Introduction to the Associate Degree Nursing Program

The nursing staff of Tarrant County College (TCC) would like to welcome you to the Associate Degree Nursing Program. The TCC nursing program is Texas Board of Nursing approved and accredited by the Accreditation Commission for Education in Nursing (ACEN). Successful completion of this program qualifies the graduate to apply to take the licensing examination to become a registered nurse (NCLEX-RN).

The student guidelines document designed to provide direction for students during the time that they are studying in the nursing program. Each course developed separate and more specific guidelines regarding tardiness, absences, make-ups, test taking, use of the nursing process, and criteria for safe performance. Please refer to the Course Information Document (CID) for the course enrolled in for the specific information on above topics.

The faculty has prepared this document to acquaint the student with important information, policies, and procedures. It is essential that the student read the information contained in this document. The information is subject to change and students informed of any changes in writing.

Mission: ADN Nursing

The mission of the Tarrant County College Nursing Program is to prepare professional nurses to deliver quality, safe care in a dynamic healthcare environment, through patient advocacy and collaboration with interdisciplinary teams.

Philosophy: ADN Nursing

The faculty supports the vision, values, and mission of Tarrant County College. The Department of Nursing is a leader in preparing nursing graduates who excel in meeting current community health needs. The department faculty creates and supports an academic climate that promotes excellence by preparing nursing graduates who are accountable, work collaboratively and are caring, competent members of the nursing profession.

The faculty believes human beings are unique, complex, and holistic. They have physiological, psychological, socioeconomic, cultural, ethnic, developmental, and spiritual needs. Human beings possess inherent dignity and worth with the right to participate in decisions that affect their health care.

The faculty believes health is a dynamic state of being, that exists on a wellness/illness continuum. The nurse cares for human beings at all points on the continuum in a variety of health care settings.

The faculty believes nursing is patient-centered practice within a legal-ethical and regulatory framework that adheres to the standards and conduct rules of professional practice. Nursing works in coordination and collaboration with the interdisciplinary health care team to provide care for patients of all ages and the family in the promotion, maintenance, restoration of health, or assistance at end of life.

The faculty believes nursing education teaches the values, ethics, skills and knowledge of the nursing profession and supported by natural, behavioral sciences, and the humanities. The curriculum and teaching methodologies are continuously revised using evidence based practice. The curriculum design fosters learning that is increasingly complex through course progression. The nursing faculty promotes learning by fostering systematic problem solving and clinical reasoning. Nursing education takes place in a variety of settings and in collaborations with other disciplines. Students are accountable for self-directed learning as an inherent part of becoming a lifelong learner.

The faculty believes the graduate is prepared to enter professional nursing practice as a patient centered care provider, patient safety advocate, member of the health care team, and member of the profession. The concepts of professionalism, caring approach, clinical reasoning, teaching, collaboration, communication, and safety threaded through the program. With additional experience and continuing education, the associate degree graduate can develop advanced practice skills and receive specialty certification.

Curriculum Concepts and Definitions:

Provider of Patient Centered Care Teaching A system of activities intended to produce learning that can include a diverse patient and family education system, in multifaceted, and involves promoting, protecting, and maintaining health (Kozier and Erb’s Fundamentals of Nursing 3e, 2015).

Clinical Reasoning The ability to systematically identify the problem and make a judgment based on data analysis.

Caring Approach

Member of Health Care Team Collaboration The actions of nursing and interdisciplinary teams in the management of patients, families, and/or communities.

Communication Exchange of information that is timely, clear, accurate, concise and complete in therapeutic, professional, and technological environments.

Member of the Profession Professionalism A set of attitudes and values, which includes providing quality patient, centered care, accountability to a legal and ethical system, and the commitment to maintaining competence.

Patient Safety Advocate Safety Practice that ensures protection from harm and injury to self, patient, and communities across the lifespan.

Student Learning Outcomes

Upon completion of the program, the student will be able to:

1. Provide safe, culturally competent care to patients and families across the lifespan using evidence-based practice and clinical reasoning to achieve quality outcomes.

2. Use effective communication and informatics to plan, teach, and share information with patients, families, communities, and members of the interdisciplinary health care team.

3. Collaborate with patients, families, the interdisciplinary team, and others to plan care, promote health, consider health-care costs, and health-care policies that impact patient outcomes.

Performance Requirements

In order to accomplish the objectives of the nursing program, students must be able to meet the following performance requirements:

1. Visual acuity with corrective lenses to identify cyanosis, absence of respiratory movement in patients, and to read small print on medication containers, physicians’ orders, monitors, and equipment calibrations.

2. Hearing ability with or without auditory aids to understand the normal speaking voice without viewing the speaker’s face and to hear monitor alarms, emergency signals, call bells from patients, and stethoscope sounds originating from patient’s blood vessels, heart, lungs, and abdomen.

3. Physical ability to stand for prolonged periods, perform cardiopulmonary resuscitation, lift patients, and move from room to room or maneuver in limited spaces.

4. Ability to communicate effectively in verbal and written form. Ability to speak clearly and succinctly when explaining treatment procedures, describing patient conditions, and implementing health teaching. The ability to write legibly and correctly in patient’s chart for legal documentation.

5. Manual dexterity to use sterile techniques, insert catheters, and prepare and administer medications by various routes.

6. Ability to function safely under stressful conditions.

Section II: Student Information

Code of Student Conduct and Honor Code

The Tarrant County College Nursing student is to display professional behaviors that are conducive to the practice of professional nursing at all times. Professionalism is a concept that embraces conduct, attitudes, values, and standards relating to the practice of a profession. The nursing student is to display behaviors described in the Tarrant County College Code of Conduct and the behaviors described in the TCC Nursing Division Professional Code of Conduct derived from the policies and regulations established by the following ethical and professional entities:

1. American Nurses Association (ANA) Code of Ethics2. Texas Board of Nursing Laws & Rules - Nursing Practice Act3. National Student Nurses’ Association (NSNA) Code of Ethics

TCC Nursing Division Professional Code of Conduct As members of the profession, students are to commit to maintain high ethical standards and moral character. Nursing students are accountable for their own academic and professional behaviors and the resulting consequences. Students will demonstrate self-discipline throughout all aspects of their education, including meeting academic responsibilities and exhibiting professional conduct in the classroom, clinical setting, and in the community. It is each student’s responsibility to promote scholastic honesty and professional ethics by actively participating with faculty in maintaining a quality academic environment.

