Welcome to WellnessPays! CHG’s WellnessPays program is in its fifth year, and we’re excited to offer you this opportunity to earn a discount on your health insurance for participating in CHG Wellness activities.

When you put in the time and energy to make healthy choices like exercising, eating well, and managing your stress, you’re happier and healthier. But did you know that you, your fellow employees and CHG also save money? That’s because everyone’s insurance premiums are lower when the plan doesn’t have to pay as much in claims. Passing that savings along to you in the form of a wellness discount is one way CHG is Putting People First and rewarding you for your commitment to your healthy lifestyle.

Our program makes it easy to choose activities that help you maintain and improve your health, whether you’re just getting started or already living a healthy lifestyle. You can earn points for things such as being physically active, getting your preventive medical and dental care, and completing health coaching.

Highlights of the 2015 program include:

• Healthy Lifestyle points: Living a healthy lifestyle is a big commitment, and as a reward, you can earn up to four of your eight elective points for having Healthy Numbers during our free annual health screenings. In addition, you can earn one more point for being tobacco- and nicotine-free.

• Marathon Health coaching points: If you’re ready to work on your health by losing weight, quitting smoking or managing a health condition like diabetes or asthma, take advantage of our free and confidential health coaching through Marathon Health. Working with a health coach can be just the boost you need to successfully improve your lifestyle and health. Since healthy choices start at home, your spouse or partner can participate in our free health coaching too!

• Rock Your Body: In the last six years, our people have lost almost 15,000 pounds through our Rock Your Body contest. Watch for some exciting changes this spring as Rock Your Body gets a major update to focus on healthy and sustainable weight loss.

• NEW! Colonoscopy point: This year, we’re adding another elective point opportunity for those who need a preventive colonoscopy.

SUMMARY

WellnessPays is your opportunity to earn a discount on CHG’s medical insurance by completing designated wellness activities. The points you earn in 2015 count toward a discount on your 2016 insurance premiums. The actual discount amount gets announced at Open Enrollment each year, but for last year, the discount was $15 per paycheck!

To earn the discount, you must complete all three required activities and earn at least eight points for elective activities. Use the chart below to track your progress, and check the WellnessPays portal on the benefits intranet page throughout the year to make sure your points are recorded.

2015 WellnessPays Detailed Point DescriptionsDetails about how to earn each point are listed below. Also, watch your emails for instructions and sign-ups for various activities. Make sure to review this brochure carefully and learn how to get started on your points early. Although many activities are available to family members, WellnessPays points are only awarded when the employee completes the activity, unless otherwise designated. Remember: To earn your WellnessPays discount, you need to complete the THREE required activities and earn at least EIGHT elective points by Oct.31, 2015.

Required ActivitiesThe following THREE activities are required for the WellnessPays discount because they are simple ways everyone can recognize potential health risks before they cause serious health problems. In addition, the combined CHG data from the biometric screenings and Health Risk Assessments give us a way to show that our wellness initiatives are helping our people become healthier overall from year to year.

Biometric Health ScreeningsBiometric health screenings are held on-site during Wellness Week, July 27-31, 2015. To earn the point, you must get your health screenings at a CHG event.

At the health screening, you will have the opportunity to earn additional points for Healthy Numbers. If needed, our screening vendor (US Wellness) will establish weight-loss progress goals for the following year’s Healthy Numbers points. Instructions and details about the health screening will be available midsummer.

• A makeup opportunity is offered, but it’s up to you to request the makeup right away if you miss the screening.

• Typically, the makeup option is available from July 1 through Aug. 15, 2015, and involves either a voucher for a local lab or a form your physician can submit. Because US Wellness establishes progress goals at the

on-site screening event, we highly recommend getting your screenings on-site.

• Outside health screenings are not an acceptable substitute because we need consistent screening methods to accurately measure improvement over the years.

• We will automatically award your point based on a completion list from US Wellness. No personal results or information will be provided to CHG, and any Healthy Numbers points you earn will be combined into a total score so your actual screening values are kept confidential.

