To Our Foundation Friends:

Thanks to the generosity of over 900 donors and the dedication from volunteers across the community, 2018 was another remarkable year for Community Memorial Foundation!

For 29 years, Community Memorial Foundation has operated as the fundraising arm for Froedtert & the Medical College of Wisconsin Community Memorial Hospital. In 2018, over $900,000 in donations were received by Community Memorial Foundation. These donations have been directed to the areas of greatest need or specific departments and programs. As you will see from the giving highlights, every donation truly makes a difference.

In 2018 our signature fundraising events continued to flourish raising over $282,000 for Community Memorial Hospital. The traditions behind these events raise money to support the hospital while bringing together people from across the community who all share a passion for having exceptional care close to home. In the pages that follow, you will see highlights from these events:

• $154,000 raised at the 28th annual Spring Splendor Gala, Elegance and Espionage, to benefit the Behavioral Health Unit at Community Memorial Hospital.• $82,000 raised at the 20th annual Golf Classic benefiting CMH’s Joint and Spine Center• $32,000 raised by Wheeling for Healing for the CMH’s Cancer Care Center. • $14,000 raised at the Fashion Show & Luncheon to benefit the Breast Care Center while celebrating breast cancer survivors.

Giving HighlightsJanuary 1 to December 31, 2018

Foundation Recipients

• 17 scholarships totaling $53,000 were awarded to area students pursuing careers in health care. Six of those scholarships were awarded to current Community Memorial Hospital employees.

• 22 qualified patients needing help beyond hospital care received medications, supplies, equipment, and services thanks to the Altrusa Fund and/or the Patients First Fund in 2018.

• Thank You to our 200 Volunteers in Partnership with Community Memorial Hospital (VIP), 57 Community Outreach Health Clinic Volunteers, and 33 of Volunteens who support the hospital in many ways, including talents, treasures and valuable time.

Small Stones Wellness Center

• Community Outreach Health Clinic

provided 497primary care visits, 122 medication pick-up visits, 90 counseling visits, 23 psychiatry visits, and 13 nutrition education visits. • The Small Stones Wellness Center supported the efforts of the Cancer Care Center at Community Memorial Hospital by providing 39 wigs to patients being treated for cancer.

• Flu immunizations were provided to 112 individuals at St. Boniface Church, Menomonee Falls, Sussex, and New Berlin food pantries, the free Community Outreach Health Clinic in Menomonee Falls, and Touch Twice in Hartland, a one-day free clinic.

Highlights

Rhoda the Service Dogp

• 176 hand and foot massages were given in the infusion rooms and cancer clinic lobby.

• 3 new Sara Stedy lifts were purchased for the Rehabilitation Services to help safely and comfortably transfer patients who are unable to list and move their feet, or who struggle with balance.

• A Car Transfer Simulator was purchased for the inpatient rehabilitation services. This equipment provides up to date training and education to patients and caregivers on how to transfer in and out of a modern day vehicle after leaving the hospital.

• A summer Health Care Academy provided 24 high school juniors and seniors from Menomonee Falls, Germantown, and Sussex Hamilton High Schools with rotational experiences to learn more about a career in healthcare.

• In the last fiscal year, Rhoda the service dog visited with patients at Community Memorial Hospital for 650 hours.

Elaine feels blessed to have the opportunity to advance exceptional care at Community Memorial Hospital for twenty-two years (May, 1990-April, 2012). Giving always follows gratitude for blessings – is what her parents taught her.

Elaine began her health career as a Clinical Laboratory Scientist. After earning a graduate degree in Administrative Leadership, she taught clinically for many years before she came to CMH to manage Educational Services. After a few years, it evolved into a robust Organizational Development department. Her passions for extraordinary customer service and problem-solving enabled her to facilitate teams

across the hospital. Helping these teams to resolve issues and improve the patient experience gave her much joy!