Students are to guard public safety by immediately reporting to faculty any observed incident that allows incompetent, unethical, or illegal practice by another individual. Having knowledge of such behavior and failing to report it constitutes a breach of both academic and professional responsibilities.

Lack of compliance with any portion of the TCC Nursing division Professional Code of Conduct will result in written counseling and/or up to dismissal from the nursing program as determined by the nursing faculty and administration. (Nursing Students Guidelines, posted on About Our Program)

TCC Nursing Honor Code On my honor, I pledge to uphold the TCC Nursing division Professional Code of Conduct. I promise to maintain the high ethical values of the nursing profession, be accountable for my own academic and professional behaviors, and maintain scholastic honesty, professional ethics, and safeguard the public’s safety at all times. I promise that I will submit only work that I personally create or contribute to group collaborations, and I will appropriately reference any work from other sources. I will follow the highest standards of integrity and uphold the spirt of the honor code.

I understand that any breach of this code by my action can lead to disciplinary action including dismissalfrom the program.

Texas Nursing Students’ Association

Tarrant County College Nursing Students' Association is an organization composed of the college's nursing students. TCC-TNSA is a member of the Texas Nursing Students' Association and the National Student Nurses Association, which are the largest independent student health organizations in the country. The organization monitors legislative work that affects nursing education and practice at the national, state and local levels. State and local chapters promote participation in community projects to enhance learning experiences and to encourage continuing community involvement.

Membership is not required, but it is strongly encouraged. Membership serves as a lead into the professional organizations of the Texas Nurses Association and the American Nurses Association. Affiliation with the pre-professional and professional organizations enables the student to have a voice in the aims and purposes of the nursing profession.

TNSA holds monthly meetings throughout the academic year. Notices of meeting times and places are on the TNSA bulletin board in the nursing building. Any nursing or pre-nursing student is encouraged to attend the monthly meetings to learn more about the organization. Students encouraged to join the organization and to contribute to this successful chapter's work.

There are annual conventions at the state and national levels. Students may receive an excused absence from theory and/or clinical if selected as delegates to conventions. Final permission and the excused absence obtained from the appropriate instructor(s). Proof of attendance at conventions will be required.

The Organization for Associate Degree Nursing

In June 1984, the Texas Association for Advancement of Associate Degree Nursing (T-OAADN) was organized. The organization's main purpose is to advance the status of the Associate Degree Nurse in education and in practice. In June 1986, the National Organization for the Advancement of Associate Degree Nursing (N-OAADN) was established.

Subsequently, the names of the organizations have been modified to OADN (Organization for Associate Degree Nursing) www.oadn.org and TOADN (Texas Organization for Associate Degree Nursing). www.toadn.org

OADN is the leading national advocate for associate degree nursing education and practice and promotes collaboration in charting the future of health care education and delivery.

TOADN represents the ADN on various state legislative and nursing committees.

Alpha Delta Nu The Epsilon Eta Chapter of the OADN Alpha Delta Nu Nursing Honor Society established to recognize the academic excellence of students in the study of Associate Degree Nursing at Tarrant County College. The

society encourages the pursuit of advance degrees in the profession of nursing, as well as, continuing education as a life-long professional responsibility.

Students who meet the requirement are invited to join the Society after the third semester as provisional members. The student is inducted to full membership in the final semester provided they complete the required project and that they continue to meet the academic requirements as listed:

• Minimum of 3.0 GPA in all general education classes • Earned grade of B or better in each nursing class • No previous failures of any nursing class • Demonstrated conduct that reflects integrity and professionalism

Student Representatives on Faculty Committees

Student representatives are selected each semester to serve on various faculty committees. All students are encouraged to discuss current issues with the student representatives to share the students’ voice in faculty committee meetings.

The committees with student representatives include:

1. Program Effectiveness Committee facilitates program evaluation and development through implementation of the systematic evaluation plan for program evaluation and promotes use of the data collected to make program improvements.

2. Teaching Effectiveness Committee promotes teaching effectiveness through the support of technology, testing, library materials, and teaching strategies.

3. Curriculum Committee facilitates implementation and evaluation of the curriculum. The committee is composed of the Assistant Deans, Dean, and Coordinators.

4. Student and Faculty Affairs Committee supports and directs policy, development and services for faculty and students

Dean’s List

Nursing students are eligible for the dean’s list if achieved a grade point average of at least 3.5 on eight semester hours or more in one semester after admission to the nursing program. Upon achieving this GPA during a long semester (fall or spring), the student should take his/her transcript to the Vice President for Teaching and Learning Service’s secretary to have this recognition published.

Legal Requirements for Licensure

Nursing students must initiate the process for a declaratory order prior to admission to the nursing program because the process can be lengthy. The Texas Board of Nursing must clear applicants prior to entering the TCC Nursing Program.

The Texas Board of Nursing requires students to acknowledge in writing the receipt of information related to eligibility for licensure. Signature of receipt is required. Full information from the Nursing Practice Act/ Rules and Regulations provided in the first nursing course.

Texas HB 1508 mandates that students in health care professions be notified that criminal charges/activity occurring prior to or during the course of their program, unless approved by the Texas Board of Nursing, may affect their eligibility to be licensed following graduation. Texas H.B. 1508, Chapter 53, Subchapter E

The Texas Board of Nursing must determine the student applicant eligible to take the exam to schedule the NCLEX-RN or give a temporary permit. Information regarding the process to petition for a declaratory order to determine eligibility obtained from the Board of Nursing at 512-305-6889 or www.bon.state.tx.us. Registration

The nursing program assigns entering students to nursing courses in the first year. This allows for timely onboarding, which can take up to 2 months. Additionally, facilities required attestation letters sent up to 6 weeks prior to clinical start. Information about nursing registration found on the nursing web site www.tccd.edu/nursing

It is necessary for students to have up-to-date immunization records on file in Castle Branch. In order for students to attend clinical and nursing courses, the student must submit the compliance summary from Castle Branch on one week prior to the start of class:

1. Up-to-date immunizations and annual TB screening record including a current TDaP (within 10 years).

2. Negative Drug screening results. 3. No disqualifications from the Criminal Background Checks. 4. Current CPR for Healthcare Professionals – must be approved by the American Heart Association

(AHA). Online CPR not accepted. 5. Proof of Health Insurance. 6. Blue Care or TBON letter of Clearance

Special note for Maternal and Child Health

Maternal and Child Health Nursing consists of two eight-week courses: maternal-newborn and pediatric nursing. At mid-semester, students who are in maternal-newborn and pediatrics change clinical sites and instructors. See the campus Financial Aid Office regarding distribution of funds for this special semester.

