The keynote will address health transformation, reform, and preparing for the future influencing leadership decision-making and value-based accountable care and the role of the clinical leader. The focus will be on the requisite of all clinical leadership in reformatting clinical systems and practice, as leadership recalibrates the immediate future for triple-aim and value-driven healthcare. Leadership implications are included for the leader with regard to reflecting the leadership skills, capacity, and environment necessary to support good decisions and their fit with interdisciplinary relationships, MACRA, continuity across populations, service innovation, and value-driven clinical practice. This session will challenge existing mental models and help

participants think differently. Conversation will focus primarily on contextual changes in this new world for work having an impact on the clinical leader’s obligation to create a relevant context for reframing care delivery rather than simply focusing on reaction, action, and the usual functional responses themselves. Discussion will center on the forces around the changes executive leaders can expect over the short term in their role in a transformed healthcare environment.

Joseph Cofer, MD FACS Erlanger Health System Dr. Joseph Cofer, a founding member of the Tennessee Surgical Quality Collaborative, will present on the successes of a statewide, multi-hospital, multi-disciplinary collaborative for surgical quality improvement and the benefits of developing a culture to share outcomes and build trust among traditional competitors and disciplines. The quality of surgical care delivered among the collaborative hospitals over the last seven years has improved substantially, including reducing mortality by 34% year after year from 2009 through 2015 and avoiding $2,357,707 per 1,000 cases in the cost of complications.

10:50 a.m. “improvements and sustained change through the implementation of high reliability units”

Tammy Van Dyk, RN, MSN, CPENEast Tennessee Children’s Hospital East Tennessee Children’s Hospital has utilized principles and tools from high reliability training to further their focus on patient safety across the hospital with significant and sustained results in preventing hospital-acquired conditions. A high-reliability unit requires unit leaders and staff to accept personal responsibility for delivering high-quality and safe care. Staff incorporates reliability principles into their daily work to reduce errors, improve care collaboration, foster near miss reporting, engage families, celebrate successes, and learn from failures.

11:10 a.m. “Care Management/Care Transition”

Jutanna Fulbright, RN, BSN, ACM Morristown Hamblen Healthcare System Morristown Hamblen Healthcare System developed a comprehensive care manage-ment program to reduce readmissions and improve care transitions. A multi-faceted approach was deployed with a focus on communication with primary care providers and outreach to community post-acute services to share data, improve communication, and address potential issues.

11:30 a.m. “Nurse Residency Program”Carolyn Shipley, RN, BS Roane Medical Center The CNO implemented a nurse residency program at Roane Medical Center, a small, rural hospital to improve nurse recruitment and retention. Specific goals were to address the frequent RN turnover within the first year of employment. The program resulted in 100% retention of new nurses for the first year, and the residency program has been spread to other hospitals in their system.

11:50 a.m. “Patient Blood Management: Getting Started and Making a Difference”Anna Rains, BSN, RN-BCUniversity of Tennessee Medical Center Red blood cell transfusion has been directly linked to increased patient morbidity and mortality. University of Tennessee Medical Center instituted a program to implement a patient blood management program using the AABB guidelines. Specific goals included a 10% reduction in the annual red blood cell transfusion rate and a reduction in the 2-unit orders. Using education and feedback to providers, the transfusion rate was decreased 12.9% and non-emergent 2-unit orders decreased by 55%.

12:10 p.m. Lunch/Break 1:00 p.m. “Rapid Reliability: Using Safety Briefings to Hardwire Excellence”Michelle Smith, MSN, RN, NEA-BC Baptist Memorial Hospital Baptist Memorial Hospital implemented daily Safety Briefings in January 2016 with the CNO leading the effort and a focus on reducing CAUTIs, CLABSIs, and falls with harm. Approximately 150 individuals attend each day for the briefing, which lasts nomore than 15 minutes. The discussion focuses on any of these conditions that may have occurred within the past 24 hours, actions taken, and what will occur within the next 24 hours. 1:20 p.m. “Significant CAUTI Reduction in a Tertiary Hospital Environment”

Lisa M. Smithgall, PhD, RNC-NIC, CPNP, NEA-BC Wellmont Holston Valley Medical Center Wellmont Holston Valley’s Chief Nurse Executive will describe their comprehensive CAUTI reduction program that resulted in a 50% decrease in CAUTIs since 2015. Keys to their success included active leadership by senior administrators, competency verification for all nurses, nurse-driven protocols and continuous audits of expected practice.

1:40 p.m. “Stopping Medication Errors at the Door: An Advance Medication Reconciliation Program”

Tim Pierce, DPh, JDFort Loudon Medical Center Nationwide, inaccuracies in medication reconciliation account for as much as 50% of all medication errors. Fort Loudon’s project assessment data revealed that approximately 25% of their medication errors were due to inaccurate medication reconciliation spurring anew process and design. Since implementing the program, over 5500 errors have been prevented over 18 months.

The program has decreased readmissions and improved patient satisfaction scores.

2:35 p.m. “Parkwest Radiation Safety and Dose Reduction Program”Ryan Tipton, CRA, ARRT (R)(CT)Parkwest Medical Center The Joint Commission and CMS guidelines require medical imaging facilities to haveprograms in place to ensure that radiation use and exposure levels adhere to theAs Low as Reasonable Achievable (ALARA) principle. Parkwest Medical Center assem-bled a cross-functional team to create an innovative/comprehensive radiation safety and radiation dose reduction program. The program has significantly reduced the radiation exposure to patients and employees, by promoting/providing the resources and education necessary to create and maintain a safe environment at the facility.

2:55 p.m. “Nurse-Patient Medication Communication Impact of Teach-Back on Patient Satisfaction”Patricia Baise, DNP, RN, NEA-BCMountain States Health Alliance Research has shown that when patients and caregivers do not adequately understand medication teaching, it leads to unmet care needs, increased risk of complications, medication and dosing errors, and increased readmissions. Learn how Franklin Woods Community Hospital implemented a standardized medication communication process between the nurse and the patient in the practice setting to improve patient satisfaction through deployment of a teach-back tool.

3:15 p.m. “Improving Stroke Care: A Multidisciplinary Effort”Jennifer Henry, BSN, RN, CNRN, SCRNUniversity of Tennessee Medical Center The University of Tennessee Medical Center employed enhanced efforts to improve stroke care. The process established by the stroke team for on-going process review fosters a collaborative environment with a high degree of engagement from the multidisciplinary team. Opportunities for improvement are addressed real-time, and the protocol is refined and revised to ensure optimal treatment. Results include a decrease in door to treatment time and high compliance with defect free processes of care.
