Guest Player Form

This Guest Player Permit is for use by the team borrowing the registered guest player who has been given permission by his/her USSF affiliated (Lending) Team coach to participate as a guest player in a FWUSA hosted soccer activity (tournament, tryouts, indoor game, etc.) with the Borrowing Team. This form must be submitted along with all other required documents to the FWUSA tournament registrar prior to guest players playing in the Fort Wayne Invitational event.

1. Enter the required information as specified. 2. The Guest Player must be a currently registered USSF player who has been released by

his/her registered (Lending) team coach to play as guest player with the Borrowing Team for the activity indicated. The Guest Player must use the player pass from his/her USSF currently registered league team or his/her current USSF Tournament Pass.

3. If player is not currently registered with USSF affiliated team the player must obtain a Tournament Pass from the player’s resident state association.

4. List the name of the Borrowing Team’s player that is being replaced by the Guest Player.