Health Risk Assessment (HRA)CHG partners with Marathon Health to offer an annual online Health Risk Assessment (HRA). Depending on when you started

with CHG, you can earn this point for completing either the initial Health History and Risk Assessment (HHRA) or the HRA Annual Update. In the summer, we will open the Health Risk Assessment portal and send out instructions via email. We typically leave this portal open from July 1 through Sept. 30, 2015, but there won’t be a makeup opportunity after the closing date.

• Marathon Health, our on-site health partner, will give us a completion list for the Health Risk Assessment so we can record your point. However, no personal health information will be provided to CHG.

• Please note: The Health Risk Assessment available through your Cigna or SelectHealth insurance does NOT count towards the WellnessPays program. Also, the written questionnaire you typically fill out with a Marathon Health coach is called a Medical Health Questionnaire (MHQ) and it does not count for the HRA point. If you have questions about whether you completed the right HRA, contact benefits at 800.811.1796.

Annual Physical or Well-woman ExamBecause preventive medical care is key to early detection and treatment of medical conditions, getting your annual physical or well-woman exam is a required point.

• To earn the point, you must get your exam between Nov. 1, 2014, and Oct. 31, 2015. This gives us time to calculate the discount at Open Enrollment.

• If you have CHG’s medical insurance, we will award your point based on a list from the insurance carriers of who completed an exam. (No personal health information will be shared with CHG.)

• If you don’t have CHG’s medical insurance, or if your visit is billed differently and doesn’t show up on our list, you may show us an Explanation of Benefits (EOB) showing a zero copay for a preventive exam or a note from your doctor verifying that you received your exam. Please don’t send lab results, procedure details, statements of health or other documents that contain your personal health information. The written note from your doctor should state, “On [date], I saw [your name] for a [physical or well-woman] that covered all the preventive care appropriate for [his/her] age and gender.”

The Wellness Week health screenings take place at the end of July, and they are offered on-site in all offices at no charge to employees.

• Your 2014 screening established a baseline so you know which points are already in the optimal range and where you can improve. Your 2015 screenings are the ones that count for this year’s Healthy Numbers points.

• To protect your privacy, health screenings are conducted by an outside vendor, US Wellness, who will convert your 2015 results into a Healthy Numbers score of 0–4 that we will add to your elective points. (Your actual results for each screening will not be shared with CHG.)

• These four screenings identify risk factors for metabolic syndrome (i.e., diabetes, heart disease, hypertension, etc.). We chose waist circumference because it’s a better predictor of health than BMI; it measures the abdominal fat around your organs, which is more detrimental than the subcutaneous fat under your skin.

Elective PointsElective points are divided into categories to help you figure out which point opportunities are best for you. WellnessPays is about rewarding you for what you’re already doing well (Healthy Lifestyle), the things you do to take care of your body (Preventive Care) and the areas you’re focusing on improving (Marathon Health Coaching and CHG Wellness Activities). You can earn your EIGHT elective points from any of the categories below.

Healthy NumbersCHG is excited to offer this elective point option to reward you for the hard work and time it takes to keep yourself healthy and active. You can earn up to four points for Healthy Numbers at the 2015 Wellness Week biometric health screenings. That’s one point each for keeping your blood pressure, glucose, cholesterol and waist circumference measurements within the optimal range below:

At 2015 health screenings, achieve the weight goal* set at 2014 health screenings.

* The weight goal is a 20% reduction in the difference between your actual weight and the weight that makes your BMI 25. For example, at the 2014 health screening a 6’2” man weighs 250 pounds (BMI of 32: Obese). To have a normal BMI of 25, he would have to weigh 195 pounds, a difference of 55 pounds. His weight goal is to lose 20% of that difference by the 2015 health screenings, or 11 pounds in one year.

Healthy Numbers points are meant to reward you for being healthy, but there are a lot of other ways to earn the discount if you don’t earn all four points. If you want to maintain or improve these results, take advantage of free health coaching or wellness activities. These activities are worth points this year, and the resulting health improvements may help you earn more Healthy Numbers points next year. Remember, you won’t know your Healthy Numbers score until mid-August, so make sure to complete enough elective activities in case you don’t earn all four points.

Nicotine- and Tobacco-freeOne of the best things you can do for your health is to avoid smoking and using other tobacco and nicotine products. For making this healthy

choice, you’ll earn one WellnessPays point! Being nicotine- and tobacco-free means not using cigarettes, cigars, pipe tobacco, hookahs, e-cigarettes, chewing tobacco, snuff, nicotine patches and gum, or other smoke or smokeless tobacco or herbal products.

