prone position blood flow is greatest in the dorsal (non-dependent) parts demonstrating the importance of the arterial tree

• Total ventilation may increase by a more homogenous pleural pressure and by elimi-nating the weight of the heart

ciprofloxacine

Laatste  opname  ruim  2  maanden  terug  

1.  Bilaterale  pneumonie  met  pseudomonas  aeruginosa.  Differentiaal  diagnostisch  bij  hypogammaglobulinemie  in  combinatie  met  bronchiëctasieën  en  CLL.    Onderhoudende  factoren:  roken.  

 2.  Licht  obstructief  gestoorde  longfunctie,  waarschijnlijk  passende  bij  een  onderliggend  COPD  Gold  I.

1.  Koorts,  hoge  infectieparameters  bij  luchtweginfecties/pneumonie  dd/  pseudomonasinfectie  

2.  Nierinsufficientie,  wsch  prerenaal  bij  dehydratie  bij  3.    3.  Diarree  dd/  AB,  infectieus  

Behandeld  met  start  Tazocin  en  Colistine  vernevelingen

IC  opname  Opnieuw  resp  insuff,  pneumonie  /  atelectase?        Piptazo.  Scopie:  pus,  geen  bloed,  alles  open.  

 

Na  1  dag  Beademing  slechter.  Koorts.  Meronem.      X-­‐thorax:  dbz  infiltraten  wrs  oedeem

   Start  NO-­‐beademing.  Ontwateren.  ECMO?  Na  1  dag  Verslapping.  Peep  18.  Stop  NO.    Na  1  dag  Weer  op  buik  met  goed  resultaat,  pO2  9,8  à  14,6.  Na  2  dagen  Stop  verslapping.  Na  2  dagen  Terug  naar  rug.  Influenza  A.  Oseltamivir.      Daarna  langzaam  acouwen  beademingsvoorwaarden  met  verbetering  van  X-­‐thorax.      

Plan:  Na  verder  acouwen  PEEP  PDT  plaatsen?      Verder  ontwateren  +  vrij  water  geven  ivm  hyperNa.
