Igniting Action Through Regional Collaboration -Why and How It Works – Elizabeth Mitchell, President & CEO, Network for Regional Healthcare Improvement (NRHI)Regional Health Improvement Collaboratives (RHIC) are multi-stakeholder organizations which include providers, health plans, employers and community leaders working to transform the healthcare delivery system to improve the patient experience of care and the health of populations and reduce the per-capita cost of healthcare. Ms. Mitchell will share why RHICs are important and give examples of how RHICs across the country have improved local care delivery systems.

What Can Employers Do to Support Patient Centered Care?– Marci Nielsen, PhD, MPH, CEO, Patient Centered Primary Care Collaborative (PCPCC)Employers are seeing the benefits of patient centered primary care and learning more about value-based purchasing as part of payment reform. Dr. Nielsen will share how employers and purchasers have partnered with primary care to manage costs, provide better access to care, and begin to engage patients/employees. Learn about incentives that engage patients to take responsibility for their health and well-being, and part-ner with clinicians to promote quality improvement. Community First has two strategies to achieve sustainability in healthcare: To tip the paradigm of healthcare from treating disease to caring for health through grassroots initiatives. Secondly, to foster collaboration and trust between the healthcare stakeholders so they can make necessary structural changes in the system. The East Hawaii Regional Health Improvement Collaborative (RHIC) is the vehicle for doing this. Before the RHIC, no one could remember a time when physicians, the hospital, health plans, businesses, and the County were at the table together.  Panelists will discuss the difference this has made for them as initiatives are implemented to design the best heart care and recruit/retain physicians in East Hawaii.

Improving Health Via Community Outreach and Engagement– David D. Derauf, MD, MPH, Executive Director, Kokua Kalihi Valley Comprehensive Family ServicesKokua Kalihi Valley Comprehensive Family Services (KKV) is a Federally Qualified Healthcare Center (FQHC) providing comprehensive support to the community. Learn how KKV is able to work beyond the exam room to address the root causes of health disparities through innovative programming involving all age groups. Discover how KKV successfully works to integrate medical treatment with social, spiritual, and cultural well-being through programs such as Ho‘oulu ‘Āina, K-VIBE, chronic disease self-management groups, Kalihi Elders, and Seams Wonderful. The presentation will demonstrate how these programs have cost effectively improved the health of participants and review lessons learned from “walking the talk.”

Improving Patient Outcomes Through Health Coaching and Health Literacy– Rachel Willard-Grace, MPH, Research Manager, Center for Excellence in Primary Care (CEPC) University of California San FranciscoMs. Willard-Grace will speak about the importance of health coaching and how it is different from the traditional approach to primary care. She will illustrate how it works and review evidence on why it is effective in managing cost and providing better care. She will cover how a small practice can use medical assistants and other existing staff and how a community of physicians might build capacity for health coaching. Ms. Willard-Grace will also explain health literacy and how it is critical to empowering patients to partner with healthcare providers.

Empowering Patients and Providers to Choose Wisely– Beccah Rothschild, MPA, Senior Outreach Leader, Consumer ReportsMs. Rothschild will share why Consumer Reports is doing “Choosing Wisely” and its key partnership in it. She will describe resources that Consumer Reports has created, as well as interventions and outcomes used nation-wide to advance the campaign. SUMMER ALLEN, MD, is an Assistant Professor of Family Medicine and consultant in Family Medicine at Mayo Clinic, in Rochester, Minnesota. Dr. Allen serves as Medical Director of Patient Experience for the Mayo Clinic Health System (Southeast Minnesota region). Previously, she led Patient Engagement for the Mayo Clinic Office of Population Health Management. Dr. Allen has been with the Employee and Community Health practice in Rochester since 2010. Her practice comprises the full spectrum of Family Medicine with an obstetrical practice that focuses on patient education and women’s health. She received her BA from the University of Florida in Gainesville, FL, graduated from Mayo Medical School and completed her Family Medicine residency training at Mayo Clinic in Kasson, Minnesota. Her research interests include understanding how to engage patients, improve their experience and healthcare delivery innovation. She is involved in the Knowledge Education Research (KER) unit at the Mayo Clinic to understand the impact of Minimally Disruptive Medicine and Shared Decision Making in practice.

