REBATE RULES Qualified Independent Repair Facilities (IRFs)/Fleets are entitled to rebates indicated on this form. Rebate amount includes GST/HST/QST. Excludes auto auctions. To be eligible for the rebate, the purchase must be made from the following approved Motorcraft parts: new or remanufactured alternators, radiators, brake pads or shoes, brake drums or rotors, gas engine oil filters, gas engine air filters, gas engine fuel filters, batteries, windshield wipers, brake calipers, loaded struts or chassis components, or the following Genuine Ford parts: new or remanufactured gas engine assemblies or new or remanufactured transmission assemblies. The purchase must take place between 09/01/18 and 12/31/18 (the “Rebate Program Period”) to be eligible for the rebate. Purchase invoice and completed rebate form are required. Rebate form must be submitted by IRF/Fleet Repair Facility. Purchase invoice date and sale date must fall within Rebate Program Period. Rebate form must be completed in full, legible and submitted online or postmarked by January 31, 2019 along with supporting documentation. Product must be purchased from a Ford of Canada Dealership. Non-Ford automotive OE Dealerships are eligible if they do not share ownership (private, public, whole, part) with a Ford or Ford Lincoln Dealership. Open to legal residents of Canada only. • Limited time offers. Offers may be cancelled at any time without notice (except in Quebec).

• Ford Motor Company of Canada, Limited reserves the right to reject claims it deems to be invalid at its sole discretion.

• Dealers may NOT complete rebate forms on behalf of customers. All rebate claims must come directly from IRFs (installers) or Fleets. Rebate claims submitted by Dealers will be automatically rejected.

• Any notifications regarding your rebate status will be sent to you via USPS or email. A Rebate notification letter will provide reason(s) for an unaccepted status.
