Awards Application
The Gold Crown Chapter Awards honor outstanding achievements by American Student
Dental Association chapters in serving members in accordance with the Ideal Chapter
criteria. All materials must be uploaded and received by Jan. 31, 2014. Please note: All
entries must be submitted as both a PDF and a hard copy or iBook. Applications cannot
be submitted by a district trustee or any ASDA national leader.

Socials
As the end of the 2013-2014 school year approaches, the University Florida chapter of the American Student Dental Association is wrapping up a busy but exciting and fulfilling
year. Our overall goal this year was to encourage our student population to get excited about dentistry and to get excited about ASDA as a forum for positive change within our
chosen profession. Looking back, our most successful events were the ones that were, simply put, fun. Dentistry should be fun, and so should ASDA. This was a year to celebrate our
profession and our students’ accomplishments, and we look forward to what the future will bring to our chapter of ASDA.
• Spring Banquet: About 250 students and faculty members attended for dinner, dancing, and an awards presentation that included awards for our ASDA members, faculty
members, and our entire ASDA membership at UF (250 attendees)
• Annual Mighty Molar Picnic and Competition: faculty, students, and family members came to compete against each other in events such as tug of war, trayless impressions
(using hands and gloves), relay races, and a balloon toss in the battle for a year’s worth of bragging rights. UF ASDA provides food, t-shirts, a raffle with over $2500 worth
of prizes, and a dunk tank. After a day of sun, rain, competition, and bonding, we were able to raise $2456 for the organization Smile Train, an organization that benefits
the families of children in need of cleft lip and palate surgery. (300 attendees, raised $2456 for Smile Train)
• Halloween Social: Approximately 80 people from first year to fourth year came out in spooky attire to mingle. (80 attendees)
• Welcome Week activities for incoming freshmen: put on a Happy Hour social event, planned and participated in Organization Fair, and funded the gift of 114 tackle
boxes as storage for new students’ dental supplies (200 attendees)
• 2 FDA Socials: funded and organized Happy Hour socials for alumni and current students to make professional connections over food and drinks (110 attendees each
event)
• 2 Happy hour socials for ASDA members (100 attendees each event)
Professional Events and Fundraisers
• Annual Freshman Loupes Fair: 7 vendors for dental loupes and eyewear were present to help freshmen and sophomore ASDA members have the opportunity to try out
loupes (114 attendees, $4,200 dollars raised with no expenses)
• Vendor Fair: Over 30 vendors attended, with ASDA providing lunch and a raffle. (Over 300 attendees including dental students, predental students, faculty and staff)
• Business Symposium: practice and financial management seminar including 9 available presentations (listed below). Over 30 members of LECOM School of Dental
Medicine were in attendance. The event included breakfast, lunch, a raffle, and a mini vendor fair. (130 attendees, raised $750)
• UF ASDA Election Day: Lunch, speeches, and an election (130 attendees)
• Lunch and Learns and Dinner and Learns: available once a week throughout the year. 1 hour sessions on a weekly basis with free lunch provided for chapter members.
(100% attendance from ASDA members throughout the year)
• UF ASDA Merchandise fundraiser: featured hats, sweatshirts, and t-shirts, and sold over 300 units of merchandise, including our official UF ASDA t-shirt (sold over 230 units
of merchandise, raised over $400)
• Pre Dental day: Pre-Dental students from schools across Florida came to get a tour of the University of Florida College of Dentistry, had a lecture from members of the
Admissions committee, and did waxing and impressions in the clinic. were also able to attend vendor fair (60 attendees including those from UF, Nova, Lecom, USF, UCF,
UWF, UNF, University of Miami)
• Impressions Program: Title sponsor of the annual UFCD SNDA Impressions Program. A day dedicated to the exposure of minority and underrepresented undergraduates to
the dental field through experiences like waxing sessions, diagnostic impressions on each other, and a Q&A. (84 attendees)
Organized Dentistry
• Florida Dental Association House of Delegates: the President and President Elect of UF ASDA attended the FDA HOD as 1st and 2nd delegates. (2 attendees)
• Florida National Dental Convention (8 attendees from UF ASDA)
• ASDA District 5 Meeting in Destin (3 attendees from UF ASDA)
• National Leadership Conference (NLC) (10 attendees from UF ASDA)
• Annual Session (10 attendees)
• Florida Dental Association Dentist Day on the Hill: Students accompanied members of the Florida Dental Assoc. to Tallahassee, FL to discuss dental legislative issues with
the Governor, House Representatives and Senators. (8 attendees)
• State Lobby Day: Students meet with House Representatives and Senators to discuss issues directly related to dental school, student loans and access to care. (9 attendees)
• ASDA National Lobby Day: Chapter Legislative Liaisons visited Washington D.C. to accompany other ASDA members in discussing dental issues with government officials.
• Lunch and Learn featuring District 5 Trustee, Jason Watts: Discussion with ASDA general body members on how to get more involved with ASDA both on a chapter level.

Community Service:
• As of the most recent US census, about 249,365 people permanently reside within our very own Alachua County, and of those 23.6% are found beneath the poverty level.
• Dinner and “Oral Health 101” talks for local homeless shelter St. Francis House once a month--9 events total (16 dental school student participants at each event, serve
about 36 lodgers at each events)
• Great Gator Health Fest: Health fair hosting all the professional schools at the University of Florida. Organized a booth and passed out oral hygiene supplies and raised
oral health awareness of students, alumni, and their families (300 attendees)
• Lessons in a Lunchbox: An oral health literacy program designed to ensure teachers, parents and children are all working together to help eliminate tooth decay within
children. UF ASDA members visit elementary schools, give a 60 presentation on oral hygiene and healthy eating, and demonstrate proper brushing and flossing. (30 ASDA
members participated, 60 school children, parents, and teachers in attendance)
• ASDA-Predental Mentorship Program: ASDA members currently enrolled at UFCD are paired with an undergraduate predental student to offer advice and tutelage on
undergraduate subjects, application to dental school, and the transformation from undergrad to dental school. (86 mentors, 116 mentees participating)