MARK BAKER, MD, FACEP, is a full-time emergency medicine physician at Pali Momi Medical Center in Hawaii (Oahu). He has been at Pali Momi since 1989 where he currently serves as Medical Informatics Director and Chief of Staff. He is a Fellow with the American College of Emergency Physicians and is Board certified in Emergency Medicine and Clinical Informatics. He holds a BA in Biology from the University of California in San Diego, earned his MD from Vanderbilt University School of Medicine in Nashville, Tennessee, and was an Emergency Medicine resident at Harbor – UCLA Medical Center in Los Angeles.

HOWARD BECKMAN, MD, FACP, FAACH, is a Board certified internist and Clinical Professor of Medicine, Family Medicine and Public Health Sciences at the University of Rochester School of Medicine and Dentistry. He is Chief Medical Officer of EagleDream Health (EDH), a company that offers medical groups, ACOs, health plans, regional health collaboratives and employers assistance in using data to improve the value of care by identifying areas of overuse and underuse of services. EDH also help clients create respectful, non-judgmental programs in collaboration with practitioners to reduce that over- and underuse. EDH’s additional primary tools are providing evaluation and predictive modeling of a population’s health and real time dashboards for practitioners to improve patient management. Dr. Beckman’s particular expertise focuses on helping organizations successfully engage their practitioner network in these activities by focusing on appropriateness of care and quality improvement. Dr. Beckman also serves as a senior consultant to the Finger Lakes Health Systems Agency where he focuses on creating a community wide high blood pressure registry incorporating de-identified data from 9 counties, 95 practices and over 175,000 patients. Dr. Beckman’s work on promoting partnerships has been published widely in JAMA, the Annals of Internal Medicine, AcademicMedicine, the American Journal of Medical Quality, the Journal of Healthcare Management, and Health Affairs.

DAN BRINKMAN, RN, MPA, is CEO of the East Hawaii Region of Hawaii Health System Corporation (HHSC), which includes Hilo Medical Center, Ka’ū Hospital and Hale Ho’ola Hamakua. Mr. Brinkman joined Hilo Medical Center in 2007 and was Chief Operating Officer before being appointed CEO in 2014. Prior to moving to East Hawaii, he was Assistant Administrator of Interventional Cardiology and Cardiac Surgery at Vanderbilt University’s Medical Center in Nashville, Tennessee. He holds a BA from Augusta State University in Augusta, Georgia, and an MPA from the University of Colorado, Colorado Springs, Colorado.

DAVID D. DERAUF, MD, MPH, is Executive Director and a physician with Kokua Kalihi Valley Comprehensive Family Services (KKV), a Federally Qualified Health Center (FQHC) in Honolulu, Hawaii. He has been at KKV for the last 27 years and has been recognized for his leadership to help move the work of a FQHC upstream by organizing comprehensive community programs for all ages. Dr. Derauf earned his MD at the University of Minnesota Medical School in Minneapolis, Minnesota, was a resident in Family Practice there and in Preventive Medicine at the University of Hawaii. He earned his Masters of Public Health at University of Hawaii at Mānoa.

LAURIE HOPMAN, MD, is an independent family medicine physician in Hilo who has been practicing in Hawaii since 1988. She serves as the East Hawaii IPA (EHI IPA) representative on Community First’s Regional Health Improvement Collaborative’s Task Force to recruit and retain physicians in East Hawaii. Dr. Hopman is Board certified in Family Medicine and Hospice and Palliative Medicine. She serves as one of Hospice of Hilo’s physicians and is an EHI IPA Board member. Dr. Hopman served as a director and officer of the American Academy of Family Practice and remains actively involved in the organization. Dr. Hopman received an Excellence in Teaching Award as an assistant professor at the University of Hawaii John A. Burns School of Medicine. She earned her MD at the University Of New Mexico School Of Medicine and spent her residency at the Southern Colorado Family Medicine Residency in Pueblo, Colorado.

RANDY KUROHARA is Managing Director for the County of Hawai’i where he previously served as DeputyManaging Director from December 2012 to January 2016 and as Director of the Department of Research and Development from 2008 - 2012. A longtime Hawai‘i Island businessman, he is an owner of Creative Arts Hawai‘i, the Parker Ranch Store and Aloha Grown retail operations. He holds a BA in business administration from the University of Hawai‘i at Mānoa. Mr. Kurohara is a board member and past president of the Rotary Club of South Hilo, director and past president of the Japanese Chamber of Commerce and Industry of Hawai‘i, board member of the Hawai’i Island Chamber of Commerce, member of the Hawaii Health System Corporation’s East Hawai’i Regional Board, board member of the Kona Kohala Chamber of Commerce and Emeritus board member for the Boys and Girls Club of the Big Island.