Degree Plan and Graduation Information

College transcripts from other colleges must have both a transcript evaluation and degree audit in the Counseling Center or Registrar’s office prior to filing a degree plan. A degree plan must be on file in the counseling office prior to the petition for graduation. A degree plan may be filed any time after enrollment in the first nursing course.

To be considered a candidate for a degree or certificate, the student must submit an on line application for graduation (https://www.tccd.edu/academics/graduation/) before the date specified in the College Catalog. The candidate must indicate the Catalog year of his or her degree plan.

Students should contact the Counseling Center with any questions about their degree plan and/or graduation requirements.

Only one formal graduation exercise is held each year at Tarrant County College— a formal commencement at the end of the spring semester. Students completing graduation requirements during the summer or fall semesters may choose to participate in the formal commencement exercise conducted at the conclusion of the following spring semester.

Pinning

Each year there will be a Pinning ceremony in the months of December and May upon completion of the complex nursing course, graduates may participate in this formal celebration. The graduate may invite family and friends to attended. The graduate will dress in professional business attire and a white lab coat.

Scholarships and Financial Assistance

For students in need of financial aid, pamphlets and applications describing all programs of financial assistance at TCC are available in the Financial Aid Office.

Periodically, the TCC Nursing division provides names of nursing students who are eligible for scholarships or tuition assistance. Information about these opportunities will be available in the Nursing Administration office and/or will be available from the nursing faculty.

The Tarrant County College Foundation provides scholarships for students majoring in nursing. Visit Scholarships for more information.

The Texas Nursing Practice Act considers failure to repay a guaranteed student loan as unprofessional conduct, as provided in Section 570491 of the Texas Education Code. This rule reflects a requirement passed by the Legislature.

Change of Name, Address, and Phone Number

A nursing student who has a change of name, address or phone number, must first notify the registrars’

office of this change. Once the student officially changes name with the registrar, the change will be

updated with the Nursing division by emailing [email protected].

APA Style Format

TCC nursing students when writing professional papers must use APA format. All levels will use the Purdue OWL website as their guide to APA. The link is https://owl.english.purdue.edu/owl/resource/560/01/

Section III: College/Department Policies

Tarrant County College District Policies

Nursing students enjoy the benefits and rights of all students at Tarrant County College. They are also subject to the same responsibilities as other college students. These rights and responsibilities are in the Tarrant County College Student Handbook. Tarrant County College - Student Handbook

The Department of Nursing utilizes this handbook as a reference to identify disruptive classroom and clinical behavior. Disruptive behaviors are tardiness, cell phones, children, etc. Failure to adhere to guidelines or policies leads to consequences outlined the handbook directives.

Minor Children on Campus (District Student Handbook)

On occasions, TCC offers classes and activities for minor children. Minor children are not permitted to remain unattended on campus, to attend classes with their parents, or to be in the testing center either attended or unattended. For more details, please refer to https://catalog.tccd.edu/content.php?catoid=7&navoid=384, Article III, number 26.

Financial Aid Federal guidelines prohibit federal funds release until 10 days prior to the start of class. In 8-week courses, federal funds for the second 8-week course will not be available until 10 days prior to the start of the second 8-week course. Students will only receive funding for the first 8-week course at the beginning of the semester and must wait for the balance of the Federal funds until mid-semester.

Inclement Weather

Students are to attend class or practicum during inclement weather unless the College is officially closed. Official closings announced on radio and television stations, a taped message posted on each College switchboard and at www.tccd.edu. Students are expected to exercise good judgment in

determining whether or not to attend class or clinical during inclement weather. However, the student is absent unless the college is officially closed.

TCC Attendance/Absence Policies (District Student Handbook)

Regular and punctual class attendance expected at Tarrant County College. In case of absence, it is the student's responsibility to contact the instructor. Students absent on official school business are entitled to make up schoolwork missed. In all other cases, the instructor determines whether the student to make up work and decides on the time and nature of the makeup. However, the student is expressly responsible for any work missed regardless of the cause of the absence. The student must discuss such work with the instructor and should do so immediately on returning to school. Communication between the student and instructor is most important, and it is the student's responsibility to initiate such communication. If students do not appear at the prearranged time for makeup work, they forfeit their rights for further makeup of that work.

Division of Nursing Attendance Policy

The Nursing division adheres to the TCC attendance policy. In addition, the Nursing division follows the policies listed below:

Attendance in class and clinical is essential to the development of the student’s knowledge and mastery of the skills. Each course has developed guidelines that address this issue. Students are required to adhere to the guidelines stated in each course CID regarding notification of the instructor and clinical facility. Make-ups, and continuation in the course when absences and tardiness are excessive by the course guidelines, are considered within each course. In all cases, the ability of the student to meet the objectives of the course used to determine whether or not a student continues in the course.

High Risk Student Advisement

A student-faculty advising process is available to support returning, transfer or those students who indicate a need. Entry into high risk advising determined by a student's inability to complete a learning objective(s). For example:

1. Test score < 75% at any point in the semester 2. Skills Laboratory performance evaluation failure 3. Dosage Calculation Exam failure 4. Unsatisfactory clinical performance as determined by faculty 5. Coordinator recommends advising 6. Student’s 2nd admission to nursing program

The Course Coordinator determines at the beginning of the semester whether a student is high risk:

1. Faculty refers student to course coordinator

2. Students who fail to follow the advising process may be ineligible to progress in the nursing program

Tarrant County College Withdrawal Policy (District Student Handbook)

A student may withdraw from a course or from the College with a grade of “W” any time on or before the Friday of the 12th class week in the fall or spring semester, on or before the Thursday of the fourth class week in each summer term, and on or before the Friday of the 6th class week for 8 week courses in the fall or spring semesters.

