express written permission of the homeowner. PLEASE ASK IF YOUR RENTAL ALLOWS

PETS, DO NOT ASSUME THAT IT IS OK TO BRING A PET INTO A RENTAL UNIT WITHOUT FULL DISCLOSURE AND WRITTEN

CONSENT OF THE LANDLORD/AGENT. Pets include dogs, cats, birds, fish, turtles, etc. If pets are allowed at your rental unit,

there is a one-time, non-refundable pet fee due and payable prior to move in or prior to pet occupancy that must be paid. The

following pets will not be allowed under any circumstances: German Shepherds, Dobermans, Pit Bulls, Chows or Rottweilers.

Medically necessary service animals are exempt from the afore mentioned pet fee and other requirements, however proper

documentation of the service animal must be submitted upon request of the landlord/agent.

Collection Policy: We reserve the right to report all non-compliance with the Lease Agreement or failure to pay rent or any

other amounts owed to the credit bureau and/or collection agency in accordance with the law. Homeowners reserve the right

to pursue such amounts due by initiating their own legal action in civil court against the tenant(s) exclusive of Florida State

Unlimited Realty, Inc. management of the rental unit and tenant.

Rent: Due on the 1st and late if not paid in full by midnight on the 3rd day of each month. Late fees are a flat 10% of the

monthly rental rate. Accumulated unpaid late fees and other fees such as NSF, etc. may be deducted from tenant(s) security

deposit at the termination of occupancy.

Multiple Applications: Management may receive multiple applications from unrelated individual applicants on the same

property at the same time. If such is the case then all applications will be processed for equal consideration. Final approval of

any application will be determined by the owner of the property and not by order in which the application is received.

Applications may continue to be accepted and placed in a backup position until a lease has been fully executed between tenant

and landlord.

Repairs/Maintenance: All work orders must be placed online through Appfolio. Tenant(s) are 100% financially responsible for

repairs which are a direct result of tenant negligence and/or if tenant initiates their own service call. Tenant acknowledges full

financial responsibility for any HOA fees or fines levied against homeowner by the HOA which are the direct result of tenant

negligence. Tenant is responsible to change the air conditioning filter every 30 days. Failure to do so may cause the A/C

system to malfunction in which case tenant may be financially responsible for repairs if it is determined that malfunction was a

direct result of tenants failure to change the filter on a regular monthly basis. Tenant will be charged a $75 service call fee if

tenant is not available to grant access to a vendor or agent at an agreed upon time for a maintenance service call.

Tenant will be provided with a move-in inspection form which should be completed, signed and returned to our office within 5

days of move in or it will be considered VOID. Upon lease termination, unit should be left in same clean and ready repair

condition as it was upon move in. A move out inspection will be done after you have completely vacated the property and

returned keys and remotes signifying your surrender of possession of the property. Inspections are done by the Property

Manager alone, please DO NOT ask that we delay our inspection or honor any request that you be present. Your move in form

will be utilized to assist in determining any charges to be levied against the Security Deposit.

This Release and Authorization acknowledges that Florida State Unlimited Realty, Inc. may now, or at any time

while I am renting, conduct a verification of my current and previous tenant history, current and previous tenant

history, current and previous employment, credit history, contact personal references, and to receive any criminal

history information pertaining to me which may be in the files of any Federal, State or Local criminal justice

agency, and to verify any other information deemed necessary to fulfill the tenant/application requirements. The

results of this verification process will be used to determine tenant eligibility under Florida State Unlimited Realty,

Inc. tenant policies. In the event that information from the report is utilized in whole or in part in make an adverse

decision, we will provide you with an Adverse Action Notice and a description of your rights under the law.

I authorize Background Info USA and any of its agents, to disclose orally and in writing the results of this

verification process to the designated authorized representative of Florida State Unlimited Realty, Inc.

I have read and understand this Release and Consent, and I authorize the background verification. I authorize

persons, schools, current and former employers, current and former landlords and other organizations and

agencies to provide Background Info USA with all information that may be requested. I hereby release all of the

persons and agencies providing such information from any and all claims and damages connected with their

release of any requested information. I agree that any copy of this document is as valid as the original.

I do hereby agree to forever release and discharge Florida State Unlimited Realty, Inc., Background Info USA and

their associates to the full extent permitted by the law from any claims, damages, losses, liabilities, costs and

expenses, or any other charge or complaint filed with any agency arising from retrieving and reporting of
