WELCOME TO THE UNT HEALTH SCIENCE CENTER!The University of North Texas Health Science Center (UNTHSC) is a 33-acre graduate university located in Fort Worth’s Cultural District. The campus includes the Texas College of Osteopathic Medicine (TCOM), the Graduate School of Biomedical Sciences (GSBS), the School of Public Health (SPH), the School of Health Professions, which includes the departments of Physician Assistant Studies (PAS) and Physical Therapy (DPT) and a UNT System College of Pharmacy (SCP) that will welcome its inaugural class of students to our campus in fall 2013.

HISTORYOne of the nation’s distinguished academic health science centers, dedicated to education, research, patient care and service, the University of North Texas Health Science Center began when the Texas College of Osteopathic Medicine accepted its first students in 1970. With the establishment of GSBS in 1993, the name was changed to the University of North Texas Health Science Center at Fort Worth. In 1999, UNTHSC joined the UNT System.

WHAT SHOULD I EXPECT DURING NEW STUDENT ORIENTATION? Youareabouttoembarkonanewandexcitingpathinyourlife.Thefaculty,staffandcurrentstudentsareeagerlylooking forward to providing resources and guidance to support your success. New Student Orientation is mandatory for all incoming UNTHSC students. This is your opportunity to learn about academic and community expectations, campus culture, and services and resources available to you. We welcome your questions and full participation in all parts of the program! You are also encouraged to start building relationships with your peers. Along with your family and friends, we understand that this is a vital part of your support network. Orientationsessionswillbeheldbetween8a.m.and5p.m.Thenumberofdayswillvarydependingonyourprogram. In preparing to attend orientation, please consider the following:

1) Punctuality: Throughout your education and in your professional career, the habit of tardiness is one of the single most destructive traits you can possess. We ask that you be on time for orientation, every class, workshop, and meeting that you attend. It is a matter of courtesy to your fellow participants, orientation organizers, and session speakers. All orientation sessions are mandatory unless otherwise indicated. Failure to attend every .. session may result in dismissal.

2) Attire: Dress for orientation is usually business casual. Your schedule will indicate when business casual attire is not required. It is important that you comply with the UNTHSC student dress code at all times when on ourcampusorinoff-siteclinics.

3) Please do not bring a spouse, parent, guest, child, etc. with you to orientation. Orientation programs are designed for incoming students only. Any individual not accepted into a graduate program will be asked to leave.

4)Breakfastandlunchwillgenerallybeprovided,includingvegetarianoptions.Seeorientationschedules for details.

New Student Orientation is a process, not merely an event. In order to best serve you, we have prepared a Getting ready for UNTHSC checklist for you. You can get started with your checklist right away! Th e NSO schedule will be posted on the website no later than one month before orientation. WHEN SHOULD I PURCHASE BOOKS? Outside of certain fi xed college costs, the most expensive purchase will be books, required or recommended. Booklists will beprovidedtoyoubeforeclassesbegin(usually2–4weekspriortothefirstdayofclass).UNTHSCdoesnothaveanon-campus bookstore Here are a few guidelines to consider as you purchase your textbooks: (1) consider buying used textbooks from current students at the beginning of each semester, and (2) don’t be afraid to consult current students within your program for advice on textbooks and about the possibility of borrowing/buying their books.

GET CONNECTEDstudents. It is our goal to provide you, our students, with fi nancial assistance and advice to help you manage the costs associated with your educational pursuits. Th e Free Application for Federal Student Aid (FAFSA) is available January 1st of each year at www.fafsa.ed.gov. Th e FAFSA is your application for federal direct student loans and state grant funding. You will be notifi ed via email once you have been awarded fi nancial aid. Additional instructions will be included on how to accept/decline/reduce your aid. OnceyouacceptyourfederaldirectstudentloansonyourMyHSC,youmustthencompleteEntranceCounselingandsign a Master Promissory Note at www.studentloans.gov. IMMUNIZATION REQUIREMENTS & BACTERIAL MENINGITISImmunization requirements of ALL UNTHSC [TCOM, GSBS, SPH, PAS, DPT & Pharmacy] students are: • HepatitisBSeries • Measles,mumps,rubella(MMR) • InactivatedPolio(IPV) • Diptheria,tetanus,pertussis(Tdap) • Varicellaorevidenceofchickenpoxduringchildhood