ANNA LOENGARD, MD, is CMO and Executive Director of the Queens Clinically Integrated Physician Network (CIPN). Her greatest interest is developing solutions to improve quality and simplify systems of care for patient and physicians alike. She is currently board certified in geriatrics and palliative medicine, and serves as an Assistant Clinical Professor of Geriatrics at the University of Hawaii John A. Burns School of Medicine. Before joining Queen’s, she was St. Francis Healthcare System CMO, involved in hospice, home health and the rebuilding of their Liliha campus geriatric care center. Dr. Loengard moved to Hawaii in 2009 from New York City where she was on faculty at the Mount Sinai School of Medicine. Her teachings focused on the complexities of caring for the home-bound elderly and those with serious illness. Dr. Loengard has received many teaching awards, has been named Best Doctor in Hawaii for geriatrics and internal medicine since 2010 and received the HAH “physician of the year” Award in 2013.

GREGORY LONG, MD, is CMO and Senior Vice President, Systems of Care for ThedaCare. Dr. Long possesses an extensive record of progressively responsible leadership, innovation and improvement, including “Lean Transformation,” ambulatory EHR, computerized physician order entry and physician leadership and engagement. Dr. Long was responsible for leading ThedaCare’s open-access and office practice redesign effort for which ThedaCare Physicians received significant recognition. Those accomplishments resulted in being selected as faculty member for multiple national and international forums sponsored by the Institute for Healthcare Improvement. Dr. Long received his medical degree from Southern Illinois University School of Medicine in Springfield and completed his residency in family medicine at Memorial Hospital of South Bend.

ELIZABETH MITCHELL is President and CEO of the Network for Regional Healthcare Improvement (NRHI), a national network of multi-stakeholder Regional Health Improvement Collaboratives with over 35 members across the US. She is Vice Chair of the Physician Focused Payment Technical Advisory Committee, a Guiding Committee Member of the Health Care Payment Learning and Action Network (LAN), and on the Coordinating Committee of NQF’s Measure Application Partnership. Prior to leading NRHI, Ms. Mitchell was MaineHealth Management Coalition CEO, an employer-led, multi-stakeholder regional collaborative. Prior to this, she worked with employers at MaineHealth plus served two terms in the Maine State Legislature. She has served on the Board and Executive Committee of the National Quality Forum (NQF), was a member of the Institute of Medicine’s Consensus Committee on Core Metrics for Better Care and Lower Costs and served on the National Business Coalition on Health Board.

MARCI NIELSEN, PHD, MPH, is the President & CEO of the Patient-Centered Primary Care Collaborative (PCPCC). Dr. Nielsen’s previous positions include serving as Vice Chancellor for Public Affairs and Associate Professor at the University of Kansas School of Medicine; directing the State of Kansas Health Care Agency (Medicaid, CHIP, State Employee Health Plan); health lobbyist and assistant director of legislation for the American Federation of Labor and Congress of Industrial Organizations (AFL-CIO); and legislative assistant to US Senator Bob Kerrey (D-Nebraska). She is Board Director for the American Board of Family Medicine and the National Academy for State Health Policy, was a committee member for the Institute of Medicine’s Leading Health Indicators for Healthy People 2020 and Living Well with Chronic Illness: A Call for Public Health Action. She has served as a Peace Corps volunteer and in the US Army Reserves. She holds a PhD from Johns Hopkins School of Public Health, an MPH from George Washington University, and a BS in biology and psychology from Briar Cliff College.

JUDY MOHR PETERSON, PHD, is Administrator of the State of Hawaii’s Department of Human Services (DHS) MedQUEST Division (MQD), a position she has held since July 2015. Dr. Peterson is a nationally recognized leader in Medicaid, healthcare transformation and Affordable Care Act implementation. From 2009 - 2015 she served as Medicaid director for Oregon where she helped lead Oregon’s health system transformation and helped expand their Medicaid program, providing health coverage for an additional 250,000 people. During her tenure, local community and primary care increased through coordinated care organizations, which reduced emergency department visits and hospitalizations for chronic conditions for Medicaid clients. She worked for the state of Oregon for 18 years serving as Assistant Budget Director for the Department of Human Services, and managing Quality Improvement and Research and Data Analysis for Medicaid programs. She earned her PhD from the University of Texas in cultural anthropology.

