CORNER

Psalm 74:15-17 (NRSV) 15 You cut openings for springs and torrents; you dried up ever-flowing streams. 16 Yours is the day, yours also the night; you established the luminaries and the sun. 17 You have fixed all the bounds of the earth; you made summer and win-ter. Job 38:4-7 (NRSV) 4 “Where were you when I laid the foundation of the earth? Tell me, if you have understanding. 5

Who determined its measurements—surely you know! Or who stretched the line upon it? 6 On what were its bases sunk, or who laid its corner-stone 7 when the morning stars sang together and all the heavenly beings shouted for joy? I share with you this issue not one but two passages of Scripture. We are into summer now (note the mention of summer in the Psalm). But both passages relate one of the key motifs of God – that of Creator. The Psalmist proclaims

Presidents Report Greeting to everyone, We just held our monthly meeting at Station 67, and once again the Firefighters from Colum-bus outdid themselves. We were treated to a de-licious lunch, brats, burgers, beans all the trim-mings and desserts courtesy of our friends at Co-lumbus Municipal Employees Credit Union. We had 145 members present along with ladies from CME who also brought along gifts for our mem-bers. Lt. Dan Schafer was in charge of keeping us happy this year. He along with his crew went above and beyond in their service to us. I am still trying to compile a list of the names of eve-ryone who helped serve us and will post them when complete. I did not want to leave anyone out. Thanks to President Dave Montgomery and everyone that helped out. As everyone is aware of, Health Care is the hot topic in the Nation these days. We as fire-fighters never thought that we would see such drastic changes to ours. With Gary Siniff step-ping down we are not as privileged as we once were. I checked with the PFRO prior to the May meeting and there was no meeting in April. So I have no new information to report other than the report that everyone should have received from the Police and Fire Pension Board that said they were working on it. Charlie Burkhart seems to be the only person that has any experience with a similar system, PERS. Charlie predicted these changes a few years ago and he was correct. It may not be as bad as we suspect, Charlie thinks it is working out very well for him. But we all have different health care needs and what works for one may not for another. Only time will tell. We are not the only people in the nation dealing with healthcare issues. We are also working on the upcoming Ban-quet. The date will be June 15, 2017 at Station 67. Time: Meet and Greet 5:30PM – 6:30PM. Dinner at 6:30. In an effort to make the banquet better we have changed the caterer and we now have a different menu. This year we will have as our Honoree, Mr. Larry Baker, aka. Chaplain Baker. Larry might be the hardest working re-tired guy I know. He does an awful lot for this organization, visits to our families, attending funerals, making hospital and nursing home vis-

itations, he even makes time to do a little fishing. Congratulations to Larry. As most of you know that attend our monthly meetings we are in need of filling key positions within 4 Unit, we are still looking for a replace-ment for Jerry Mason who has moved to Florida. Jerry has done a great job but it’s time for him to enjoy his retirement. We also have upcoming elections for Sgt. at Arms, Trustee, and Presi-dent. Not to mention Gary Siniff’s position. Please get involved. We have even attempted to get some of our active brothers and sisters in-volved if they are in the drop program. Also remember to pay your PRFO dues thru us, it makes things much easier to process and please let Terry Hetzer know if you have any changes, phone numbers ( home or cell ) email address or home address, so he can keep our data base updated. the glory of God in God’s creating work. God is of course the sustainer of this marvelous creation too. In the Job account, this is God speaking (questioning) to Job. I urge you to read from here to the end of the book. It is marvelous He-brew poetry and gives us a true sense of God’s grandeur and power. My point is that this summer gives you a grand opportunity to be outside in God’s won-derful handiwork. Golf, fish, hike, take a ride or picnic and you can ponder the wonders you be-hold. As you observe the “luminaries”, the “ever-flowing streams”, the seashore or the mountains you can also consider the loving God that made all of this for our enjoyment. Executive Summary:

When OP&F began sponsoring health care benefits in 1974, expenditures were $3 million. More than 40 years later in 2016, OP&F's gross health care expenses were over $213 million. The Board of Trustees has worked diligently over the years to develop a health care plan that will help keep the cost of the plan afforda-ble while also creating solutions to improve quality, increase member compliance and adherence, and reduce fraud and waste. Cost savings measures and efficiencies have helped to extend the life of the health care stabilizat;ion fµnd (HCSF). Howev-er, its current value is l ess than $908 million and, at the current spending levv'el, the fund is estimated to be depleted in less than 10 years. Background:

In 2015, the Board approved the retention of a health care consultant. After an RFP process, Gabriel, Roeder, Smith & Company (GRS) was engaged to help analyze the current plan and develop a benefit program that would be fiscally sustainable. Since then, GRS has presented modeling and analyses on several occasions, and in tandem wi th Buck\Conduent, with recommendations to preserve the HCSF as long as possible to allow for an orderly transition of Medicare and non-Medicare eligible groups to the individual marketplace. The activity is recapped below for your review:

GRS outlined OP&F's current healthcare pro-gram by comparing OP&F to peer organizations. GRS provided input on different healthcare de-livery models and alternative plan designs with-in the market and made several short and long-term solutions to reduce financial pressures and costs.

Additional analysis and identifica-tion of medical and prescription programs to add value and create savings will be conducted and pre-sented in the future.

HC Commitee April 26, 2016

GRS presented analysis and provided options for change. There was much discussion regarding potential changes to the benefit plan design such

as eliminating Med B reimbursements; eliminat-ing health care eligibility for spouses and\or dependents ; indexing of health care premium s; and the addition of new clinical programs in 2017.

Opportunities were presented to help stabilize the financial status of the plan, al-lowing more time for OP&F to explore a long-term strategy. Board wants additional discussion with GRS in May.

