2015 BC ABORIGINAL PROVINCIAL VOLLEYBALL CAMP Fort St. James, BC March 21, 2015 The Aboriginal Sport, Recreation and Physical Activity Partners Council (Partners Council) of BC is pleased to announce a 2015 BC Aboriginal Provincial Volleyball Development Camp will be hosted by Nak’azdli First Nation on Saturday, March 21 st , 2015 in Fort St. James, BC. Sanctioned by Volleyball BC, this 1-day co-ed camp is for Aboriginal youth volleyball players, ages 11 to 17, who want to improve on existing skills and gain quality coaching and instruction in a positive, fun and professional atmosphere. Lunch included. With the support of our partner, the Province of BC, this camp offers tremendous value and is a rare opportunity for volleyball lovers! It is FREE to participate, but, you must PRE-REGISTER! Who: Male and female players of Aboriginal ancestry (First Nations, Métis, Inuit) ages 11 to 17. Fee: Free! Participants must pre-register. The camp includes lunch and a souvenir camp t-shirt. Camp Facilitator: Guest Coach Facilitator - Jeff Ross. Jeff Ross, Assistant Coach for the UBC Thunderbirds, is a Level 2 coach with over 10 years of coaching experience. He served as Head Coach for the Team BC U16 Western Elite's 2014 summer program and is in his 3rd year of coaching the WPGA Wolves Senior women's team and 5th year coaching with Thunder Volleyball club in Vancouver. Jeff was also the Head Coach for Team Saskatchewan Senior Women's Volleyball team at Cowichan 2008 NAIG. His athletic career includes playing volleyball with UBC men's program and also with MacEwan University Griffins and Red Deer College Kings in Alberta.

WAIVER & RELEASE OF LIABILITYIn the consideration of the Aboriginal Sport, Recreation and Physical Activity Partners Council (Partners Council) accepting my registration and allowing me to participate in the 2015 BC Aboriginal Provincial Development Camp (Camp), I myself, my heirs, executors, administrators and assigns hereby agree to: 1. RELEASE the BC Association of Aboriginal Friendship Centres (as the host organization of the Partners Council), Nak’azdli Band (event host), Kwah Hall (venue),

Volleyball BC, its partners, volunteers, servants, agents, employees and other participants of the Event (all of whom are hereinafter collectively referred to as the “Releasees”), from any and all claims, actions, costs, demands and expenses arising out of or in consequence of any loss, injury or damage to my person or personal property incurred while attending at or participating in the Camp, notwithstanding that any such loss, injury or damage may result from the negligence of the Releasees. In addition, permission is granted to administer any medical treatment that may be required.

2. WAIVE ANY AND ALL CLAIMS that I, my heirs, executors, administrators, insurers, successors and assigns have or may have in the future against the Releasees.3. GRANT to the BC Association of Aboriginal Friendship Centres (as the host organization of the Partners Council), the right to use, without payment of any fee, charge or

compensation of any kind, including royalties, and all written information, and/or any and all photographs, video tape or other visual media of myself taken during the Camp for non-commercial, promotional purposes, educational programs and I also agree to waive any right to approve such use.

I understand the rules and regulations are designed for the safety and protection of participants and hereby agree to abide by the rules, regulations set by the host, coach(es) and the Partners Council and BC Association of Aboriginal Friendship Centres. I have read this release of liability and assumption of risk agreement, and fully understand its terms. I understand that I have given up substantial rights by signing it, and sign freely voluntarily, without any inducement.
