MEDICAL BENEFITS

Volusia County School Board offers three medical plan options through Florida Health Care: the Point of Service (POS) Plan, the Health Maintenance Organization (HMO) Plan and the Triple Option Plan.

The POS Plan gives you the freedom to choose any provider you wish, but you will receive the highest level of benefits when you stay within the network. The HMO Plan offers low out-of-pocket costs, but requires you to choose a physician from within the network and obtain referrals for specialist care.

The Triple Option Plan gives you complete control of your out-of-pocket expenses. The amount you pay is determined by your choice of provider. Option 1 allows you to receive care from any provider within the Florida Health Care HMO network. You may also self-refer to any provider listed as a Florida Health Care Option 2 Provider. MEDICAL BENEFITS

Volusia County School Board also offers one medical plan option through Florida Blue: the Health Reimbursement Account (HRA) Plan.

The HRA Plan offers you the freedom to use either in- or out-of-network providers and does not require referrals. It also includes a Health Reimbursement Account, which is established and funded by Volusia County School Board to help you pay for your first eligible medical expenses. Volusia County School Board will contribute $600 to your HRA for the year. You can use this money towards eligible expenses such as deductibles, coinsurance, prescription drugs, dental care and vision care. You will receive an HRA debit card that you can use to pay for these expenses, or you can pay out of pocket and submit a claim form to be reimbursed from your account. Any unused account balances will roll over to the next plan year as long as the program is offered and you remain enrolled.

That’s why Volusia County School Board has partnered with Florida Health Care Plans and Florida Blue to establish a comprehensive Wellness Program designed to improve your health, well-being and productivity.

FREE GYM ACCESS

Volusia County School Board employees and their covered dependents have free access to over 70 participating gyms throughout Volusia, Flagler, St. John's, Seminole and Brevard Counties. For a list of participating gyms, visit www.wellnessatfhcp.com/vcsb.

WEIGHT WATCHERS REDUCED FEES

Weight Watchers, now known as WW, is available at reduced fees to all benefit-eligible employees. For more information, visit the Wellness Program tab on the intranet or email us at [email protected].

WORKFORCE WELLNESS CENTERS

WorkForce Wellness Centers offer employees the convenience of extended clinic hours and same-day appointments.

♦ Florida Health Care plan members can visit any of our Workforce Wellness Centers for an $8 copayment.

♦ Florida Blue plan members have the same convenience of utlizing our WorkForce Wellness Centers but at the cost of their medical benefit.

Can’t get in to see your doctor for an acute issue? Call the FHCP Central scheduling department at 1-386-676-7198 for same-day scheduling requests at one of our WorkForce Wellness Centers. Representatives are available Monday through Friday from 7 a.m. to 7 p.m.

CRITICAL ILLNESS INSURANCE

Critical Illness Insurance reduces the financial impact of a major illness, such as a heart attack, stroke or cancer. The policy pays a lump sum benefit directly to you once you or

a covered family member is diagnosed with a covered condition. You can use this benefit any way you choose — for deductibles and coinsurance, to pay expenses your family incurs to be by your side or simply to replace lost earnings from being out of work. You choose the benefit amount when you enroll.

CRITICAL ILLNESS INSURANCE HOSPITAL INDEMNITY INSURANCE

Even with medical insurance, a hospital stay can cost you thousands of dollars in deductibles and coinsurance. Hospital Indemnity Insurance pays a benefit directly to you if you or a covered family member receives inpatient hospital care. You receive a benefit for being admitted to the hospital and then for each day you’re confined. Additional benefits are paid based on the type of services you receive.

LIFE & ACCIDENT INSURANCE

BASIC TERM LIFE AND AD&D INSURANCE

Volusia County School Board provides eligible employees with Basic Term Life and Accidental Death and Dismemberment (AD&D) coverage at no cost and enrollment is automatic. The Basic Term Life benefit is equal to 1 times your annual salary. If you are seriously injured or lose your life in an accident, you will be eligible for an AD&D benefit equal to your Basic Term Life coverage.

SUPPLEMENTAL TERM LIFE AND AD&D INSURANCE

You may also purchase Supplemental Term Life and Accidental Death and Dismemberment (AD&D) coverage as a complement to the company-paid benefit. You pay the total cost of this benefit through payroll deduction.

You may choose a benefit of 1, 2 or 3 times your annual salary. The maximum benefit for Basic Life or Basic and Supplemental Life combined is $500,000. Spouse and children's coverage amounts are offered based on the employee's earnings. If you earn over $20,000, you may elect benefits of $10,000 for your spouse and $5,000 for each child. If you earn under $20,000, you may elect benefits of $5,000 for your spouse and $2,500 for each child.

The benefit amount for employee and spouse Supplemental Term Life Insurance may carry an age-based benefit reduction schedule. Refer to your policy/certificate documents for complete details.

If a husband and wife both work for the school district, they cannot cover each other, nor can they cover the same child(ren).

PERSONAL ACCIDENT INSURANCEGroup Voluntary Accidental Death & Dismemberment (AD&D) Insurance pays a lump sum benefit in the event that you or a covered family member sustains an injury that results in any of the losses listed below. Benefits are paid for specific losses caused by a covered accident in accordance with the policy's schedule of benefits.*

Benefit Amounts

You may select a benefit amount in $10,000 increments up to 10 times your annual salary. Your family members may be insured for a portion of your benefit amount. Your spouse will be covered for 50% of your benefit amount, or 40% if you have eligible children. Each of your eligible children will be insured for 15% of your benefit amount, or 10% if your spouse is eligible for coverage. You will receive detailed information on the plan and costs during your enrollment session.

* Loss must occur within 365 days of the date of the accident.