BECCAH ROTHSCHILD, MPA, is the Senior Outreach Leader for the Choosing Wisely campaign at Consumer Reports. The Choosing Wisely campaign provides resources for consumers and physicians to engage in important conversations about the overuse or misuse of medical tests and procedures that provide little benefit and, in some cases, harm. Prior to joining Consumer Reports, Ms. Rothschild was the Director of Health Literacy Projects at Health Research for Action, at the University of California Berkeley School of Public Health. She also managed the California Health Literacy Initiative and served as the family literacy coordinator at the Second Start Adult Literacy Program in Oakland, California. Ms. Rothschild has worked for almost 20 years in the fields of adult and health literacy and health communication. This included leading health literacy partnership projects with private corporations, health plans, public-sector agencies, and non-profit groups. Ms. Rothschild has served on the boards of the Pharmacy Foundation of California, the Center for Disability and Health Policy, the Friends of the Oakland Public Library, and the Manatee Aquatic Masters swim team. She has master’s degree in public administration and non-profit management from San Francisco State University and a BA from the University of Wisconsin – Madison with majors in Spanish and Global Cultures.

MIKE SAYAMA, PHD, is Executive Director of Community First and VP at Pono Corporation. He has worked in the health care industry for over thirty years, including 17 years as Vice President for benefits management and customer relations at Hawaii Medical Service Association (HMSA). Dr. Sayama is a graduate of Yale University and the University of Michigan (PhD Clinical Psychology). His goal is to create a sustainable health care system through payment reform, information integration, care coordination, and community engagement. He is the author of Samadhi, Self-Development in Zen, Psychotherapy, and Swordsmanship andco-author of Focused Psychotherapy with Dr. Nick Cummings.

JACK SILVERSIN, DMD, DRPH, is a nationally and internationally recognized speaker on physicians and change and a regular speaker at Institute of Healthcare Improvement (IHI) conferences. He serves as a faculty member for Medical Group Management Association (MGMA), sponsored executive education programs and is content expert for the Minneapolis-based Institute for Clinical Systems Improvement’s (ICSI) collaborative on building a culture of quality. As a principal of Amicus, a healthcare consulting firm, he has helped organizations including group practices, hospitals, academic medical centers and systems in the US, UK and Canada create physician-organization compacts that support the desired vision. He has co-authored with Mary Jane Kornacki, A New Compact: Aligning Physician Organization Expectations to Transform Patient Careand Leading Physicians Through Change: How to Achieve and Sustain Results. Dr. Silversin is a graduate of the Harvard University School of Dental Medicine, holds a doctorate in public health from Harvard University and serves as a member of the Harvard University School of Dental Medicine faculty.

BARBARA SPIVAK, MD, is a practicing internist who has served as President and Chairperson of the Mount Auburn Cambridge Independent Practice Association (MACIPA) since 1997. She serves as a Mass Health Quality Partners (MHQP) Board member and was appointed to the AMA National Committee on Delivery System Reform in 2011. MACIPA is a physician membership organization with most of the physicians in small private practices, some owned by Mount Auburn Hospital or Cambridge Health Alliance. MACIPA provides support systems to help physicians manage population health, quality improvement, electronic health records and data exchange and analytics. Dr. Spivak graduated from Tufts University School of Medicine and completed her residency at St. Elizabeth’s Hospital. She is a Clinical Instructor in the Department of Medicine at Harvard Medical School.

BARRY TANIGUCHI is Chairman and CEO of KTA Super Stores and Chairman of Community First. Regarded as a key leader in Hawaii Island’s business and civic community, Mr. Taniguchi is committed to improving Hawai‘i Island’s health, wellness, and way of life. He currently serves as President for the Aloha Council of the Boy Scouts of America and the Hilo High School Foundation. He is a Director of American Savings Bank F.S.B., Hawaiian Electric Industries, Inc., Valley Isle Produce, Inc. and Hawaii Island Economic Development Board, and a Trustee of Hawaii Community Foundation, Public Schools of Hawaii Foundation, Tax Foundation of Hawaii, Queen’s Health Systems, Lyman House Memorial Museum and Pacific Tsunami Museum. Mr. Taniguchi holds a BA in Accounting from the University of Hawaii.

RACHEL WILLARD-GRACE, MPH, is Research Manager in the Center for Excellence in Primary Care (CEPC) at the University of California San Francisco. Her research tests new models of healthcare delivery. Among her current projects are several randomized controlled trials of health coaching in the primary care setting and an evaluation of primary care transformation across 20 sites in San Francisco. She has a Master in Public Health from the University of North Carolina Chapel Hill. Prior to joining the CEPC, Rachel was the executive director of a free clinic and worked as a public health researcher with an interest in organizational culture and change.
