ECR 2013 (European Congress of Radiology) was held in Vienna from March 7-11, 2013. As expected, it is a hugely successful event with 20,098 delegates from 101 countries, plus 244 exhibitors. 13 participants were from Malaysia. As CoR President, I was invited to attend the ESR Executive Meeting and ESR General Assembly.
The lecture topics were diverse and the symposiums were stimulating. There were at least 10 concurrent sessions at any one time. The symposium most interesting to me was on ‘Making Radiologists Visible’.
Radiology as a profession has come a long way since the discovery of x-rays in 1895. In the early days, most radiologists were not medical doctors. They could be technicians, physicists or anatomists. They would take the radiographs and interpret them themselves. Overtime, with advancement in medicine and sophistication of new imaging technology, only medical doctors would become radiologists and images are taken by radiographers or technologists.
For many years, the image of radiologist is that of a doctors’ doctor. The doctor would request (some say order) for an x-ray or scans, the radiographer will perform them and once images are available, the radiologist will sit in his darken room interpreting the images and preparing the report. Whether his report makes any clinical sense, is least of his priority.
Over time, communication between radiologists and referring physicians improve and radiologists started to be involved (although reluctantly) in clinical meetings or conferences. The value of radiologists’ opinion and reports become more important. Radiologists started to feel they are now better appreciated and on equal par with their physician colleagues.
Interestingly and unfortunately, about 50% of patients do not know that radiologists are medical doctors themselves. Most patients are not able to appreciate the value of radiologists in the management of patients.
The fault lies in the radiologists themselves who prefer to stay aloft and refuse to talk to patients or referring clinicians. By abandoning the patients, they damage their own profession, disadvantage the patients and indirectly may lead to increase in healthcare cost. Getting radiologists closer to patients makes radiologists more relevant and reduce healthcare spending.
All that should change.
A balanced role between patients, radiologists and referring physicians ought to be established. The interaction is no doubt complex and delicate. With patients becoming more aware of their medical conditions, they demand more from their doctors and referring physicians is unlikely to be able to support that. Imaging technology has become more complex and interpreting the x-ray reports to patients is no easy task. Now is the time for radiologists to start talking to patients directly.
However, a few questions arose. Are radiologists trained to talk to patients? Imaging is just one part of the jigsaw - others being medical history, physical findings, blood tests and many others. Is knowing one part of the puzzle sufficient to allow radiologists to have an effective communication with the patients?
Equally important, radiologists should continue to talk to their clinicians. With the right clinical perspective, they will be able to prepare a more meaningful report. If the clinicians find the radiologists’ report relevant, the value of imaging becomes more important.
From the perspective of medical ethics, radiologists are physicians. They have the same professional obligation as any physicians to patients. Patients’ well-being is to be respected, diagnostic or therapeutic options are optimised and harm are minimised.
The referring physicians also need to respect radiologists as equal colleagues. They should be open to discuss cases freely with radiologists either ad hocs or during weekly clinical conferences. If imaging is required, they should request for it rather than order them. Radiologists should take active parts in all clinical conferences.
The time for change has come.

Tips for Visitors To Malaysia
KLIA arrivalAfter clearing Customs, suggest you head for the registered airport limo counter or the KLIA Express train counter to purchase a coupon for your ride to downtown KL. The train ride will take about 28 mins while the limo ride will be about 45 mins depending on traffic flow. There are 2 limo rates – economy and premier. The express train ride costs RM70 return and stops at KL Sentral. From there, please take a taxi to the hotel.
Arrival by railTake a taxi from the train station to your hotel. Taxi fare may vary but insist on the meter.
Arrival by roadIf you are arriving by express bus, take a taxi from the bus station to the hotel. If you are driving into Kuala Lumpur, try to arrive during off-peak hours. Otherwise, you will be caught in traffic jams from most entry points into the city. The peak hours are usually from 7.30-9.30am in the mornings and 4.30-7.30pm in the evenings. Downpours will lead to premature and extended jams.