To withdraw from one or more courses at or prior to the withdrawal date of a semester or term, students must complete a withdrawal form in the Registrar's Office. Until the registrar has officially withdrawn the student, the student remains on the class roll and may receive a grade of “F” for the course. It is the student's responsibility to withdraw on or before the withdrawal date. Division of Nursing Academic Progression and Dismissal Policy

A student may be dismissed from the nursing program or be unable to progress for the following reasons.

1. Failure to achieve a 2.0 (C) grade in all nursing courses 2. Excessive absences as identified by each clinical course with demonstration of inability to meet

course requirements. 3. Unsafe clinical performance or unprofessional conduct in clinical area, including but not limited

to unsafe/unprofessional clinical practice as outlined in the Criteria for Safe/Professional Performance in the Guidelines for Nursing Students at TCC.

4. Failure to comply with rules and regulations of the nursing program, Tarrant County College, or any affiliating agency. This includes behaviors that are violations of good professional character, including but not limited to honesty, accountability, trustworthiness, reliability, and integrity.

5. Failure to obtain a minimum score of 90% on the Drug Calculation Test in each course where it is given.

6. Failure to meet the required performance evaluation on medication administration by the second opportunity in the Nursing Skills Course. Failure to comply with College policy regarding incomplete coursework.

Note: If a student fails/withdraws from the Skills Course, he/she must also withdraw from the Foundation’s Theory and Clinical Courses. Likewise, if a student fails/withdraws from the Foundation Theory and Clinical Courses, the student must also withdraw from the Skills Course.

Procedure for Behavioral Dismissal from the Nursing Program

A student dismissed prior to the end of a semester follows the steps below:

1. The faculty will complete a student exit summary form that both the faculty member and student signs. The student receives a copy and a copy placed in the student’s permanent record with referral and recommendations.

2. Nursing administration may appoint a faculty committee to review the recommendation for dismissal.

3. The nursing faculty committee, if applicable, will review the recommendation and refer their findings to nursing administration.

4. The Dean and/or Assistant Dean of Nursing will meet with the student and make a decision on the dismissal.

If dismissal upheld, the student may opt to

1. Withdraw prior to the semester drop date. 2. Initiate an appeal through the department and/or college Student Grievance Procedures 3. Seek readmission into the nursing program (Refer to the Division of Nursing readmission and

transfer policies).

Clinical Safety

To protect patient safety in the clinical setting, TCC Nursing division faculty members will remove a student from a clinical setting if the student poses a risk of harm to patients, faculty, or staff. Students may be removed at any time including before beginning the clinical experience. Examples of situations that pose a risk of harm include (but are not limited to) any student who:

1. Does not meet the Performance Requirements in the Student Guidelines. 2. Does not conform to the Criteria for Safe / Professional Performance in the Student Guidelines. 3. Appears impaired by the use of chemicals (over the counter, prescription, or illegal). 4. Appears to be excessively fatigued or emotionally distressed. 5. Appears to pose a risk of transmitting a communicable disease. 6. Appears to pose a risk of violence. 7. Is unable to calculate medication doses accurately. 8. Is unprepared to provide patient care. 9. Does not demonstrate effective/understandable verbal communication with patients, families,

staff or instructors.

Procedure for Student Withdrawal from the Nursing Program Due to Safety Issues

The student must adhere to the criteria for safe/satisfactory performance and demonstrate professional conduct while in the clinical setting. The faculty will initiate the following disciplinary procedure when the student exhibits unsafe performance or unprofessional conduct.

The faculty member counsels the student and completes a Student Counseling Summary form, which becomes a part of the student’s permanent record. Documentation of the counseling session will include:

1. A description of the unsafe clinical performance/unprofessional conduct. 2. Indication by student signature that the faculty/student reviewed the criteria for safe

performance professional conduct with the student. 3. Recommendations to avoid repetition of the unsafe clinical performance/unprofessional

conduct (e.g. going to the skills lab for re-mastery of the skill/remediation, as indicated). 4. Timeline for the demonstration of safe/satisfactory performance or professional conduct. 5. Signatures of both the student and faculty.

Following the disciplinary procedure, failure to demonstrate safe/satisfactory clinical performance or professional conduct in the clinical setting, within the designated period, will result in a failing grade in the course and dismissal from the TCC nursing program.

1. Students who wish to appeal the evaluation exit summary should schedule an appointment with the course coordinator

2. Schedule an appointment with the Assistant Dean. 3. Students re-enrolling in theory sections must also enroll in the related clinical sections. To be in

compliance with the BON Rules and Regulation relating to Nursing education, Licensure and Practice, specifically BON Rule 215.9, f.2, “didactic learning experiences shall be provided either prior to or concurrent with the related clinical learning experiences.”

4. Re-entry and returning students return to the program based on a space available basis. 5. Only one re-entry allowed in the nursing program.

Student Grievance Procedure (District Student Handbook)

If a student has a grievance, the following procedure should be initiated:

1. Discuss the problem with the instructor or individual involved. 2. If student is not satisfied with decision after discussing problem with instructor or individual

involved, contact the Coordinator. 3. If student is not satisfied after discussing problem with the Coordinator, contact the Assistant

Dean and/or the Dean of the Nursing division. 4. If the grievance is not resolved at the Departmental level, the grievance processed through

normal channels beginning with the Divisional Dean followed by the Vice President for Teaching and Learning Services and the Campus President.

Please encourage students to follow these steps to ensure fairness and thoroughness during the process.

District Policy: Grade Review

Students may petition for review of a grade within thirty calendar days after the first class day of the next long semester by following- these procedures:

1. The student must inform the instructor and the department chairperson in writing of the reason for the review of grade;

2. The instructor should then examine the records and submit his/her recommendations through administrative channels to the campus Vice President for Teaching and Learning who will make the final decision regarding the grade and notify the student. If the instructor recommends not changing the grade, the decision explained in a memo.

3. If the decision is to change the grade, the instructor should submit a completed “Change of Grade Request” form. In all cases, copies of the student’s written appeal, attendance records, and grade report (with legend) must be attached. Grade review process concludes within thirty calendar days from receipt.

4. If the instructor is no longer employed, the department chairperson will then examine the instructor’s record, make a recommendation, and send the recommendation through the proper channels to campus Vice President for Teaching and Learning who will notify the of the decision.

Procedure

1. Student should initiate a meeting with the instructor. 2. If not resolved the student should schedule an appointment with the course coordinator. 3. If not resolved the student should follow the District Policy for Grade Review

Reinstatement

A student who drops or withdraws from a course reinstates with the approval of the Vice President of Academic Affairs.

