Dear Parent/Legal Guardian of a Connections Academy Kindergarten Student,

We hope you have had a successful school year and we thank you for being a part of Connections Academy.

Due to increased enrollment of Kindergarten students for the 2012-2013 school year, we are asking for your

assistance in ensuring new Kindergarten students have all of the materials they need to be successful for theupcoming school year by returning certain items from your students Kindergarten curriculum.

We have provided a list of the items we would like to get back. If you have any of these items in re-usablecondition, please follow the return instructions below and use the enclosed prepaid UPS shipping label.

IMPORTANT POINTS: You will receive one pre-paid UPS shipping label for each Kindergarten student in your household to

return the curriculum materials.

If you have Connections Academy-issued computers, which need to be returned, you will receive anadditional set of instructions and UPS shipping labels for the return of the computer equipment.

Please contact us at 800-382-6010 option 1, ext. 2308, if your address or phone number has changedor if you have any questions regarding the enclosed instructions.

EACH Student Is Requested To Return The Specified Kindergarten Materials AtThe End Of The School Year.Please follow the instructions below to ensure a fast and accurate return:

1. Review the list of Kindergarten Materials to Return on the following page to determine which materialsneed to be returned.

2. If you have more than one Kindergarten student, use a separate box for each student. Securely packall items so they will not shift during shipping using old newspaper or packing materials.

3. Securely tape the box shut with packing tape and tape over any holes in the box.

4. Peel the backing off of the UPS label and place it on the center of the box for the correct student.5. Locate your local UPS center using one of the following methods:a. Go to UPS.com. After you select your location, click on the Find Locations option under the

Quick Links menu. Then, follow the onscreen directions.b. Call 1-800-PICK-UPS for a location near you or to inquire about a pick-up. There may be a

charge for the pick-up service.c. Call us at 800-382-6010 option 1, ext 2308, and we will tell you the closest UPS location to your

home.6. Drop the box off at your local UPS center. Most UPS shipping outlets are open from 8 am 7 pm. Please

check your local center for its hours of operation.

KEY POINTS: Make sure to use the UPS labels that are marked EOY 1112 Curriculum for these returns. Make sure each students materials are packed in separate boxes if you have more than one student. Make sure to use the correct student label on that particular students box of curriculum materials. Make sure to cross out or cover any old barcode labels or previous address labels. If you have received labels and you do not have any materials to return, please discard the labels.