HC Committee May 17, 2016

GRS presented information on several potential changes to OP&F's health care plan design, eligi-bility requirement s, and the addition of new clin-ical program s for 2017. Optum and UHC pre-sented information regarding medical necessity programs.

There was much discussion regard-ing potential changes and the Board approved 2017 rates.

GRS made recommendation s to add new clinical programs for 2017; change benefit level elements, such as moving to a four-tier prescription list; and eligibility requirements to remove coverage for

any new enrollees with access to another group benefit program , and any new stepchildren de-pendents.

The Board approved 11 of 12 rec-ommendations for 2017. The Board did not approve a reduc-tion to the Medicare Part B subsi-dy currently provided.

Fall Board Retreat November I , 2016

GRS and Buck presented considerations for 201 8 and beyond that could have significant fiscal impact and be a foundation for future, longer-term strategic direction including elimination of the Medicare Part B premium , restricting eligi-bility to members only, and exiting the plan in lieu of a fixed contribution .

After significant discussion, the Board will continue to discuss OP&F's healthcare sustainability options.

Spring Board Retreat March 2, 2017

GRS and Buck presented information regarding healthcare plan and sustainability considera-tions for 2019 and beyond, which include al-tering subsidization rates to extend solvency, and exiting the current group plan and provid-ing participants with a fixed contribution for an interim period to ease the transition period.

The Board asked for recommenda-tions and motion s to be presented for consideration at the March Committee meeting.

Board must take action to keep momentum moving forward so that an implemen-tation p lan can begin to be built based on the full value of the expertise gained from the past several months.

Once this substantial decision has been reached, additional flow-through decisions will need

to be discussed and decided for 2019 and beyond. Because these decisions are so significant, ample time will be needed to refine and apply a successful implementation plan with as few unknowns and unintended consequences as possible. Decisions now will allow OP&F to begin a focused and methodical implementation and communication plan to both retirees and active members who need to make informed decisions about their retirement.

Rationale for Action:

Recommendations for 2018

As our consultants have noted, two primary facets of our plan leave OP&F so far out of market compared to peers that it compromises our efforts to secure additional state and/or federal funding sources for pension and/or healthcare. These are our premium subsidization and Part B reimbursements. Sadly, our peers have reduced or eliminated these subsidies. The Board should adopt these changes for the 2018 plan year in order to demonstrate we have done all we can to sustain the program. 1. Alter the Medicare Part B reimbursement from $104.90 to the statutory amount of $96.40. This change creates savings to the fund of $1.5M with minimal member impact of $8.50 per month. 2. Considerations for 2019 and Beyond

If the Board's decision is to exit the current group health care space, the points below must be discussed and fleshed out for further action by staff and our consultants.

o Consider for 2020 no longer offering an OP&F sponsored plan for

the Medicare eligible group and transition this population to the indi-vidual marketplace (Medicare Advantage, Medigap), which will of-fer greater choice and greater affordability to participants. Individu-als will make personalized buying decisions based on their current medical and prescription needs instead of accepting OP&F's "one size fits all" medical and prescription plans. Numerous options are available in this mature marketplace, which offers relatively low out of pocket expenses with premiums averaging around $183 a month. This change is expected to create savings to the plan of $45.6M in year one.

o Consider for 2020 no longer offering an OP&F sponsored self-insured

plan for non-Medicare eligible group and transition this population to the individual marketplace (commercial, off-exchange) . This change is expected to create savings to the plan of $30M, which in combina-tion with the Medicare group changes, is expected to extend health care solvency an additional 4.4 to 6.7 years.

Consider for 2020 replacing the percentage subsidy currently provided to a capped dollar amount stipend for medical expenses payable to retirees who did not waive OP&F's health care program. No capped dollar amount stipend will be provided to retirants who do not cur-

rently participate in the plan. Every OP&F retirant had an opportunity to enroll in OP&F's health care plan so those ineligible for or who

declined enrollment do not receive the value of subsidy or claim coverage paid by OP&F.

o Consider providing a capped dollar amount stipend to retirees for

an interim period to simplify and ease a transition period. Additional analysis would be required to determine an appropriate level as the proposed amounts were developed as a means to reach a specific

HCSF time horizon.

Consider pursuing legislation to eliminate the Medicare Part B reim-bursement in a 3-year phase-in. Elimination of this reimbursement could be part of the strategic plan and offset by the addition of a

capped dollar amount stipend for medical expenses. Phasing in the reduction will give recipients time to absorb and adjust to the

change, which would be counter-balanced by a capped dollar amount stipend in 2020. This change will generate significant savings to the plan of $1.5M year one, $1OM in year two and $1OM in year three.

Alternative Suggestions:

If the Board 's decision IS NOT to exit the current group health care space and in-stead continue the plan as long as possible, an alterative proposal is presented for discus-sion and consideration .

o Notification to the membership, both active and retired, that the HCSF is estimated to run out in 2025 if the current group health plan and subsidy levels are maintained. OP&F will retain a vendor to assist and advise members who will be encouraged to secure alternative coverage and members may begin a self-transition with the understanding OP&F coverage will end De-cember 31, 2024. No additional amounts or stipends for health care will be provided after that date.

This alternative assumes the Board has enacted the reductions to

subsidy and Med B reimbursements for plan year 2018 .

Conclusion:

The recommendations and considerations are presented to preserve the HCSF for as long as possible. Without additional funding sources, the challenges facing OP&F are too far-reaching to continue to main-tain the current group-sponsored health care plan. If a group-sponsored plan is no longer offered, new resources and tools could be introduced to help support and educate members with the individual health care options available, as well as to help members considering or reevaluating their plans for retirement. The recommendations above and the following motions are presented for your consideration and vote.