The student must initiate any request for reinstatement within seven (7) calendar days of the official date of drop by personally contacting the Instructor involved.

Student Re-Entry Policy

A student may be eligible to re-enter the program one time. Exceptions to this rule may include a student who withdrew due to a serious personal illness, injury, and/or pregnancy documented by a physician. The TCC Health Clearance form is required to be completed prior to re-entry. A student’s reentry to theory and/or clinical will be coordinated by the Assistant Dean of Nursing. The student must concurrently enroll in both the theory and clinical courses after re-entering the program. If it is the second attempt at passing a course, then the co-requisite waived.

Serious safety violations or unprofessional conduct may provide grounds to be refused re-entry to the program. These will be documented in the student’s exit summary.

Re-Entry Procedure 1. A student who withdraws from or is unsuccessful in a nursing course must meet with an

Assistant Dean of Nursing prior to re-entry. Eligibility is based on available space, completing the recommendations stated in the exit summary, and/or providing a health clearance, etc.

2. A student who has been out of the nursing program for the majority of one long semester must repeat the drug screen and background check as required by the nursing division.

Criteria for Re-Entry

1. Absence of one semester or less: a. Fulfill recommendations as set forth in Exit Summary. b. Students who retake a theory course only will be required to perform a clinical skills

assessment as recommended. All students reentering the program after being out of a clinical course for the majority of the semester will successfully complete a skills assessment evaluation prior to re-entry.

c. Group 1 and urine drug screen if the student has missed the majority of the semester (has a break in enrollment).

2. Absence for two long semesters: a. Complete all requirements listed above. b. Remediate and demonstrate skills proficiency and/or audit nursing skills course. All

graded evaluations must be completed by the auditing student. 3. Absence longer than two long semesters:

a. Re-apply to the program as an entry level student. b. Program admission requirements must be met.

4. Students must complete the program in 1.5 times program length (ACEN Standards), generic students complete in 3 years, Fast track in 23 months.

Students selected for re-entry according to the following:

1st. Returning students without academic problems - medical withdrawals with medical clearance to return.

2nd. Returning students with academic problems who met faculty recommendations: Students ranked according to total points for the course at the time of withdrawal. Students considered for re-entry according to their rank and who have completed recommendations for return.

3rd. Students who have failed a dosage calculation exam. 4th. Transferring students without academic problems who are eligible for re-admission to a

previous school and have completed remediation. 5th. Transfer applicant selection determined by the ranking and prioritization of the HESI A2

Admission Test scores. 6th. Students identified as unsafe in the clinical area on the Exit Summary may be re-

admitted after meeting with the faculty/administration and completing specific recommendations. Unsafe clinical behaviors cited in Criteria for Safe/Professional Performance (Nursing Student Guidelines), and identified in the Clinical Evaluation Tool per the Criterion-Referenced Standards of Clinical Performance (Krichbaum, 1994).

Reentry Revised 2/06/2020

Readmission Policy to the Associate Degree Nursing Program Nursing students interested in readmission to the nursing program after an academic dismissal must:

1. Be separated from the nursing program for 2 semesters from the date of dismissal (the extended summer is equal to one semester).

2. Apply for admission by posted deadline for semester applying to re-enter, meeting the current admission criteria as a generic student with the exception of the HESI A2 A&P requirement.

3. Submit a letter stating the reason for previous academic dismissal and what adjustments made to ensure a successful second attempt/admission.

4. Once readmitted in the program, the student will be given the opportunity to continue as a generic student or take a series of HESI placement test (starting with the Foundations of nursing course) to determine placement in nursing program. The student will progress through placement test in course sequence until the point of previous academic dismissal or until the student does not achieve the required score of at least 850 on a particular course placement test. The student must achieve at least 850 on each course placement test attempted to be successful. The student will get only one attempt at each course placement test.

5. The student will be admitted in the program and placed in the course based on the last successful placement course test score of at least 850.

6. The student who separated from the program for more than two semesters are not eligible for the course testing placement option and must apply for readmission as a regular generic student.

7. Once readmitted, the student must complete and pass the Group I background check and drug screen.

8. The student is required to purchase a Castle Branch account to track immunizations requirement for the program. The student’s immunizations must be current for the entire academic semester admitted.

Transfer Student Admission Transfer student applicants are required to complete the following:

1. Admission to Tarrant County College (TCC). 2. Intent to graduate from TCC. 3. Letter of “Good Standing” from Director/Dean of previously attended nursing program that

demonstrates a GPA of 2.0 or higher. 4. Completion of all General Education courses with a letter grade of “C” or better. 5. Provide official transcripts with a completed transcript evaluation. 6. Completion of the HESI Admission Assessment (A2) exam meeting minimum entry requirements. 7. Required testing for clinical preparation (DFWHC Orientation, HIPAA, TCC Nursing Student

Guidelines Examination, Dosage Calculation Examination). 8. Remediation of essential Nursing Skills Course (RNSG 1105).

9. *CPR Certification (American Heart Association). 10. *Up-to-date immunizations uploaded to Castle Branch. (CPR Certification and immunizations are

required and must remain current, for each semester, throughout the entire program.) 11. Cleared Group 1 background check and drug screen. 12. Submission of TBON blue card or letter of Declaratory Order from TBON.

Guidelines for Testing

Testing Procedures

1. If a student is unable to take a test at the regularly scheduled time, it is the student’s responsibility to contact the theory instructor prior to the scheduled test. Contacting a Divisional Administrative Assistant does not constitute adequate notification.

2. If student fails to inform instructor, the TCC attendance/absence policy will be enforced. 3. Students who are tardy for a test will need to complete the test in the remaining time allotted

for the test. 4. Students will not discuss the exam. Tarrant County College District policy for academic

dishonesty enforced. 5. The exam is administered as a computer based test. In the event of computer failure, a

traditional paper and pencil exam will be administered. 6. The instructor will not discuss exam items during the testing period. 7. Students requesting reasonable accommodations must follow the Tarrant County College

District’s policy to receive certification through the campus Student Accessibility Resources. Failure to follow proper protocol will result in the student taking the exam according to the traditional testing guidelines.