DISABILITY INSURANCE

If a disability kept you out of work and prevented you from earning a paycheck, how would you cover your bills and other household expenses? Educators' Salary Protection Insurance

can help replace a portion of your income if you become disabled as the result of a covered sickness or injury.

Volusia County School Board offers two plan options: the Gold Plan or the Platinum Plan. The difference between the two plans is in the duration of benefits, or length of time you would be paid a benefit while you are disabled. The Gold Plan is a short-term disability plan and the Platinum Plan is a long-term disability plan.

Benefit Amount

You may purchase a monthly benefit in $100 units, starting at a minimum of $200, up to two-thirds of your monthly earnings to a maximum monthly benefit of $7,500.

Elimination Period

The elimination period is the length of time of continuous disability due to a covered sickness or injury, which must be satisfied before you are eligible to receive benefits. You may choose an elimination period of 14 days, 60 days or 90 days.

Duration Of Benefits

Under the Gold Plan, your duration of benefits is based on your age when the disability occurs and whether the disability is due to an injury or sickness.

FLEXIBLE SPENDING ACCOUNTS

Volusia County School Board offers two types of Flexible Spending Accounts (FSAs): a Health Care FSA and a Dependent Day Care FSA. These accounts allow you to set aside pre-tax dollars to pay for eligible out-of-pocket health care or dependent day care expenses. Because your contributions are deducted from your paycheck pre-tax, you reduce your taxable income and save money.

How Flexible Spending Accounts Work 1. Each year during the enrollment period, you decide how much to set aside for health care and/or dependent

care expenses.

2. Your contributions are deducted from your paycheck on a pre-tax basis in equal installments throughout the calendar year.

3. The Health Care FSA includes a debit card that you can use to pay for expenses at the point of sale. You also have the option to pay for expenses out of pocket and then submit a claim to be reimbursed from your account. The Dependent Day Care FSA does not include a debit card. You must pay for expenses out of pocket and submit a claim for reimbursement.

Please note that these accounts are separate. You may participate in either or both of them, but you cannot use money from the Health Care FSA to cover expenses eligible under the Dependent Day Care FSA or vice versa.

Use-It-or-Lose-It Rule

Be sure to calculate your expenses conservatively when making your FSA elections. IRS regulations require that you forfeit any money left in your account after the claims submission deadline. The exception to this rule is for the Health Care FSA where funds up to $500 may carryover to the next plan year.

You must actively re-enroll in the FSAs each year. You are not automatically re-enrolled.

RETIREMENT PLANS

FLORIDA RETIREMENT SYSTEM

Whether retirement is a wish for the future or approaching rapidly, everyone should familiarize themselves with the Florida Retirement System (FRS). Volusia County School Board employees are automatically enrolled into the FRS. As a member of the FRS, you will automatically contribute 3% of your gross earnings to your account. Volusia County School Board will make the rest of the contribution that is set and required by the Florida legislation each year. There are two plans available:

♦ Investment Plan: A 401(k)-type investment plan designed for employees who want greater control over their retirement plan and who want flexibility in how their benefit is paid at retirement.

♦ Pension Plan: A traditional retirement pension plan designed for employees who are not comfortable with choosing investments and managing their own portfolio, and who want a guaranteed monthly retirement benefit.

403(B) PLAN

Employees may elect to participate in the 403(b) Plan by making pre-tax contributions through a salary reduction agreement. Contributions grow tax-deferred until the time of retirement when withdrawals are taxed as ordinary income. To set up an account, you'll need to select an agent from the Approved Company/Agent List and complete a Salary Reduction Form available on the Tax Sheltered Annuities 403(b) and ROTH Plans tab on the intranet under the Insurance and Employee Benefits Department page.

ROTH PLAN

A Roth Plan is also available. This plan is similar to the 403(b) Plan except that your contributions are taken on an after-tax basis. This means you will pay taxes on your contributions now, but distributions in retirement will not be subject to taxation. To set up an account, you'll need to select an agent from the Approved Company/Agent List and complete a Salary Reduction Form available on the Tax Sheltered Annuities 403(b) and ROTH Plans tab on the intranet under the Insurance and Employee Benefits Department page.

FLORIDA SCHOOL RETIREE BENEFITS CONSORTIUM

Beginning January 1, 2020, retirees' benefits will be administered through the Florida School Retiree Benefits Consortium (FSRBC). Plans available through FSRBC are retiree-focused, offering benefits that have your needs in mind. When you transition to FSRBC, these plans will take the place of the plans offered through Volusia County School Board, except for pre-65 medical plans. Pre-65 medical plans will continue to be the Volusia County School Board plans administered through FSRBC.

Annual enrollment for FSRBC benefits takes place October 15 through November 5, 2019 with benefits becoming effective January 1, 2020. Please watch for more information from FSRBC coming in September.

♦ Pre-65 Retirees: If you are enrolled in medical and dental coverage, you can continue the same plans through FSRBC. You may also enroll in Vision, Hearing Aid, Pet Insurance, Pet Pharmacy, Identity Theft Protection and IT Tech Support benefits through FSRBC.

♦ Post-65 Retirees: You will need to enroll in an FSRBC Medicare plan to replace your Volusia County School Board medical plan. If you are enrolled in dental coverage, you can continue the same plan through FSRBC. You may also enroll in Vision, Hearing Aid, Pet Insurance, Pet Pharmacy, Identity Theft Protection and IT Tech Support benefits through FSRBC.

EDUCATORS' PERSONAL LIABILITY INSURANCE

The Florida Department of Education provides Personal Liability Insurance to all full-time instructional personnel. The program provides protection from liability for monetary damages and the costs of defending actions resulting from claims arising out of occurrences in the course of instructional personnel’s professional activities. This coverage is provided at no cost and enrollment is automatic.
