The Publishing Industry Freight & Distribution Plan has been a project of the Book and Periodical Council (BPC) since 1982 and is supported by its members. An industry committee, which annually recommends vendors chosen for the excellent rates, performance, technology, and value-added services they offer to the publishing industry, directs the Freight & Distribution Plan. The committee’s initiatives also include research and education on freight and warehouse operations issues. Current committee members:

The freight rates in this Plan are designed for books and periodicals and therefore are based upon shipments with a minimum density of approx. 20 lbs (9 kg) per cubic foot.

Ancillary and Miscellaneous Rates and Terms and Conditions

Collect On Delivery (C.O.D.) Canada: Ground: Single piece: $3.50 plus $1.00 per $100 (or part) over $100 per shipment; Multi-piece (2-25 pieces): $7.00 plus $1.00 per $100 (or part) over $100 per shipment. Canada: Select and Overnight: $10.00 plus $1.00 per $100 (or part) over $250 per shipment. Maximum C.O.D. value per shipment is $40,000 with no single piece to exceed $20,000.

Declared Value (DV) Canada and USA: Ground: $1.00 per $100 (or part) of the declared value in excess of $100 per shipment. Select, Overnight and International: $1.00 per $100 (or part) of the declared value in excess of $250 per shipment

Extra Care (XC) All services: $10.00 per package. XC applies to packages that require special handling due to their shape, packaging or lack of packaging, or size. In the case of size, any item with a combined length and girth greater than 130 inches/330 cm (but less than or equal to 165 inches/419 cm) or items with a single dimension greater than 84 inches/213 cm (for USA 60 inches/152 cm) but less than or equal to 108 inches/274 cm are considered XC. XC charges may be levied when not indicated on the pick up record (manifest)

Pick Up Tag (P.U.T.) Canada Ground: $4.50 per shipment plus shipping charges. Call in/ fax in $6.50 per shipment plus shipping charges.

Address Correction: Canada: $4.50 per package. Maximum - $9.00 per shipment. U.S.A.: $8.00 per package.

Extended Area delivery: Canada: Extended Area delivery postal codes and charges can be viewed on your customer portal and are in addition to the shipping charges. USA: Commercial - $1.60 per package, Residential - $5.60 per package.

Dimensional or Actual Weight: Charges will be levied based on actual or dimensional weight. Ground: dimensional weight is based on 10 lbs. per cubic foot. Select, Overnight and International: dimensional weight is based on 15 lbs. per cubic foot.

Fuel Surcharge: Fuel surcharges can be viewed on the Canpar Website. Fuel surcharges are in addition to the shipping charges.

Missing Collect on Delivery (C.O.D.) amount on a pick-up record (manifest): $8.00 per shipment.

Missing or invalid postal code or account number on a pick up record (manifest): $5.00 per shipment.

Missing Weight: Where the weight is not provided on the pick-up record (manifest) CANPAR will calculate the shipment weight based on 35 lbs. (16 kg) per package and rate the shipment on the resulting weight.

Oversize and Overweight: Canpar, at its’ discretion, may deliver oversize and/ or overweight packages found in our system. The charge for this will be $40.00 per package in addition to the regular shipping charges.

Residential Delivery: Canada: $1.75 per shipment, USA: $2.80 per package.

* Return To Shipper (RTS): If for any reason Canpar cannot deliver a parcel and returns it to the shipper, a charge equal to the out bound delivery rate will be applicable. (*New this year)

Service Charge: $6.00 per week.

Applicable Taxes Extra

Liability: Ground: Maximum liability - $20,000 per shipment with maximum for any one (1) package of $5000 if value declared. Otherwise, liability is $100 or $2.00/lb ($4.41/kg) per shipment, whichever is greater. Select, Overnight and International: Maximum liability - $25,000 per shipment if value declared. Otherwise liability is $250 or $2.00/lb ($4.41/kg) per shipment, whichever is greater. For shipments containing personal articles or defective goods, maximum liability is limited to $2.00/lb ($4.41/kg) per shipment, with no additional valuation allowed.