• During Wellness Week, we’ll send out a short survey asking if you are nicotine- and tobacco-free.

• If you’re not nicotine-free right now — especially if you’re using nicotine patches or gum to help you quit — you can earn two points for working with a Marathon Health coach instead. Check out tobacco cessation coaching under the Marathon Health coaching section for more information.

• Please note: You can earn either the nicotine- and tobacco-free point or the tobacco cessation coaching points each year, but you can’t earn both in the same year.

Preventive Care PointsDental Cleaning and Exam Research has found that oral hygiene can play a role in seemingly unrelated health issues, like heart disease. Your CHG dental insurance covers two free dental exams

and cleanings each year, but they have to be six months apart. We’ll give you one WellnessPays point for getting your first cleaning between Jan. 1 and June 30, 2015.

• If you have CHG’s dental insurance, we’ll know you earned the point because your name will be on a completion list we get from MetLife. (No personal health information will be shared with CHG.)

• If you don’t have CHG’s dental insurance, you can show us an Explanation of Benefits (EOB) or bill to earn the point.

• Getting one cleaning between Jan. 1 and June 30, 2015, means you can take full advantage of your benefit and get your second free cleaning later in the year.

Fall Flu ShotGetting your annual flu shot each fall is worth one WellnessPays point. CHG offers free flu shots on-site in each office in October. You can also get one through

your own provider or local pharmacy, which should be covered 100 percent by your insurance.

• To earn this point, sign the attendance sheet at the CHG flu shot clinic, or show us a receipt by Oct. 31, 2015, showing you got your flu shot somewhere else.

MammogramMammograms aren’t exactly the most fun thing in the world, but they could save your life! Getting your mammogram between Nov. 1, 2014, and Oct. 31, 2015, is worth one WellnessPays point.

• If you have CHG’s medical insurance, we’ll know you earned the point because your name will be on a completion list we get from Cigna and SelectHealth. (No personal health information will be shared with CHG.)

• If you don’t have CHG’s medical insurance, you can show us an EOB or a note from your provider confirming the date you had a mammogram. Please don’t send lab results, radiology reports or other documents that contain your health information. We just need to know that you were there and the service was completed.

• As long as you are over 40, or if you are under 40 and your doctor requests a screening mammogram because you have a family history, insurance will pay for the full cost of a preventive mammogram once a year. However, if a preventive mammogram finds something suspicious, it gets billed as a diagnostic mammogram and you may be responsible for a portion of the charges.

*NEW* ColonoscopyIf you’re over 50, a colonoscopy is one of the best things you can do for yourself to detect colon cancer early when it is still treatable. Getting your colonoscopy between Nov. 1, 2014, and Oct. 31, 2015, is worth one WellnessPays point.

• If you have CHG’s medical insurance, we’ll know you earned the point because your name will be on a completion list we get from Cigna and SelectHealth. (No personal health information will be shared with CHG.)

• If you don’t have CHG’s medical insurance, you can show us an EOB or a note from your provider confirming the date you had a colonoscopy. Please don’t send lab results, radiology reports or other documents that

contain your health information. We just need to know that you were there and the service was completed.

• If you are over 50, insurance will pay for the full cost of a preventive colonoscopy once every five years. However, just like with a mammogram, if a preventive colonoscopy finds something suspicious, it gets billed as a diagnostic colonoscopy and you may be responsible for a portion of the charges.

CHG Healthy Pregnancy ProgramIf your family is expecting a baby, the benefits team will help you with your maternity or paternity leave and get you started on the Healthy Pregnancy program.

Completing this program is worth two points toward the WellnessPays discount plus up to $300 when the mother completes the prenatal section in her first trimester.

• The expecting mother should start the CHG Healthy Pregnancy program as soon as she learns she is pregnant. Points are awarded in the same calendar year as the due date, when the cash incentive is paid.

• Fathers and domestic partners can take advantage of this program, too! Completion of the Healthy Pregnancy program is worth two WellnessPays points, regardless of whether the pregnant mother is the employee or the spouse/partner of the employee. However, if both parents work for CHG, the WellnessPays points and the incentive are only awarded to one employee.

