Instructions1. Enter your name and date of birth on the label of the container.

2. Attach the Fe-Col® to the toilet seat.

3. Fill the container halfway with stool (Pic. 1).

Please be advised: not below the minimum, not exceeding the maximum.

Additional instructions: take a small amount of stool from 6 different places.

4. Screw the lid of the container shut (Pic. 1) and insert it into the RigidSafety-

Bag®. Insert the RigidSafetyBag® into the hard plastic case (Pic. 2).

The RigidSafetyBag® contains absorbent material which protects the container

during transport.

5. Remove the white RigidSafetyBag® release liner (Pic. 2) and seal the

RigidSafetyBag®.

6. Note the date of collection on the application form.

7. Please fi ll out the application form completely and make sure your

information is correct.

8. Insert the application form and the RigidSafetyBag® into the mailing envelope

and seal it. The envelope is now ready for shipment (Pic. 3 and 4).

Be advised: send us your stool from Monday to Thursday!

Gebrauchsanleitung1. Tragen Sie Ihren Namen und Ihr Geburtsdatum auf das Etikett der Stuhlröhre ein.

2. Befestigen Sie den Fe-Col® auf der Toilettenbrille.

3. Befüllen Sie die Stuhlröhre (Abbildung 1) nur innerhalb der min. und max.

Markierung mit Stuhl.

Zusätzliche Erklärung: Nehmen Sie bitte von sechs verschiedenen Stellen einen

kleinen Löffel Stuhl ab.

4. Schrauben Sie die befüllte Stuhlröhre zu (Abbildung 1) und geben Sie diese in

die Plastikhülle der RigidSafetyBag® (Abbildung 2).

In der RigidSafetyBag® befi ndet sich saugfähiges Material welches zum Schutz der

Stuhlröhre während des Transportes dient.

5. Entfernen Sie die weisse Folie der RigidSafetyBag® (Abbildung 2) und

drücken Sie die freiliegende Klebefl äche fest zusammen, so daß die Tasche

gut verschlossen ist.

6. Tragen Sie das Abnahmedatum der Stuhlprobe auf dem Auftragsformular ein.

7. Achten Sie darauf, daß Ihre persönlichen Daten korrekt und vollständig auf dem

Auftragsformular ausgefüllt sind.

8. Geben Sie das Auftragsformular und die RigidSafetyBag® in den Versan-

dumschlag und kleben Sie ihn zu. Der Versandumschlag kann nun verschickt

werden (Abbildung 3 und 4).

Achtung: die Stuhlprobe von Montag bis Mittwoch verschicken!

2. Fixer le Fe-Col® autour de la toilette.

3. Insérez les échantillons au tube de détente à moitié (Image 1).

Note: pas plus du maximum, pas inférieure au minimum.

Des explications supplémentaires: prenez une cuillère de selles

d’au-moins de 6 endroits différents.

4. Fermez le bouchon du tube de selles (Image 1) et insérez ce tube au

RigidSafetyBag® dans le porteur plastique (Image 2).

Le RigidSafetyBag® se contient de matériel absorbant qui protège le

tube de selles pendant le transport.

5. Retirer le plastique blanc de RigidSafetyBag® (Image 2).

Pousser sur l’adhésif jusqu’au le sachet soit bien fermée.

6. Indiquez la date d’où les selles ont été prélevées sur les formulaires

de laboratoires.

7. Vérifi er que vos informations se sont complétez accurate sur les

formulaires de laboratoires.

8. Insérer les formulaires de laboratoires et le RigidSafetyBag® aux

enveloppe d’envoi et fermez-les. L’enveloppe c’est prête d’envoyer

(Image 3 et 4).

Note: envoyer des échantillons de selles du lundi au mercredi!

1. Vul uw naam en geboortedatum in op het etiket van het ontlastingsbuisje.

2. Bevestig de Fe-Col® rond de toiletbril.

3. Vul het ontlastingsbuisje tenminste tot de helft (Afb. 1).

Let op: niet minder dan minimum, niet meer dan maximum.

Extra uitleg: neem van tenminste 6 verschillende plaatsen een klein

schepje van de ontlasting.

4. Schroef het ontlastingsbuisje dicht (Afb. 1) en stop dit buisje in de

RigidSafetyBag® in het hard plastic hoesje (Afb. 2).

De RigidSafetyBag® bevat absorberend materiaal dat het ontlastingsbuisje

beschermt tijdens transport.

5. Verwijder de witte folie van de RigidSafetyBag® (Afb. 2).

Druk de vrijgekomen plakstrip aan zodat het zakje goed is afgesloten.

6. Vul de afnamedatum in op het aanvraagformulier.

7. Controleer of uw gegevens juist en volledig zijn ingevuld op het

aanvraagformulier.

8. Doe het aanvraagformulier samen met de RigidSafetyBag® in de

verzendenvelop en plak deze dicht. De verzendenvelop is gereed voor

verzending (Afb. 3 en 4).

Let op: de ontlasting binnen Nederland versturen van maandag t/m

donderdag! Vanuit België versturen van maandag t/m woensdag.
