All conference registrants are entered for a chance to win. The grand prize will be awarded at the

conference closing lunch. Must be present to win!

CLE ACCREDITATION Application will be made to Illinois and Kansas for MCLE credit. Estimated 18 MCLE hours, including 8 ethics hours.

GOLF CANCELLATION POLICY Refunds will not be given on cancellations made after May 30, 2013. You may transfer your reservation to a colleague.

CONFERENCE REFUND POLICY• A full refund will be given if you cancel by 5:00 p.m. on

May 23, 2013. • Those canceling after 5:00 p.m., May 23, 2013 through

June 10, 2013 at 5:00 p.m. will receive a refund of the registration fee minus a $65.00 administrative fee. A link to the course materials will be sent to you by email. Course materials on CD-ROM or USB drive will be mailed to you after the conference. If you prefer printed course materials, you may order them individually for $75.00.

• NO REFUND will be given for cancellations made after 5:00 p.m. on Monday, June 10, 2013; the course materials selected on your registration will be mailed to you in consideration of tuition paid. Only on-site registration will be available after 12:00 p.m. (noon) on June 10, 2013.

• All cancellations must be in writing. If you cannot attend, a colleague may attend in your place if s/he could have registered at the same price.
