IntroductionThis workbook looks at the 4WayGlide and TwinSheet4Glide in bed systems for positioning users with complex needs It includes ldquohow tordquo videos a look at single handed care bariatric users incontinent users pressure care optimal positioning and safe transfers with contributions by Mike Cowan-Jones Independent OT with ARMS Rehab Ltd

4WayGlideThis in-bed system is for turning and positioning immobile users with ease either manually often with a single carer or with a hoist It can be used on pressure care mattresses and is ideal for use with a profiling bed It is available in 2 sizes - standard or bariatric and can be used with some of the most challenging cases whether bariatrics or complex medical conditions

The 4WayGlide consists of a nylon base sheet and a top quilt (either breathable PU or cotton polyester) The underside of the glide mattress and the nylon sheet combine to provide an ultra-low friction surface for movement in all directions for easy turning and positioning

TwinSheet4Glide SystemThe in-bed system for incontinent users This 2 part glide mattress is for turning and

positioning incontinent users with complex needs - very limited mobility dementia pressure sores or bariatric patients To further assist the carer with turning the patient it can be used with a hoist (see page 5)

Disconnecting the lower part of the glide mattress provides easy access for personal hygiene care of pressure sores and dressing

Immedia 4WayGlide amp TwinSheet

In-bed systems for complex needsSingle Handed CareThere are sigificant benefits in using products that promote single handed care including lower costs and a positive impact on a users privacy and general well being

The 4WayGlide and TwinSheet4Glide systems enable users to be moved safely with minimal disruption with and without a hoist and using a minimal number of carers

Above client is supported on their side by use of a hoist The position of the top half of the sheet is helpful to stop the client hitting out at the carer or prevent injury during a spasm or seizure

Incontinent usersIn addition to the benefits of the 4WayGlide the TwinSheet4Glide system is designed for incontinent immobile users

The split design of the Twinsheet makes delivering personal care easier As shown right the bottom half of the sheet is removed for personal care

It can be used for patients with behavioural challenges dementia spasms seizures and bone metaseses (cancer)

As Mike explains

ldquoFor palliative care when clients find hands on care painful the 4WayGlideTwinSheet provides a dignified alternative It is compatible with pressure relieving mattresses and is comfortable for the clientrdquo

Pressure Relief amp Shear ReductionIn the UK pressure ulcer care costs pound14bn ndash 21bn per year More than 90 of the cost of treatment is the cost of nursing time

The best way to prevent pressure ulcers is pressure distribution and minimising shear and friction and also frequent repositioning

The 4WayGlide system reduces the amount of handling required to move a client As it

Bariatric usersThe 4WayGlide system can reduce the number of carers needed bariatric users It allows easy low friction movement and provides a permanent on the bed solution where clients require frequent repositioning enhancing dignity and reducing the risk of injury to the client and carers

The system comes in a wider option to fit bariatric beds

Benefits of Optimal Positioning

Learn more about the Positioning Wedge

Find out about MultiGlide Glove

Sitting up in bedThe 4WayGlides ldquolock on sheetrdquo system allows more control and prevents the patient from sliding down the bed

The non-slip section can be covered or uncovered depending on whether or not movement is desired Being able to stay sitting up has many benefits including

bull A good functional position - ie organs in the right place

bull Well positioned for feeding

bull Better social interaction

bull Reduction of friction and shearing for which has an impact on carers time

Safer transfers for non weight bearing clients

Learn more about Ceiling Track Hoists

Find out about Molift Duo

The common way of moving non mobile clients is by hoisting The number of carers required is determined by risk assessment but mobile hoists often require a minimum of 2 carers This decreases independence and increases care packages The following provide a more client focused alternative

Maintaining the health of the carerThe Positioning Wedge supports the patient on their side and avoids the carer being in a static holding position It is used after turning when resting for personal hygiene and wound care

Non slip polyurethane keeps the wedge in position It can also replace pillows which can slide out of place for a more secure and comfortable sleeping position

Reducing risk for less mobile clientsThe MultiGlide Glove allows the carers hand to be placed easily between the patient and cushionmattress for small repositioning adjustments ie smoothing out bed sheetsclothing skin pressure tension release and fitting slings It is an important tool in the prevention of pressure sores

Why invest in a specialist mattress if its effectiveness is reduced by creases in the bed sheet

A ceiling track hoist eliminates the need for a mobile hoist This increases the amount of space for the carers to work in Also without a mobile hoist to move their risk of musculoskeletal injury is reduced

In a carenursing environment a carer can react immediately when a patient requires transferring rather than waiting for another member of staff to become available

Gantry SystemsThese are ideal for temporary solutions where a ceiling track is indicated The Molift Duo is easy to install move and dismantle

Whichever option is used a hoist can maximise the potential of the 4WayGlide

Ceiling Track HoistsSubject to a risk assessment this could encourage a client to do their own transfers increasing independence dignity and general well being

Even when a client requires assistance the number of carers can be reduced in many cases Other than the cost saving this promotes better client centred care as the client is the focus of the carers attention

How to Control Upper Body MovementThe corner straps on the 4WayGlide can be used to prevent a service user leaning off to one side if they have left or right side obliqueness perhaps as a result of a stroke

Using corner straps to control upper body movement