Evidenceofhavingreceivedtheseimmunizationsmaybemadeeitherbyprovidinganimmunizationrecordfromthestudent’s primary care provider, or by having titers drawn. Titers are blood tests that show whether you have immunity to a specifi c disease. Immunizations may also be provided at the Student Health Clinic, and will be covered as outlined under the student’s individual insurance plan. Incoming students (those who are entering UNTHSC for the fi rst time, or returning aft er being out for more than one fall or spring semester), are additionally required as of January 1, 2012 to produce evidence they received an initial bacterial meningitis vaccination dose or booster during the fi ve year period preceding the admission. Th is must be submitted at least 10 days prior to the fi rst day of the fi rst semester in which they enroll.(MoreinformationandexceptionstotherequirementforbacterialmeningitisislistedontheStudentAffairswebsite at www.hsc.unt.edu/departments/registrar/MeningitisInformation.cfm). Tuberculosis testing (PPD) is provided annually by the Student Health Clinic personnel at various times during the year. If you have a history of a positive PPD, please schedule an appointment with the Student Health Clinic and bring all medical records with you, including your most recent chest X-ray.

Additional immunization requirements of TCOM, PAS, DPT & Pharmacy students are:

If you are a TCOM, PAS, DPT or Pharmacy student, you are also required to have a complete history and physical within one year of matriculation. You will also need boosters related to the Tetanus, Diptheria, Acellular Pertussis (Tdap) doses if you have not received these within the last two years.

Student email and login accounts are automatically processed and created for all UNTHSC students. You will receive your email addresses and login information at New Student Orientation. WHEN WILL I GET MY ID BADGE?YouwilltakeyourIDbadgephotoduringNewStudentOrientation.TheIDbadgeofficeislocatedinthelobbyofthePoliceDepartmentatthecornerofMattisonandMontgomerystreetsacrossfromtheMedicalEducationTraining(MET)building.TheBadgeofficehoursareMonday–Friday8:30a.m.to4:30p.m. AllreplacementIDbadgeswithanewphotoare$25.Anyreplacement,nameorclasschangeusingthecurrentphotoonfileis$20.PleasenotethatanameorclasschangerequiresasignaturefromtheRegistrar’soffice. ReportlostorstolenIDbadgestothePoliceDepartmentimmediately.Thecardwillbeforcedinactiveuntilwearenotified that the badge had been found or until a new one is made. LOCKERSUNTHSClockersareavailabletoallstudents.LockersarelocatedintheEverettEducation&AdministrationbuildingandtheMedicalEducation&Training(MET)building.Lockersarecommonlyusedbystudentstostorebooks,laptops, research materials, and lab items. During New Student Orientation, you will receive a locker request form via email with instructions on how to request a locker. Lockers are assigned on a first-come, first-serve basis and are assigned within a week of submitting your request.

PARKING ON CAMPUSToreceiveavalidparkingpermit,youmustobtainaparkingpermitfromtheParkingOfficelocatedwithinthePoliceDepartment.Allcampusparkinglotsrequireapermit.Studentpermitsare$90each.Permitsmayalsobeobtainedonlineatwww.unt.edu/transit/HSCParking.html,onceregisteredstudentinformationisprovidedtotheParkingOfficebytheRegistrar’sOffice.Anadditionalpermitmaybepurchasedforasecondvehicleaslongasitisregisteredtothesame household; however, only one vehicle at a time may be parked on campus. (It will be an automatic cancellation of parking privileges if additional parking permits are purchased and then given or sold to someone other than the purchasing party).

YouwillhavetimetovisittheParkingOfficeduringNewStudentOrientationtopurchaseyourparkingpermit.PermitsaredistributeduponpurchasewiththecompletionoftheParkingPermitVehicleInformationForm.Youwillneed your license plate number as well as the make and model of the vehicle (s) you wish to register. Payments may be by cash, check or credit card. Parking permits will need to be purchased each year. Garage access is granted only to those who purchase a permit. Aparkingpermitdoesnotguaranteeaparkingspace,butallowsparkingifaspaceisavailable.Eachvehicleoperatorisresponsible for fi nding a legal space in the parking area designated by the permit they possess. Lack of space is not a valid excuse for violating any parking regulation. UNTHSC assumes no responsibility for any vehicle or its contents while the vehicle is parked or operated on campus. All vehicle operators should lock or otherwise secure their vehicles to deter theft or other criminal acts.
