located in the patient pendant. Pneumatic cordsetshouse the electrical switch at the end of the cord thatplugs into the wall socket of the nurse call system ratherthan in the patient pendant, thereby reducing the possi-bility of shock. The cord that runs from the patientpendant to the plug contains an air tube, andthe pendantcontains a push-button plunger, squeeze bulb, or pres-sure pad that, when pressed, transmits air pressuredown the tube and closes the electrical switch at theopposite end. Puff/suck pneumatic signal tubing is oftenprovided for incapacitated or paralyzed patients.

Other call devices include disposable pull cords,which are often used in bathrooms, and systems withswitches that allow the staff attendant to send a callresponse message, as well as the location of the call, tothe central console.

Many nurse call systems permit two-way conversa-tions by means of a handset, headset, or speaker/microphone at the nursing station and a speaker/microphone at each call station. Call stations amplifypatients’ voices; however, some patients may requirecord-attached pillow speakers to facilitate communica-tion. Many pillow speakers also provide controls fortelevisions and radios and may interrupt sound duringa call.

Corridor lights outside patient rooms and zonelights at corridor intersections are illuminated whencalls are signaled to assist the staff in locating thecalling room. The signal is relayed to the central con-sole, triggering an audible tone and a display (e.g.,numbered indicator lights; LCD, LED, or CRT screen)of the calling room number. Signals from bathroomsand showers trigger a tone and light signal (usuallypulsating) different from the calls from beds becausethey may require more immediate attention.

In addition to call prioritization, the central consoleusually displays the room number and status of thecall. The central console may also track and display thelength of time that a call has been in the system —from the time the call is initiated until a room responseis indicated. A call reminder feature alerts the atten-dant if a patient has not received attention within apredetermined amount of time, usually programmedby the hospital.

Microprocessor-based nurse call systems’ centralconsoles can be preprogrammed to hospital specifica-tions or can be user programmed, permitting staff toenter the name, room number, and medical data ofeach patient. These systems may interface with thehospital information system (HIS) and admission/dis-charge/transfer (ADT) system; they may also have anintegrated staff-locator system — staff manually

press buttons at each room entrance or may wear aninfrared transmitter badge, wristband, or similar de-vice that interfaces with an infrared locator system toidentify themselves. Infrared sensors are sometimesincorporated into the room stations or may be locatedin the ceiling or elsewhere in the structure of thehospital. They transmit identification information tothe nurse call system and are most commonly used tolocate staff, but they can also be used to locate patientsor equipment. With an infrared locator system interface, the mas-ter station can provide a constant display of stafflocation. With appropriate software, the display can bealtered to show the location of staff by level (e.g.,registered nurse, housekeeping). The system may alsohave nurse follower capabilities — the system con-stantly senses a staff member’s location and routes allcalls to the location. The locator system can also beused to automatically cancel patient calls when a staffmember of the appropriate level or one assigned to thepatient enters the patient room.

Some nurse call systems provide programmablecode numbers, words, or red indicator lights (triggeredwhen calls are signaled from priority patients or frombathrooms) next to the standard call lights on thedisplay panel; emergency calls can be deactivated onlyfrom the room where they were activated. Other cen-tral consoles with limited display capabilities can pre-sent calls in a priority sequence, irrespective of theorder of receipt. Patients can also indicate priority bysignaling for a nurse or aide using labeled call buttonsat the bedside station. Operating room personnel caninitiate emergency signals with a footswitch, button,or other device to alert the nursing station or anotherdepartment (e.g., anesthesia). Emergency rooms andintensive care units may have emergency buttons thatare used to alert all staff to cardiopulmonary and otheremergencies. Microprocessor-based nurse call systemscan automatically route code calls to the assigned codeteam members via a radio pager or wireless telephoneas soon as the code call is placed. Other nurse callsystems display the code call at selected central loca-tions (e.g., telephone operations).

Reported problemsThe most common problem with nurse call systems

is that cordsets at patient bedside stations break; theymay also fail to signal the annunciator if they becomedisconnected. Breakage is typically caused by frequentuse (especially those with television controls) and ac-cidental dropping. However, cordsets are required by

Underwriters Laboratories (UL) standards to signalthe annunciator if a disconnection occurs (althoughsystems have failed to do so on occasion).

Cordsets that generate electric arcs could be haz-ardous when used in or near oxygen-administeringequipment such as oxygen tents. Cordsets that do nothave switches in the patient’s signaling device, such aspneumatic cordsets and pull cords, can prevent thishazard.