During the Exam

1. All textbooks, purses, notebooks, papers, drinks, and other personal belongings (including jackets and hats), will be placed in the designated area outside of the exam area.

2. Electronic devices must not be taken into the testing area. If students bring their phone to a testing site, they will be asked to return the cell phone to their car if time allows. IF time does not allow, the student will turn the cell phone off completely and give it to the instructor. A Counseling Summary will be initiated.

3. The student will check in with the instructor to confirm attendance, receive scratch paper, calculator (only the provided calculator or the online calculator may be used), and instructions.

4. The student will enter the testing area and login as instructed. 5. There will be no talking during the exam. 6. The instructor will indicate when the exam begins. 7. The instructor will proctor the exam. 8. The student will be given 15 minutes to review the rationale for all missed exam items

immediately following the completion of the exam. (Note: If the student is taking the exam prior

to the scheduled time, the student will not be allowed to review the rationale of the missed questions at that time).

9. If a student arrives late for an exam, they forfeit time to review rationale. Make Up Testing Policy

The faculty recognizes that extenuating circumstances occur that may require a student to take a test either prior to or after a regularly scheduled test.

It is the student’s responsibility to make arrangements with the instructor as soon as possible regarding the date, time and location that the makeup exam is to be taken. The makeup test should be taken within one week of the regularly scheduled exam.

If the exam is taken prior to the regularly scheduled exam, the student will not be allowed to review the test the rationales that day. A different exam with alternative format may be given in place of the original.

Failure to adhere to this policy may result in a grade of zero for the test.

Following the Exam

1. The student will turn in the scratch paper and calculator, and leave the testing area once the exam is completed.

2. The student should schedule an appointment with the instructor for content or concept clarification.

Dosage Calculation Policy

The purpose of this policy is to assure patient safety when students administer medication in the clinical setting.

Process for the Dosage Calculation Exams 1. Students must take their 1st attempt DCE in week 4/5 of Foundations and in week 1 of all other

courses. 2. Students who are unsuccessful on the 1st DCE will be given a set of remediation practice

questions that day and must schedule a meeting with their instructor/Coordinator for a counseling summary. When the Counseling summary is completed, the instructor/Coordinator will email the Student Success Coordinator (SSC) with the students name, ID#, CS# and needed retest date. The student will take a copy of the counseling summary and meet with the SSC within 1 week to review practice exam, remediate as needed and set up a 2nd DCE date and time. Students will be required to complete their 2nd attempt DCE within 2 weeks of the date of their first attempt in the Student Success Center. The Student Success Coordinator will notify the clinical instructor and Course Coordinator of the results of the 2nd DCE via email.

3. Students who are not successful on the 2nd DCE will stay after the exam and remediate. the test. The SSC will determine the required remediation time (1-3 hours) schedule the 3rd and final DCE

within 2 weeks after the 2nd exam. This information will be documented in a CS by the SSC and communicated to by email to the instructor/Course Coordinator. A copy of the CS will be provided to the student. Failure to successfully complete the 3rd DCE attempt can result in dismissal from the nursing course or program.

4. Special note for OB/PEDI courses. Students in OB and Pedi will take the DCE on the first clinical day. The 1st DCE score will be the assigned grade. Due to the shorter length of the course, students are encouraged to retest prior to the 3rd clinical day of the course. A student who is unsuccessful on the 2nd DCE may attend clinical the 3rd week, but may not pass medication. The 3rd DCE must be completed before the 4th clinical day (midway point in the course). Students who are not successful after the 3rd DCE will not be permitted to continue in the course as they will not be able to meet the course objectives.

5. Proactive Preparation: students who are unsuccessful on any attempt of the Dosage Calculation Exam are recommended to attend the Nursing Math Task Force sessions scheduled before the beginning of the next semester. This is an attempt to assist students to prepare for and ensure first time pass on the Dosage and Calculation Exam given every semester.

Students will not be allowed to administer medications in the clinical setting until ninety percent or higher is achieved on the dosage calculation exam. If the student fails to score ninety percent or higher on the third attempt, the student will be given the option to withdraw or receive a failing grade in clinical, because the student will not be allowed to administer medications, and will be unable to meet critical course/safety objectives. A critical objective is an identified behavior that is essential to the competency development of a nurse, and must be achieved to pass each course in the nursing program.

HESI Testing Policy

HESI standardized exams are administered in every course. The course information document describes the exam point distribution and grading criteria for each course exam. The HESI Pharmacology exam is required and is administered during Adult Nursing. A student receives an Incomplete in Adult Nursing if this exam is not completed. This score does not contribute to the Adult course grade; however, the score provides a baseline that gives the student information to identify areas for improvement.

HESI Practice exams are required prior to taking HESI course exams and are considered the “admission ticket” to the exam. Each course information document describes the practice exam requirements.

If a student repeats a course, the alternate HESI version of that course exam may be administered.

Students are encouraged to remediate. If the exam score is below the 850 benchmark, the following study guidelines are recommended:

• Score 850 and greater – no prescribed time, although areas of deficit should be reviewed • 800-849 - 2 hours minimum; • 750-799 - 4 hours minimum; • 749 and less - 6 hours minimum.

Remediation requires the student create a study packet based on their content deficit areas and may remediate using any or all of the following:

(1) HESI Student study packet

(2) Adaptive Quizzing (EAQ)

(3) Self-directed review (textbooks, etc.)

Student remediation progress may be monitored by the course coordinator.

In the Complex course, the HESI exit exam is required. In addition, at the end of the Complex course, students are required to attend the Elsevier 3 day LIVE Review in preparation for the NCLEX exam.

Academic Dishonesty

Each semester students are required to sign the following statement regarding academic dishonesty: I understand that academic dishonesty is a breach of the expected behavior of a health care professional and will not be tolerated in the Tarrant County College Nursing Program. I agree that I will not share any examination question(s) or answer(s) to any individual. I also agree that I will not falsify or plagiarize any assignments or examinations, nor participate in any behavior that can be construed as misconduct. I understand that if I violate any rule, policy, or procedure of the TCC Nursing Program, I will be subject to the college disciplinary procedure.

Grading

Failure to complete all required assignments in each nursing course may result in an incomplete or failing grade for that specific nursing course. A 100% - 92% B 91.9% - 83% C 82.9% - 75% D 74.9% - 68%

F 67.9% & below

Progression

Nursing students must receive a “C” in all nursing courses to progress to the next level courses. The nursing courses build on previous learning, a grade of “C” represents the minimum level of achievement that makes it possible for the student to be successful in future nursing courses.

