Dear Friends,
Many exciting things are happening at PCHC and we are pleased to provide youwith this annual report.
Expanding Access to Health Care: We added 1,000 new patients in 2006and plan to add 1,000 more in 2007. In fact, more than 18,000 people reliedon PCHC for medical, dental, mental health and pharmaceutical services lastyear.
Improving Chronic Disease Management: One-on-one personalized care isintegral for our patients’ success with managing their health. Since we startedproviding thorough self management education for diabetes care a few yearsago, the average blood sugar level of our 1,700 diabetic patients has beenreduced from 9.2% to 7.3%, which improves the health of these patients andreduces emergency room use and hospitalizations.
Preventing Future Disease: PCHC’s infant immunization program was thebest among community health centers in Colorado for the seventh consecutiveyear. Our immunization rate for 1,400 children up to two years of age is over99%, compared to the statewide average of 83%.
Ensuring Financial Viability: We recently formed PCHC Foundation and chargedits Board of Directors with the responsibility of securing long- and short-termfunds for PCHC. Look for this great group of volunteers to achieve excitingresults.
Without the support of Pueblo’s outstanding health care community, localbusinesses, community leaders, foundations, government entities and electedofficials, none of this would be possible. Thank you for caring about Pueblo’shealth. Together, our community will thrive.

Introduction
Pueblo Community Health Center is dedicated to providing topquality primary health care to those in need. Here are key 2006accomplishments:
We served 18,229 patients, up 5.8% over 2005.
We provided 93,030 medical, dental and mental healthvisits, up 4% over 2005.
We delivered 509 babies and enrolled 637 new patientsinto prenatal care, both new records.
We dispensed more than 15,000 prescriptions throughthe indigent drug subsidy program.
We case managed 1,198 people in our cancer screeningprogram, identifying 9 cancers and securing treatmentfor these women.
Our patient satisfaction scores remained above 95% inalmost every area and 99% of our patients wouldrecommend PCHC to others.
We completed our 11th consecutive year in the black.

Throughout our history, Pueblo Community Health Centerhas provided essential primary health care services to acontinually growing number of uninsured and underinsuredindividuals by expanding services and adding locations.In fact, the number of patients we served in 2006—18,229—is nearly double the number of patients we servedjust ten years ago!

PCHC’s Perinatal and Nurse-Family Partnership programswork to ensure that our pregnant patients receive the highestquality care possible. Take Cathy as an example: Cathy is afirst-time, low-income mom-to-be. She heard about PCHC’sfree pregnancy test offer and came to PCHC in May 2006asking for a test.
As Cathy waited in the examroom for test results, Certified NurseMidwife Wendy met with Cathy andtried to learn more about how thislife changing event will affect Cathy.During the conversation, Wendylearned that Cathy and her husbandwere trying to have a baby but wereconcerned about financialresponsibilities, along with manyother joys and fears.
The free pregnancy testconfirmed Cathy’s hunch that shewas indeed pregnant. Wendysuggested that Cathy enroll intoPCHC’s Nurse-Family Partnership Program. By participatingin this program, Cathy and her husband would learn aboutnutritional and healthy choices during pregnancy; stages ofpregnancy; how to budget; how to stay focused on educationaland career goals; and so much more. The Nurse-FamilyPartnership registered nurse would provide personal carethrough their child’s second birthday.
Throughout her pregnancy, Cathy has kept all of hermedical and Nurse-Family Partnership appointments. Thisfamily can count on personalized care through Nurse-FamilyPartnership, as well as a long, trusting relationship with PCHCstaff members.
