Learning from each other . . .Mental health services all over the world seek for best-practice models to support and treat people with men-tal disorders, specifically for those with severe or seri-ous mental illness (SMI). Integrated Care is a worldwide trend in health care reforms and new organizational arran-gements focusing on more coordinated and inte-grated forms of care provision. Integrated care may be seen as a response to the fragmented delivery of health and social services being an acknowledged problem in many health systems. Integrated care covers a complex and compre-hensive field and there are many different approaches to and definitions of the concept.Integrated care and assertive outreach is delivered by teams or services to reach persons with severe mental illness who are not able to seek care, who avoid care or need more intensive treatment and care to prevent crisis or admission. The aim is to support the person in fulfilling their goals, coping with the illness and achie-ving recovery and a meaningful life in the community. The person and his network are the key collaborators for the services.

EAOF Conference 2017The fourth conference of the European Assertive Ou-tre-ach Foundation (EAOF) aims to support solutions by or-ganizing communication between researchers, service users, relatives and stakeholders from different coun-tries and different health care systems.