Computer Labs

The nursing computer lab is open to all nursing students, staff, and faculty. The staff in the Resource Center assists students accessing information. A valid TCC ID is required to enter the lab. A WEPA printing station is available on the 1st floor of TRHA.

Viewing Online Videos at Off-Campus Sites TCCD nursing students should use discretionary judgment when viewing online video content off campus or when not in the nursing division. The student should avoid viewing online videos where the general public or children may be exposed to content that may not be suitable.

Student Colleague ID’s

Student Colleague IDs will be used for all computer applications, testing, and identification purposes while the student is enrolled at TCC. Social Security IDs will only be used for the purpose of licensure related documentation and clinical placement.

Official Method of Communication

The Nursing division will use MyTcc e-mail as the official method of communication for students enrolled in the program. US Postal mail is used for certified mail.

Texas Board of Nursing Requirements

After admission into the nursing program, the student must notify the Assistant Dean within 48 hours of an infraction.* See: http://www.bon.texas.gov/discipline_and_complaints_policies_and_guidelines.asp The student must complete a declaratory order with TBON. Pending the outcome of the declaratory order, the student will be allowed to continue or be dismissed from the program.

Drug Screening/Criminal Background Check

Students entering health-related programs of study at TCCD will submit a drug screen and criminal background checks.

1. New nursing students are screened after admission to the program, but prior to registration in nursing classes.

2. Drug screening and background checks will be performed by a contract service. Any positive results will be forwarded to the Assistant Dean of Nursing.

3. The students will bear the cost of all tests, checks, and vaccinations.

4. Criminal background checks will be performed by Group I. The Department of Nursing will be notified of the results of the background checks.

5. Students who fail to pass the background check and the drug screen will be dropped from the program.

6. If a student has a break in enrollment for 6 months or more, the student will be required to take a background check and urine drug screen to remain in the nursing program.

Chemical Dependency/Abuse Policy

The Division of Nursing is required to report to TBON the names of nursing students with chemical dependency. The Nurse Practice Act requires that “a nursing educational program that has reasonable cause to suspect that the ability of nursing student to perform the services of the nursing profession would be, or would reasonably be expected to be, impaired by chemical dependency shall file with the Board a written, signed report that includes the identity of the student and any additional information the Board requires”. (Nursing Practice Act, Sec. 301.404 Duty of Nursing Education Program to Report).

Gift Policy

Students cannot offer gifts in exchange for grades or favors.

Communication Devices and Social Networking Sites

Cellular telephones must be placed on a silent mode or turned off during classroom, clinical and laboratory. Failure to follow this policy may result in immediate removal of the student from the specified setting.

Social Media Policy

“Nurses need to be aware of the potential consequences of disclosing patient-related information via social media, and mindful of employer policies, relevant state and federal laws, and professional standards regarding patient privacy and confidentiality and its application to social and electronic media.” (National Council of State Board of Nursing, 2011)

1. Maintain professional boundaries in the use of electronic media. The fact that a patient or former patient may initiate contact with the student does not permit them to engage in a personal relationship with the patient. Online contact with patients and former patients blurs the distinction between a professional and personal relationship.

2. Follow TCC policies or seek appropriate guidance regarding postings. 3. Promptly report any identified breach of confidentiality or privacy. 4. Comply with college/clinical facility policies regarding use of college/facility-owned computers,

cameras, and other electronic devices and use of personal devices in the facility. 5. Must not transmit by way of any electronic media, any patient-related information or image that

is reasonably anticipated to violate patient rights to confidentiality or privacy or to otherwise degrade or embarrass the patient.

6. Must not share, post, or otherwise disseminate any information, including images, about a patient or information gained in the nurse-patient relationship with anyone unless there is a patient care related need to disclose the information or other legal obligation to do so.

7. Must not identify patients by name or post or publish information that may lead to identification of a patient. Limiting access to postings through privacy settings is not sufficient to ensure privacy.

8. Must not take photos or videos of patients on personal devices, including mobile devices. 9. Must not make disparaging remarks about patients, fellow students, or faculty (even if the

specific patient/student, or faculty is not identified).

Section IV: Clinical Policies

Nursing students must comply with policies and procedures of the clinical setting: for example, policies related to but not limited to: the maintenance of a drug free/smoke free environment and not taking verbal or telephone orders.

CPR

Current CPR certification is required to participation in the clinical experience. Successful completion of the BLS for Health Care Provider Course from American Heart Association is required for students enrolled in the nursing program. This certificate is current for two years. Students are required to provide documentation of current CPR certification at the beginning of each semester. Documentation must include dates that will not expire during the semester.

Liability Insurance

Professional liability insurance coverage is required and maintained through a policy held by Tarrant County College.

Health Care Coverage

Students are responsible for their individual health care coverage. Expenses incurred are the responsibility of the student. Each semester, all students enrolled in Tarrant County College Nursing Program are required to have proof of medical insurance. It must be current for the entire semester.

TB Screening and Immunizations

Hospitals require students to be free of communicable disease when delivering patient care. The Dallas/Ft. Worth Hospital Council also requires certain immunizations for students in health related programs. A completed up-to-date immunization record must be on file in Castle Branch. Students are responsible for entering their own information into Castle Branch and maintaining current immunizations and records. Immunizations must be current for the entire semester. Students who are not compliant will not be able to attend clinical which jeopardizes progression in the nursing program. As an institution preparing graduate nurses, TCC must comply with all guidelines as required by clinical facilities and state agencies.

Required tests or immunization include:

1. Hepatitis A series (2 doses or positive titer) 2. Hepatitis B Series (2 dose or 3 doses or positive titer) 3. MMR (Rubeola/Measles, Mumps, Rubella/German Measles) documentation of 2 doses or

positive titers. 4. TDaP (Tetanus, Diptheria & Pertusis) Booster (one dose as adult) within the last 10 years. 5. Varicella (2 doses or positive titer) 6. Influenza/Seasonal Flu Immunization (required annually during flu season from September

through April). If a student signs a waiver to the vaccine or refuses to take the vaccine they may not be allowed to enter the clinical program and therefore cannot progress in the program.

