y  contents, including details of the cleaning/proposed cleaning process and a brief 

description of the chemicals and hydrocarbon concentration remaining/to remain in theline;

y  brief details of any discharges as a result of the work to take the line out of use.

Disused Details 

y  associated equipment ± please provide details of any subsea facilities which form part of the pipeline (e.g. PLEM, UTA, riser anchor bases) including an indication of whether or not they are within a safety zone; 

y   proximity to other lines (including any replacement line) along with details of whether or not the pipeline is in a bundle, corridor or part of a piggyback system; 

y   previous history of interaction between any part of the pipeline, including theassociated equipment, and other users of the sea (e.g. fishing, cables, etc); 

y  any other relevant information.

y  date survey took place; 

y  brief description of results; 

y   presence and dimensions of spans and any corrective measures taken (you should note only spans that are greater than 10 metres in length and 0.8 metres in height need be reported); 

y  confirmation of depth of burial, if available. If not, please include depth of burial at installation; 

y  details of stability and integrity of pipeline; 

y  details of any remedial maintenance carried out/to be carried out.

Interim proposals for the pipeline prior to final decommissioning:  To include:

y  reason for decision to leave in situ and confirmation that this will not prejudice any final decommissioning solutions or pose an increased risk to other users of the sea;

y  the expected year for opening discussions regarding a formal Decommissioning 

Programme (please provide a date)y  any relevant information regarding the final decommissioning options.

Maps: (suitable scale on A3/A4 paper)

The attachment of a map is mandatory as part of the notification.

One map should be included which illustrates the whole line ± including the associated equipment, safety zones and proximity of the pipeline to other pipelines, installations,cables and infrastructure.

 Additional maps may be submitted if they are relevant to the notification or add clarity.
