Evidence (Smell) of Urinating in the Building - Basement - Zone # 15:
This picture shows a unused room in the basement being used as a toilet.
Strong urine smell in the room. Urinating in the work area is unhygienic &
against site rules and regulations!
Have this issue discussed again in their daily TBT & if possible, close off this
room.

Unsafe Stair Access - Low Overhead Obstacles - No Warning Signs in-place -
Zone # 15 - Basement:
This picture shows there is No caution or warning signs displayed warning
workers there is a low overhead hazard (i.e. low cable tray ) above the access
point!
Have suitable overhead hazard caution signs displayed on this scaffolding!

Unsafe Practice - Concrete Repair Works with NO Dust Extraction Fans in place -
Zone # 15-Basement:
This picture shows HDEC workers are doing concrete repair works without a
safe dust extraction system in place! Increase risk of respiratory problems for
these workers!
This is UNSAFE and UNACCEPTABLE for this project!
Have this work stopped & install the safe dust extraction fan system that will
reduce the dust levels in this work area.

ESSW system not implemented in ALL Locations on-site - Zone # 09- Basement:
This picture shows the Lock out/Tag out (LOTO) system is not implemented at
every location in the basement. The ESSW system for Pl ant & Equipment to be
energized should be in place based on the MEP & HDEC instructions.
Have this room & all rooms in the basement provided with the necessary locks
& warning signs as per the ESSW procedure across the Project.

Poor Housekeeping - Unwanted material stored Under Stacked Fire panel
Cartons - Zone # 08 - Basement:
This picture shows a number of stacked cartons on shelves with stored
materials under these cartons creating an unsafe worker access to these
shelves.
Have this material separated and the stacked away from the shelves so that
workers can safely access the stacked Fire Panel cartons.
"A Neat and tidy site is a safe site"!

Safe Work Practice - PTW-CSP system in Place - Zone # 08 -Basement:
This picture shows the PTW-Confined Space work permit is in place & is
relevant for the work at hand. This is an example of a good work practice being
implemented across the Project.

Dust Extraction system Not Functioning Effectively - Basement- Zones # 06-08:
This picture shows dust extraction flexible ducting NOT connected properly;
NOT extending to outside the basement and it has numerous holes & tears. All
these imperfections in the ducting is resulting in the extraction system NOT
working effectively. What is the use of this safety control if the dust is recycledback into the same work area?
Have the ducting connected properly & tears repaired so that the dust
exhausting system will work efficiently & safely!

Poor Condition of Dust Extraction Flexible Ducting - Basement- Zone # 08:
This picture shows there are a lot of holes in the dust extraction flexible ducting
being used to extract dust from a concrete repairs and other work area!
Have this hose repaired properly so that the dust exhausting system will work
efficiently & safely!

Unsafe Condition - Open DB panel Cover - Zone # 06-Basement:
This picture shows an electrical DB panel door is open and it would appear its
lock mechanism is broken. An increased risk of electric shock or misuse of this
panel by unauthorized workers!
Have the DB panel door checked by an authorized person IMMEDIATELY!

lighting! This work practice is NOT ACCEPTABLE & is UNSAFE.
Have sufficient task lighting made available for this worker.

Unsafe Scaffolding - Scaf-tag Indicate The Scaffold is Not Regularly Inspected -
Zone # 03-Basement:
The picture shows scaffolding being used displaying an "out-of-date" scaf-tag.
WHY? Every scaffold used on-site must be inspected and renewed on weekly
basis by a competent person without fail!
Have this scaffold structure inspected IMMEDIATELY and the Scaf-tag updated.

This picture shows a number sub-con workers doing painting works inside a
closed room with No Forced ventilation or pedestal fans to provide some
degree of fresh air.
Have this work area provided with fans to increase the refresh air flow through
the closed room.

Corridor:
This picture shows NO Exit or directional signs provided in the main corridor sothat people know where they need to exit out of the basement, especially in an
emergency situation.
Have this area provided proper directional or Exit arrow signs ASAP.

No Hand gloves used for chipping works - Zone # 05 - Basement:
This picture shows a worker is chipping with "home made" hand tool and
hammer without using proper hand protection. An increased risk of
hitting/slipping by hammer on the worker's hand.
Have this worker instructed to use proper chisels and wear hand gloves while
working.

Unsafe Guardrail system Used for Excavation Edge - Zone # 04-near the Live
road:
This picture shows there are No concrete barriers provided on an excavation
edge next to the live main access road. The section of hand rail is not secured
and can easily be pushed over by workers or passing vehicles.
Have this GI pipe connected to some concrete barriers along the excavation
edge next to the main access road.

No Warning Signs and Flash lights provided for Deep Excavation Next to LIVE
Main Access Road - Zone # 08-Lagoon Area:This picture shows there are NO warning si gns or Flashing roadside lights
provided on the water barriers near the LIVE road. Best practice would be to
use concrete barriers along this section of road.
Have this excavation area provided appropriate warning signs and Flashing
roadside lights fixed as required. If possible replace the W/F barriers with
concrete barriers.