7. Tuberculin (TB) Test (annual requirement) Must provide documentation of negative chest x-ray or completed treatment. The chest X-ray does not need to be repeated unless the student exhibits signs of tuberculosis or exposed to disease. Students must provide an annual TB questionnaire.

Health Clearance

Student may be dismissed from clinical if there is a reason to believe that a known or suspected problem or illness that may compromise patient or others. If a student has had a medical procedure or a prolonged illness (greater than 5 business days) the student must provide a completed Student Health Form to return to class or clinical.

In addition, notification of a change in health status will be initiated by the student. A copy of the Student Health Clearance Policy form must be completed by the health care provider and will be placed in the student’s file. All expenses incurred for the medical clearance will be the student’s responsibility.

Medication Supervision of Students by Faculty

Faculty determine the amount of supervision a student should have in administration of medications.

1. Students will be supervised during medication administration. 2. High Alert Drug List medications, such as insulin, heparin and titrated medications must be given

per hospital policy and must include RN signatures. These are administered under direct supervision.

Dress Code

Students must adhere to the dress code of the assigned hospital as well as to that of the TCC Nursing division. The dress code is to be followed whenever the TCC Nursing division uniform is worn, whether in clinical or on campus. The approved student uniform consists of a Teal scrub top, teal pants (cargo or drawstring/elastic waist pants), white lab coat or white scrub jacket and may be purchased at any uniform shop. A teal skirt may be worn with a minimum length to the bottom of the knee. White or nude hosiery is to be worn with the skirt. A white lab coat or white scrub jacket must be worn in place of a sweater or fleece jacket, while in uniform, when the temperature is cold. The school patch will be permanently affixed, centered two inches below the shoulder, to the left sleeve of each uniform top and lab coat. The uniform patches should be purchased from the TCC Campus Book Store. While in a clinical setting or on campus, a program approved photo ID badge must be worn above the waist in a location that is easily seen.

Students are also required to purchase from CastleBranch and wear an official nursing student photo identification badge when on campus. If badge is lost, the first replacement is free upon request through CastleBranch. A short or long sleeved white or black unadorned tee shirt may be worn under the uniform top, and will be tucked into the scrub pants. The color of the tee shirt should be consistent with the color of the shoes. Tops must be long enough so that there is no exposure of abdomen, lower back, or lower chest when sitting, standing, or bending. Approved shoes include predominately white or black made of predominately leather with closed heel and toe. Female students shall wear either white or tan hose or socks in white or black to match shoes. Male students will wear black or white socks to match shoes.

Head scarves may be worn if they are: (1) worn as an expression of the student’s faith or (2) to cover a skin/hair medical condition. Head scarves are to be white, black or teal in color; free of pattern or print; free of fringes or other appendages that could be hazardous, worn in a manner that presents no risk of the garment swinging in a hazardous way and must not cover the nose and/or mouth.

Students will wear a watch that measures seconds. No jewelry may be worn except wedding band without a raised stone and one small stud per ear for pierced ears. Necklaces and bracelets with identifying health information and fitness bracelets are allowed provided they are made of materials that can be disinfected with soap and water and not disallowed by the clinical facility.

Jewelry worn for pierced nose, lip, cheek, mouth, tongue, eyebrow, or other visible areas will not be worn while in uniform. Ear gauges must be covered/non-conspicuous. Perfume, cologne, and scented after shave lotion are unacceptable and are not to be worn. Fingernails should be clean and trimmed to the end of the finger to promote patient safety. Artificial nails, acrylic overlays, gel polish or nail polish are not permitted and nail jewelry is not to be worn. When in uniform students should bathe daily with breath and body odor controlled. Make-up should be kept to a minimum (i.e. small amount of mascara, light blush, and inconspicuous lip color). False eyelashes will not be permitted. Visible tattoos must be covered.

Hair should be clean and neatly controlled with long hair pulled back, up and off the collar, and worn away from the face to avoid contamination. Hair clips may be worn only to keep the hair in place. Hair should be a color that appears naturally for hair (no green, blue, purple, pink, yellow, orange, etc.). It will be neat and clean and not be a distraction. Men must be clean-shaven or have a neatly trimmed beard not to exceed one inch. Mustaches may not extend below upper lip.

Students are not permitted to chew gum in clinical settings or anytime the uniform is worn.

Students who are not in compliance with this policy will be asked to immediately correct the problem. If a student refuses to comply with the policy, or is unable to do so, she/he will be asked to leave clinical for the remainder of the day. The absence will constitute one unexcused absence under the course attendance policy.

When students are in the hospital for school business outside the regular clinical time, such as to choose pick patient assignments, they must be in full uniform including appropriate footwear and identification.

Student uniform may not be worn in private employment.

Confidentiality

Nursing students are required to maintain patient confidentiality and right to privacy laws. All information regarding a patient belongs to that patient, and persons not involved in their care should not have access to such information. (American Hospital Association, Patient’s Bill of Rights, 1992; Texas Nursing Practice Act, Rule 217.12, Unprofessional Conduct. The Health Insurance Portability and Accountability Act (HIPAA.) Removal from the hospital of any information with any type of patient identifying data is considered a breach of confidentiality. Each semester students are required to sign

the following statement regarding confidentiality: I will keep all patient information that I have the privilege of reviewing for the purpose of my educational experience confidential. I will not discuss it with anyone, except to the extent necessary to complete my educational requirements and to provide appropriate care to my patients. I understand that violation of a patient’s confidentiality may be grounds for a clinical failure and dismissal from the program.

All entry-level, as well as transfer students will complete the Standard Hospital Student Orientation from the DFW Hospital Council. Prior to their first clinical experience, their score must be 100%.

Use of recording equipment in the classroom for the strict purpose of taping lectures is permissible ONLY with the expressed consent of the instructor. To safeguard patient privacy, the use of recorded material is prohibited in any clinical settings.

Criteria for Safe/Professional Performance

It is necessary for the student and the nursing faculty to assure safe and professional nursing care.

Nursing Division Infection/Standard Precautions Policy

The Tarrant County College nursing program includes clinical experiences involving care of patients with communicable diseases. Students may care for individuals who may have blood borne pathogens.

Students will be expected to treat all patients with concern and dignity inherent in professional standards of care. Tarrant County College nursing faculty and students will be expected to follow the guidelines published by the Center for Disease Control and Prevention.