She supported the hospital and foundation for many, many years. In 2011, Elaine became an Evergreen Society member. She established a trust at her sister’s urging, and included Community Memorial Foundation as a beneficiary.

A Lasting Impact Through Legacy Giving

In 2000, Community Memorial Foundation established a planned giving program to honor those who have included Community Memorial Foundation in their estate plans. The impact of this program includes:

• 53 families becoming Evergreen Society members• 37 commitments to leaving an estate gift in the future• $2.75 million in legacy gifts have been received from 16 families *as of December 31, 2018

Thank you to those who have made a gift of a lifetime to ensure their community hospital remains a strong, vibrant resource for generations to come. We are proud to recognize our Evergreen Society members.

1. In June 2018, a stone was unveiled near the main entrance of the hospital to honor Llew Tietz. 2. Retired staff returned to CMH to enjoy a tour of the hospital and time to relax in the Healing Garden. 3. Rainbow fur from AJ Ugent Furs modeled by Annette Wallace at the Fashion Show Luncheon in October. 4. CMH employees who received scholarships from Community Memorial Foundation. 5. Spring Splendor

Community Memorial Foundation’s 28th annual Spring Splendor Gala ~ Elegance and Espionage raised over $154,000 to benefit Behavioral Health Services at Communi-ty Memorial Hospital. Guests dressed in secret agent or cocktail attire to attend Spring Splendor on Saturday, April 21, 2018 at the Hilton Milwaukee City Center. The event featured a champagne reception, hors d’oeuvres, silent and live auctions, dinner, and exciting raffles. Spring Splendor was emceed by WISN 12 evening news anchor Ben Wagner.

Congratulations and thank you to event co-chairs Daniel and Anna Ruzinski for a wonderful evening of fundraising and fun. Golf ClassicThank you to all who supported Community Memorial Foundation’s 20th annual Golf Classic on Monday, June 18, 2018. The outing was sold out for the seventh con-secutive year, and 144 golfers joined in the fun at North Hills Country Club in Menomonee Falls. The event raised over $82,000 to benefit the Joint and Spine Center at Froedtert & the Medical College of Wisconsin Com-munity Memorial Hospital. Over its 20 year history, the Golf Classic has raised more than $1.2 million.

Fashion Show 2018Fall fashions and breast cancer were center stage at Community Memorial Foundation’s Fashion Show and Luncheon on Friday, October 19, 2018 at the Hilton Garden Inn Milwaukee Park Place. WISN 12 meteorologist Lindsey Slater served as the event emcee as 229 guests enjoyed an afternoon of silent and live auctions, raffles, fashion and more as over $15,000 was raised to benefit the Breast Center at Froedtert & the Medical College of Wisconsin Community Memorial Hospital. These proceeds are specifically used to help provide breast cancer patients with items that assist with comfort post- surgery as well as materials to help navigate treatment options and recovery.

Fashions and footwear were provided by Maurice’s of Germantown, A.J. Ugent Furs and Stan’s Footwear. All models were breast cancer survivors who have been treated in the Froedtert & the Medical College of Wisconsin Cancer Care Network. Faye Wetzel, owner of Faye’s Boutiques, mixed things up this year with a live runway demonstration on how to work wardrobe magic with items found in your closet.

Wheeling for HealingOn Saturday, August 11, 2018, nearly 1,000 bike riders, walkers and volunteers gathered for Community Memorial Foundation’s 21st annual Wheeling for Healing fundraiser raising over $32,000 for the Cancer Care Center at Froedtert & the Medical College of Wisconsin Community Memorial Hospital. Participants of all ages and abilities showed up early at Community Memorial Hospital to walk or ride four different routes. Upon returning to the hospital, participants

enjoyed live music courtesy of Stray Voltage Band, a picnic lunch sponsored in part by Cousins Subs, raffles, a strolling magician, memory rock painting, a 40’ inflatable obstacle course for kids, the Optimist Club popcorn wagon and much more.