Maximum Length: 108 inches (274 cm)

Maximum Shipment: 25 packages or 500 pounds (225 kg) per pick-up record line.

Maximum Size Package: Canada: 165 inches (419cm) in combined length and girth. USA and International: 130 inches (330 cm) in combined length and girth.

Maximum Weight: 75 pounds (34 kg) per package.

Minimum weight per multi piece shipment: A minimum weight of 4 lbs (2 kg) per piece charge shall be assessed where average weight per piece in a multiple piece shipment is less than 4 lbs (2 kg) per piece.

Labeling Detail

Canpar facilities use state-of-the-art technology to sort your packages. This automated sortation process ensures proper sorting and virtually eliminates damage. In order to achieve optimum service, it is critical that we are able to scan your packages correctly. In order to maximize this automated process please ensure you follow the guidelines below:

1. Apply all barcoded labels on a completely flat surface on the exterior of the package, ensuring the label is not over the tape.

2. Ensure your computer printer has a sufficient supply of ink or toner to ensure barcodes are clearly printed.

3. Please check the barcodes to avoid wrinkles or scuffs.

4. If you are re-using a box, please remove all old barcoded labels from the box.

5. When using tubes or round packaging, ensure labels lay along the straight edge.

Please see the images below for tips on what to avoid when labeling your box:

Do not put packaging straps, shrink wrap or cellophane over barcoded labels. Do not apply any barcoded labels on box seams.

Do not apply labels over corners or edges of boxes.

Do not apply packing tape over any barcoded labels.

How to Pack

Proper packaging is critical to ensure the protection of your parcel. We suggest the following tips when preparing your packages for shipping:

1. Use new rigid corrugated cartons. 2. Check the Gross Weight Limit on the bottom of the box and do not exceed.

3. Pack securely with each item wrapped separately with shipping foam, bubble wrap or other shipping material (2 inches between carton walls and goods inside).

4. Protect sharp and pointed objects with added shipping material. 5. Separate glass containers with corrugated dividers. 6. Ship boxes separately – do not bind together. 7. Secure the lids on liquid filled containers (seal liquid filled containers in plastic bags or

packs to prevent spillage) and ensure safety caps on plastic bottles.

We are grateful to be considered by the Book and Periodical Council for the position of recommended carrier for the 2012/2013 shipping season. Calyx Ground Transportation Solutions provides a host of easily accessible services for your members domestically: expedited road & intermodal service to Western Canada (out and return), road service within Ontario & Quebec, as well as road and intermodal service to Eastern Canada.

Given cost escalations we require a 1.5% increase for this shipping season. This has been applied across the board to all points and to all levels of rates for April 1st, 2012. In an effort to mitigate this increase as much as we are able, we are holding our accessorial charges at the same level as they were at for the 2010 shipping season.

Calyx Ground Transportation Solutions will continue its current fuel surcharge plan with the Book and Periodical Council. Our monthly fuel surcharge has been harmonized between the companies and is available on either of our websites (National Fast Freight or Totalline Transport).

SCHEDULE OF ACCESSORIAL CHARGES

ISSUED TO: Book and Periodical Council

1 APPOINTMENT FEEPre-booked appointment freight, freight requiring an appointment for delivery or freight requiring a call prior to delivery will be subject to a $25.00 surcharge.

2 ATTEMPTED PICK UP FEEFreight called into dispatch and unavailable upon the driver arriving at the shipper's location will be charged an attempted pick up fee of $30 plus fuel within the major metropolitan centres.

3 BASEMENT/SECOND FLOOR DELIVERIESRates are prepared based on dock to dock deliveries. Shipments requiring delivery to consignees in multiple floor locations will incur an additional charge of $50.00.