Marathon Health Coaching Points

Comprehensive Health ReviewMarathon Health, our on-site health partner, offers Comprehensive Health Reviews (CHR) to all employees. A CHR is a great place to start if you want to work on

your health. You will meet with one of our Marathon Health coaches to review your Health Risk Assessment, biometric screening results and to discuss your health in general. Once the CHR is complete, you can schedule health coaching sessions to improve your health and manage specific risk factors or medical conditions.

• Schedule your review online at www.marathon-health.com/myphr. If you need your username and password to log in, call Marathon Health at 888.490.6077.

• To earn this point, you must complete a Comprehensive Health Review with a Marathon Health provider between Nov. 1, 2014, and Oct. 31, 2015. We will automatically award your point based on a completion list from Marathon Health. No personal health information will be released, and you do not need to disclose any results to earn your points.

• Comprehensive Health Reviews are available free of charge to all employees, regardless of whether you have CHG’s health insurance. Spouses and partners are welcome to complete a review, but WellnessPays points are only offered to employees who complete a CHR.

Marathon Health Coaching Let’s face it, life can be stressful and it’s hard to make health or lifestyle changes without support and guidance. Completing health coaching can help

you achieve your health goals, and it’s also worth two WellnessPays points.

Marathon Health offers free health coaching for a variety of personal issues or concerns, including weight loss, nutrition, stress management, low back pain and managing chronic conditions such as diabetes, heart disease and high blood

pressure. To get started, schedule a coaching appointment online at www.marathon-health.com/myphr. If you need your username and password to log in, call Marathon Health at 888.490.6077.

• You can start health coaching anytime, but it can take 8–12 weeks to complete and needs to be finished by Oct. 31, 2015, to count towards WellnessPays.

• Your spouse or partner can enroll in health coaching free of charge, but the WellnessPays points are only awarded when the employee completes the program.

• Coaching for smoking and tobacco cessation is a separate point opportunity.

CHG Tobacco Cessation Coaching — CHG Employees If you’re trying to quit smoking, this program is for you. CHG’s health insurance plans cover smoking cessation

drugs, but sometimes quitting requires a little extra help. CHG’s tobacco cessation coaching can help you quit, and completing it is worth two WellnessPays points. Because we think quitting is so important to your health, we’ll also pay you $100 when you finish coaching. Plus, completing tobacco cessation coaching sets you up to earn the nicotine-free point for next year! Marathon Health offers free in-person and telephonic health coaching to help you quit smoking. Tobacco Cessation Coaching — Spouses/PartnersWhen spouses and partners smoke, it affects the health of everyone in the family. If your spouse or partner is trying to quit, they can earn you one WellnessPays point by completing free

tobacco cessation coaching through Marathon Health.

• To sign up, have your spouse or partner schedule a coaching appointment online at www.marathon-health.com/myphr. If they need a username and password to log in, have them call Marathon Health at 888.490.6077.

CHG Wellness ActivitiesRock Your Body Weight Loss Challenge Completing our annual spring Rock Your Body challenge is worth one point towards your WellnessPays discount. For 2015, Rock Your Body is undergoing a major transformation to promote healthy

and sustainable weight loss for people who want to lose weight. If you’re working on a weight-loss goal from the Wellness Week health screening, the new and improved Rock Your Body challenge and maintenance contest can help you achieve that goal and earn another Healthy Numbers point.

• The Iron Man/Iron Woman contest will no longer be a part of Rock Your Body. Instead, it will count as a Physical Activity Challenge.

• Watch for more details and sign-up instructions in early 2015.

Rock Your Body Maintenance Contest Once Rock Your Body ends, it’s easy to let go of all of the healthy choices and regain the weight you worked so hard to lose. The Rock Your Body maintenance contest is a way to encourage you to

stick to those healthy habits. By maintaining your weight between the end of Rock Your Body and the Wellness Week health screenings, you’ll earn one WellnessPays point. This point is all about weight maintenance, which means weight loss is not required.

• To enter, you have to weigh out at the end of Rock Your Body and weigh in during the Wellness Week health screenings. If your screening weight is no more than 1 percent higher than your Rock Your Body ending weight, you will receive one WellnessPays point.

