Player Agreement

This agreement made and entered into between undersigned player party of the first party and the Atlanta Ball Association party of the

second part witness:

1. First party will become the ball player property of the Atlanta Ball Association.

2. First party agrees to play on any team of the Association to which he/she may be assigned by the Board of Directors and Officers of

said Association.

3. First party agrees to abide by the rules of the Atlanta Ball Association, and further instructions, and the orders of the manager of

hislher team.

4. First party agrees that he/she shall not play on any team other than to which he/she is assigned.

5. First party agrees to use all equipment and uniforms loaned to himlher by the Atlanta Ball Association, or is hislher team sponsor, only

in activities sanctioned by the Atlanta Ball Association, at the close ofthis regular season, for all equipment or uniforms lost or used

beyond normal wear and tear. \,

I, the parent of the undersigned player, who is a candidate to become a playing member of the Atlanta Ball Association, hereby give my

consent to his/her participation in any and all activities of the Atlanta Ball Association. I agree to assume the obligation of payment to the

Atlanta Ball Association, for equipment and uniform as stated in section 5. I do hereby release, absolve, indemnify, and hold harmless the

Atlanta Ball Association, organizers, officers, sponsors, managers, and the supervisors, any or all of them, from any claims on account of injury

received in the course of organized and supervised play.