4 BILL OF LADINGAvailable over the web at: http://www.nationalfastfreight.com

5 CHEQUE PICK UP FEEIf the shipper requires a cheque to be picked up at the time of delivering the freight, a $40 surcharge will apply.

6 CLAIMSAny claim of $50.00 or less will not be processed.

7 COD FEEDeliveries requiring a cheque to be picked up for the value of the goods being delivered, before release of such goods, a 6% fee on the value of the goods with a minimum of $50.00 will be charged. COD pick ups are only available if the freight charges are prepaid by the shipper.

8 CONSOLIDATED RATINGConsolidated rating is available only if the customer provides a master bill of lading. Shipments must be received on the same inbound unit and must be able to be delivered to the same consignee address and dock at the same time.

9 CPC PALLET RETURNIf a CPC pallet return is required, the charge assessed will be $5.00 per pallet with a minimum charge of $45.00 applied.

10 DANGEROUS GOODS On shipments moving under the regulations for the Transportation of Dangerous Goods, a charge of $15.00 per bill of lading will be assessed.

11 FLATBED DELIVERY CHARGEShipments requiring the use of a flat bed for delivery will be subject to an additional charge of $150.00 per delivery within Winnipeg, Regina, Saskatoon, Calgary, Edmonton or Vancouver. For all other locations please call for a quote.

12 HEATED SERVICEHeated service is available from October 15th to April 14th at a surcharge of 15% of freight charges or $15 minimum, whichever is greater. Please note: heated service only protects the products from freezing, it is not a temperature controlled environment.

13 LATE PAYMENT FEECalculated monthly at 1.5%, 18% per annum commencing thirty (30) days from date of invoice.

14 LIABILITY/DECLARED VALUEAll rates and charges are based upon a liability not to exceed $2.00 per pound, unless a higher value is declared by the shipper. If the shipper declares a value greater than $2.00 per pound, a charge of 2% (minimum $5.00) of the valuation in excess of $2.00 per pound based on actual weight will be assessed. If no value is declared, the Carrier's maximum liability is $2.00 per pound. Shipments with a declared value in excess of $10.00 per pound must be pre-approved by the Carrier. With respect to appointment deliveries, NFF is not liable for any penalties that may result from carrier non-performance.

15 LUMPER/SWAMPER CONSIGNEE TI-HI FEESConsignees that require reconfiguration of skids at destination terminal and/or upon delivery of product at their physical location will be subject to a surcharge of 15% of freight charges; minimum $5.00 per order, maximum $300.00 per order.

16 NON-COMMERCIAL/INDUSTRIAL & RESIDENTIAL DELIVERIESShipments that require special handling due to location and receiving facilities are subject to an additional $75.00 surcharge. This charge would apply to, but is not limited to, construction sites, rural route addresses, golf and country clubs, ski resorts, public storage lockers, schools, banks and private residences.

SCHEDULE OF ACCESSORIAL CHARGES

17 OWNER'S RISK OF BREAKAGE & DAMAGEWhen a shipment is deemed by the pick up driver to be improperly or insufficiently packaged for transport, the driver is instructed to note on the shipper's bill of lading "Owners Risk of Damage". The Carrier would then be liable for damages only if carrier negligence can be proven. Declared values will not be accepted on shipments that move at owner's risk of damage.

18 POWER TAILGATE CHARGEShipments requiring the use of a power tailgate to perform a delivery will be subject to a surcharge of $50.00.

19 PREMIUM OR NON STANDARD DELIVERY CHARGEShipments made outside of regular service parameters requiring early morning, evening or weekend pick-ups and/or deliveries will be charged at $55.00 per hour with a minimum charge of 2 hours.

21 RE-DELIVERY/WRONG ADDRESS CHARGESShipments that require redelivery will be subject to the following cwt rate scale: MIN / $35.00 - LTL / $3.50 - MAX / $200.00* Subject to 10 lbs per cubic foot* Rates are good only if re-delivery occurs in the same city as the original delivering terminal.* Fuel surcharge is applicable on these charges.