There are several problems associated with infraredlocator systems. A caregiver’s transmitter badge maynot be detected if the signal from the transmitter isphysically blocked. Infrared systems may also havetrouble locating staff when a transmitter’s battery islow. Some badges signal low battery strength, but theycan be more expensive. The location and quantity oflocater sensors is also critical to the effective operationof a locater system.

Hospitals can interface nurse call systems withother equipment such as bed-exit detectors, physi-ologic patient monitors, ventilators, and infusionpumps. However, the nurse call system must be pre-configured to connect these devices and there may belimitations in distinguishing between each device’salarm.

Purchase considerationsIn the United States, nurse call systems are re-

quired by various codes and for accreditation by theJoint Commission on Accreditation of Healthcare Or-ganizations (JCAHO) and are often included in theplanning and design stage of new hospitals.

The U.S. Occupational Safety and Health Admini-stration (OSHA) requires that all nurse call systemsbe certified and/or approved by a nationally recognizedtesting laboratory (NRTL) such as UL. Hospitals mustbe aware of the requirements pertaining to each com-ponent before purchasing or modifying a nurse callsystem.

Some U.S. states have specified their own require-ments for nurse call systems. Hospitals should checkwith their state’s department of health for any regula-tions that may apply to their area.

Systems vary in their degree of expandability.Those having only visual/audible signal displays can,under some circumstances, be modified for voice com-munication, provided the conduits are large enough foradditional wiring. Many suppliers can custom designsystems to interface with other communications sys-tems (e.g., staff registries, pagers).

Some systems interface with ADT systems that areHealth Level 7 (HL7) compliant. HL7 is defined by theseventh (applications) layer of the International Or-ganization for Standardization-Open System Inter-connect (ISO-OSI) standard. It is an electronic datainterchange standard that addresses HIS functions,such as order entry, registration, ADT, billing, andresults reporting, and clinical information systems.

Cost containment

Before purchasing a nurse call system, hospitalsshould be aware not only of the initial cost of thesystem, but also of the cost of supporting the system.These costs include servicing the system and trainingthe hospital staff. If the system is microprocessorbased, additional costs will include wiring the nursingstations to a network and upgrading software. Withany system, the costs of site planning and installationshould be considered. If the system replaces an exist-ing nurse call system, the costs of disabling and remov-ing the old system should also be considered. Fullconsideration of these costs will give the hospital amore realistic operating cost than the initial cost of thenurse call system.

Stage of developmentMany nurse call systems have remained essentially

the same for decades, signaling with simple flashinglights and audible tones. The addition of two-way audiocommunication between the patient room and the cen-tral station is common and in some cases mandatory.

Incorporating microprocessor-based nurse call sys-tems allows advancements through interfacing withinfrared locating systems, the hospital’s privatebranch exchange (PBX), information systems, andADT systems. Direct links can also be provided to radiopagers and wireless telephones. Some systems alsohave management and quality control capabilities andcan record and report call transactions and statistics.

About the chart specificationsThe following terms are used in the chart:

Meets ANSI/UL 1069: Standard for hospital signalingand nurse call equipment (see Standards andGuidelines) in the United States.

Capacity, beds: The maximum number of patient sta-tions represented on the annunciator. Most systemscan be expanded by wiring additional annunciatorsto the system.

Self-diagnostics: A self-monitoring feature that dis-plays circuitry malfunction.

Distinct audible tones: Signals triggered at the centralconsole when a call is received. Normal signals,which are constant or pulsating tones, can be can-celed at the central console unless specified other-wise. Emergency signals, which are rapidlypulsating tones, can be canceled only at the callingroom.

Display capacity, beds: The maximum number of sig-nals that the annunciator can display at one time.

Hold/recall button: This allows the central consoleattendant to hold a call in the console while anothercall is answered by means of the handset micro-phone, and then return to the original call.

Call reminder: Alerts attendant if a patient has notreceived attention within a predetermined amountof time.

Priority sequencing: Calls are displayed according topreprogrammed priority levels.

Staff register: An integral feature or separate moduleused to register available staff.

Staff follower: Automatically routes calls to appropri-ate staff member.

Staff locator: Displays staff location; can be deter-mined either manually by pressing buttons or by aninfrared tracking system.

Pendant for O2 use: A cordset that does not have aswitch in the patient’s signaling device, such as apneumatic cordset, thereby reducing the chance forelectric shock.

ESD hardened: Protected against electrostatic dis-charge.