• If you didn’t weigh in for Rock Your Body in March, you can still sign up for this point by doing an official weigh out at the end of Rock Your Body in May.

Physical Activity Challenges Being active has numerous benefits to your physical and emotional health, and adults should aim to get at least 30 minutes of exercise five days a week. To encourage you to be more active, CHG offers Physical Activity Challenges

throughout the year, worth one point each.

• Last year, CHG held its first Physical Activity Challenges — the winter and summer Step Up walking contests with goals to walk a certain amount of steps during the challenge. Both Step Up contests were extremely popular. In fact, CHG employees collectively walked over 130 million steps in these two challenges — that’s more than 52,000 miles! Look for more Step Up challenges in 2015.

• New for 2015, the Iron Man/Iron Woman challenge will be a stand-alone Physical Activity Challenge not connected to Rock Your Body. In this challenge, you can set a stretch goal to complete an organized race event and earn one WellnessPays point for

achieving your goal. Unlike prior years, the Iron Man/Iron Woman goals are not constrained by the Rock Your Body due dates. However, you must define your goal in advance of your race, and your race can take place any time between March 1 and Oct. 31, with all follow-up due by Oct. 31. Stay tuned for instructions to sign up for an Iron Man/Iron Woman goal.

• You can earn a maximum of two points for completing Physical Activity Challenges.

Financial Wellness Events Financial stress has a direct effect on emotional and physical health, and CHG offers educational activities to help our people be more financially comfortable.

In the past, events have included 401(k) Day, Retirewise courses and a financial basics series by local nonprofit credit counseling agencies. 2015 WellnessPays DefinitionsThe WellnessPays program is an incentive to complete designated wellness activities that will improve your overall well-being, whether it’s physical, emotional or financial wellness. Although we try to keep our program simple, we need to share important definitions, clarifications and details about how WellnessPays works. If you have questions that aren’t answered here, contact Jennifer Homel in the CHG benefits department at ext. 3347.

Terminology• Eligible: This means an employee can earn points

toward the WellnessPays discount. All employees who are eligible for CHG’s health insurance in 2015 are eligible to earn WellnessPays points. New employees hired in 2015 are eligible to earn points and can earn the discount. New employees who join CHG midyear may get credit for physicals, well-woman exams, dental cleanings, mammograms and colonoscopies that took place before their hire date, as long as they fall within the program deadline for that activity. However, no special makeup opportunities will be offered for employees who are hired too late in the year to complete all the required or elective activities.

• Qualify: This means an employee has met the point requirement for the year. Eligible employees who aren’t currently enrolled in our insurance can still qualify for the 2015 discount by earning enough points. However, the discount is only paid when employees enroll in our insurance for 2016.

WellnessPays PointsThe WellnessPays program awards points for completing designated activities. Points are available for participation-based activities as well as outcome-based activities, but the outcome-based points are not required to earn the discount.

• Points are available between Nov. 1, 2014, and Oct. 31, 2015, and employees must complete three required activities and earn at least eight elective points to earn the discount.

• The three required activities are structured to accommodate employees who may miss the on-site event. The Health Risk Assessment will be available online for at least two months, and the health screening will have a makeup opportunity. However, it is up to the employee to contact the benefits team in a timely manner if a makeup screening is needed. Because there are many elective point options, elective activities will not have a makeup opportunity. The required physical or well-woman exam can be completed anytime between Nov. 1, 2014, and Oct. 31, 2015.

• Some activities, like Rock Your Body, are worth prizes and incentives in addition to WellnessPays points. The rules for earning prizes are usually different from the requirements to earn WellnessPays points.

• Educational events may vary by office due to the size of the office and the resources available in each state.

• Points will be tracked manually by the benefits team. Log in to your WellnessPays points portal on the benefits intranet page to check your points total throughout the year.

• Many activities are available to spouses and partners, but the WellnessPays points are only awarded for the employee’s completion of the activity, unless specifically designated otherwise.

WellnessPays DiscountIf you meet the three-plus-eight point requirement in 2015, you’ll earn a discount on your 2016 health insurance premiums. The discount applies to the entire year of 2016. The discount is all-or-nothing; we do not award partial discounts for partial points.