22 SHIPMENT WEIGHT AND CUBEShipment weight must include both packaging and skid weight in the total amount declared on the bill of lading. National reserves the right to scale and measure each shipment that enters into our network to ensure accuracy. Should a discrepancy be found with the bill of lading, National reserves the right to apply a surcharge to the order to cover the deficit weight and or cube.

23 STORAGE CHARGESShipments held in our facilities, at the request of the shipper, will be subject to the following storage charges: $7.50 per skid per day, minimum $22.50, maximum $75.00 per order per day. A 48 hour grace period will be allowed at destination terminal prior to charges being assessed. Charges are applicable on weekends and statutory holidays.

24 TRADE SHOWS & EXIBITIONSPick up and deliveries performed at the following, but not limited to, trade shows, conventions, fairs will be subject to $4.50 per cwt on the actual weight with a minimum charge of $75.00 per shipment.

25 TRAILER/CONTAINER DETENTIONDeliveries requiring a dropped trailer for unloading will be permitted an offloading allowance of 24 hours. Days incurred in excess of the allowance will be charged a fee of $75.00 per day for dry units and $150 per day for heated units. Heated units are applicable from November 1st to March 31st. Charges are applicable on weekends and statutory holidays.

26 WAITING TIME/HANDBOMBPick up and deliveries are allotted time based on shipment size. Time in excess of this allowance will be charged at $55.00 per hour. The following unloading allowances will be permitted:

Bill of Lading (BOL)

Requesting a Pallet Shipment Freight Quote

1. Identify your company and your carrier account number 2. Describe the shipment including number and dimensions of each pallet 3. Confirm the total actual weight of the shipment 4. Give pick up and destination addresses 5. Request any special services 6. Advise when the shipment will be ready to move 7. Record date, quote number and price for your records 8. Ensure that freight, fuel surcharge and any special service accessorial charges have been included

Waybill (Bill of Lading) Preparation Bill of Lading (BOL) is the document that establishes terms of contract between the shipper and the transportation company as well as a title deed which proves ownership of the goods mentioned. Please ensure that the following information is entered on each Bill of Lading:

# of pallets and number of cartons Accurate weight (actual and dimensional weight and measurements) Shipper (where is the product being picked up) plus contact information and dock hours Consignee (where is the product being delivered) plus contact information and dock hours Internal reference no. (example Sales Order number) Who is paying for this shipment? 1) Prepaid (shipper pays) 2) Collect (consignee pays) 3) 3rd party * Product being transported (example books) Purchase Order or reference number to identify the shipment to the consignee (e.g. for Costco always

enter the PO #) Deliver by date or delivery window requirement if any Record Carrier Quote number or pick up reference (if applicable) Record Type of service (if applicable) e.g. Regular intermodal service, Expedited service, Overnight, Air Instructions for special services if required such as: delivery appt required, driver assist, tailgate

delivery, etc. If a shipment is being charged to a 3rd party account, then it must say 3rd party billing and list the name of the company to be billed as well as their carrier account number. Send a completed Bill of Lading to the pick up location (or review a copy of the bill to be used) to ensure that all information is correct. Loading/Unloading and Bill of Lading Signatures Date and printed name should always accompany each signature. Pick up: The carrier driver and the pick up shipping staff are required to sign the Bill of Lading. The BOL is a receipt for consignment and the driver signature certifies receipt of goods mentioned in the BOL for transportation to the specified destination. Since the carrier driver rarely loads his own vehicle, the driver will often sign as SLC or Shipper’s Load and Count. The pick up shipper must be very careful to confirm that the shipment loaded matches the details on the Bill of Lading. Delivery: The delivery driver and the consignee receiver are required to sign the Bill of Lading. If the pallets are wrapped and the number of cartons cannot be verified at the time of receipt, please write “subject to count” before receiver signature. Please record any damage or shortage before receiver signature.