The 2014/2015 discount was $15 per paycheck. The discount on the 2016 medical premiums will be at least that much and possibly higher, depending on how our premiums change at Open Enrollment. We will notify you of the exact discount amount at Open Enrollment in fall 2015.

CHG pays the discount by reducing the medical insurance deduction each paycheck for the plan and tier the employee elects at Open Enrollment. If an employee changes plans midyear, the discount is applied to the new election.

• The discount is only applied to medical insurance. Dental and vision are not included in the discount, and we do not “cash out” the discount.

• Employees who aren’t enrolled in CHG’s medical insurance should still earn the points and qualify for the discount. If they have a change-of-life event that enables them to enroll in the plan midyear, they will be given the discounted rate if they earned the points the prior year.

General Appeals and AlternativesBecause our program already includes both participation-based activities and outcome-based activities, we believe it has sufficient options to give everyone the opportunity to qualify for the discount, regardless of health status. However, if an employee feels that a special circumstance (like a medical condition or FMLA) interferes with their ability to earn the discount, we will be happy to work with them individually to find a reasonable alternative that fits the overall goal of the program.

• Employees with special situations should reach out to us proactively so we have time to work out an alternate option; once an activity has ended, we may not be able to provide an alternative.

• A wellness appeal point is available on a case-by-case basis starting in September 2015 to accommodate health factors that can make participating in required activities or reaching a Healthy Numbers goal medically unadvisable.

Healthy Lifestyle Point Appeals The biometric health screenings are the source for the Healthy Numbers outcome-based points. After the screenings end, each participant will receive a letter confirming which Healthy Numbers points were earned and documenting the alternate weight-loss progress goal for the following year. Participants do have the opportunity to appeal screening results. If your doctor feels that reaching a Healthy Numbers target is not reasonable due to a health factor or because it is medically unadvisable, you and your doctor are encouraged to work with US Wellness to define an alternate standard or goal. If an appeal is approved by the deadline, US Wellness will notify CHG of the new score, without revealing which additional points were earned, and CHG will adjust the point totals accordingly. • Please note: Marathon Health clinicians are not able to approve or request alternatives and appeals to the Healthy Numbers program as they are general nurse practitioners. If someone has a special circumstance that warrants an alternative standard, he or she should already be working with a specialist for a higher level of medical care. In addition, accepting appeals and waivers from Marathon Health would create a potential conflict of interest for CHG.

The nicotine- and tobacco-free point is a self-reported point available to all employees. The alternative to this point is a tobacco cessation coaching program that is actually worth two points, so employees have an incentive to work on being tobacco-free. Because our core values include Integrity and Ethics, CHG does not screen employees for nicotine. Employees cannot earn the nicotine-free point in the same year as the tobacco cessation points.

Healthy Lifestyle points are not required to earn the WellnessPays discount. They are an elective option to earn up to five of the eight elective points for having Healthy Numbers and being nicotine-free. There are over 20 other existing elective point opportunities available as built-in alternatives to the Healthy Lifestyle points, all of which are offered at no cost to employees and designed to help employees improve their health and well-being.

DeadlinesEach activity has a specific deadline to earn the point. We make our programs as flexible as possible, but we do have firm deadlines in order to administer the discount. If an employee feels he or she can’t meet a specific deadline, it’s his or her responsibility to reach out to the benefits team in advance to find an alternative.

• Oct. 31, 2015, is the deadline for most events, allowing us enough time to collect the completion reports from our vendors, finalize the points from the whole year and set up the discount at Open Enrollment.

• Some deadlines are specific to an activity. For example, the dental cleaning has to take place between Jan. 1 and June 30, 2015. CHG’s dental insurance pays for two cleanings per year, but they have to be six months apart. If you don’t get your first cleaning by June 30, you won’t be able to get a second one that year and take full advantage of your preventive care.

PrivacyThe privacy and confidentiality of employees’ personal health information are extremely important to us as we track points and administer the discount program. In the course of administering the program, we do have to receive participation reports from the insurance companies to document completion of certain points. However, they will only give us a simple list showing which employees completed a designated activity. They will not share specific health details, procedures, test results or any other personal health information.
